Delirium and Coronavirus Disease 2019: Looking Back, Moving Forward.

During the coronavirus disease 2019 pandemic, crisis changes in clinical care increased rates of delirium in the intensive care unit (ICU). Deep sedation, unfamiliar environments with visitor restrictions, and such factors due to high workload and health system strain contributed to the occurrence of delirium doubling in the ICU. As the pandemic wanes, health care systems and ICU leadership must emphasize post-pandemic recovery, integrating lessons learned about delirium management, evidence-based care, and family involvement. Strategies to empower clinicians, creatively deliver care, and inte…

Transrectal and transvaginal abscess drainage.

The TR and TV approaches to deep pelvic abscesses have been made safe and easy by improvements in endoluminal US technology. Most procedures take well under an hour, and standard intravenous sedation is usually sufficient for patient comfort. The simplest and safest technique employs a combination of endoluminal US, with biopsy guides for precision needle advancement, and fluoroscopy for dilating the tract and placing a drainage catheter. Patient acceptance of TR and TV catheters is high, and resolution can typically be expected within 3-5 days. In the majority of cases, catheter treatment, co

Relative hypoxemia during rhytidoplasty.

The results of blood gas analyses in 24 patients who had a rhytidoplasty under local anesthesia indicated that a moderate degree of hypoxemia can be elicited in these patients when they have had standard doses of the usual sedatives. While the acid-base abnormalities were generally corrected spontaneously, the extent of the hypoxemia can be aggravated further by the additional use of diazepam during the operation. Therefore, over-sedation of such a patient during the operation, without a secured airway, must be avoided. Additionally, we recommend deep breathing at frequent intervals, with or w

Opioid and benzodiazepine requirements in neonatal and pediatric patients undergoing venoarterial ECMO: insights from a predominantly cardiac cohort.

BACKGROUND: Managing sedation and analgesia in neonates and pediatric patients supported with extracorporeal membrane oxygenation (ECMO) is challenging due to altered pharmacokinetics, prolonged critical illness, and the need for deep sedation to maintain circuit integrity. Limited data describing cumulative opioid and benzodiazepine exposure and factors associated with dose escalation in pediatric ECMO populations. METHODS: We conducted a retrospective cohort study of neonates and pediatric patients aged ≤14 years who received ≥72 h of venoarterial ECMO support in a pediatric cardiac surgica…

Comparison of time-series models for predicting physiological metrics under sedation.

This study presents a comprehensive comparison of multiple time-series models applied to physiological metric predictions. It aims to explore the effectiveness of both statistical prediction models and pharmacokinetic-pharmacodynamic prediction model and modern deep learning approaches. Specifically, the study focuses on predicting the bispectral index (BIS), a vital metric in anesthesia used to assess the depth of sedation during surgery, using datasets collected from real-life surgeries. The goal is to evaluate and compare model performance considering both univariate and multivariate scheme

The early diagnosis and management of mixed delirium in a patient placed on ECMO and with difficult sedation: A case report.

Delirium represents a serious problem that impacts the physical and cognitive prognosis of patients admitted to intensive care units and requires prompt diagnosis and management. This article describes the case and progress of a patient placed on Extracorporeal Membrane Oxygenation with difficult sedation criteria and an early diagnosis of mixed delirium. During the case report, we reflect on the pharmacological and non-pharmacological strategies employed to cope with delirium paying special attention to the non-use of physical restraint measures in order to preserve vital support devices (end

Intraoperative dexmedetomidine sedation reduces the postoperative agitated behavior in elderly patients undergoing orthopedic surgery compared to the propofol sedation.

BACKGROUND: Postoperative agitation or confusion is one of the symptoms of hyperactive delirium in elderly patients. We retrospectively evaluated the incidence of postoperative abnormal psychomotor behavior in elderly surgical patients according to the use of different intraoperative sedative agents: dexmedetomidine vs. propofol. METHODS: The medical records of 855 elderly patients, who underwent orthopedic surgery with regional anesthesia between July 2012 and September 2015, were divided into two groups, the dexmedetomidine group (N.=263) and the propofol group (N.=592), and then evaluated.

Sedation management during therapeutic hypothermia for neonatal encephalopathy: atropine premedication for endotracheal intubation causes a prolonged increase in heart rate.

AIM: Heart rate (HR) plays an important role in the assessment of stress during therapeutic hypothermia (TH) for neonatal encephalopathy; we aimed to quantify the effect on HR of endotracheal (ET) intubation and drugs given to facilitate it. If atropine premedication independently increased HR, the main indicator of effective sedation, we hypothesised that increased sedation would have been given. METHODS: Thirty-two, term, neonates recruited into a randomised pilot study comparing TH and TH combined with 50% Xenon inhalation were studied. Indications for ET intubation included: resuscitation

Procedural sedation and analgesia for reduction of distal forearm fractures in the paediatric emergency department: a clinical survey.

OBJECTIVE: Distal forearm fractures frequently require reduction in children. We set out to survey how such fractures are currently reduced at Paediatric Research in Emergency Departments International Collaborative (PREDICT) sites. METHODS: A survey was completed by paediatric emergency physicians at PREDICT sites. Survey questions covered departmental guidelines and resources and individual practice, agents used and limitations of fracture management using case vignettes. RESULTS: One hundred eleven of 145 (77%) possible surveys were returned. All 12 PREDICT sites have guidelines for the u

Anti-emetic effect of midazolam added to morphine patient-controlled analgesia after total abdominal hysterectomy.

Nausea and vomiting are frequent adverse effects of patient-controlled analgesia with opioids. This study was designed to compare the effect of midazolam to that of ondansetron for prevention of nausea and vomiting during morphine patient-controlled analgesia. In a randomised, double-blind, prospective trial, 90 patients were allocated to one of three groups of 30 each, to receive one of three patient-controlled analgesia regimens to manage postoperative pain: a combination of midazolam and morphine (group M), a mixture of ondansetron and morphine (group O) or morphine alone (group C). Anesthetic Considerations for Patients With Hypokalemic Periodic Paralysis Undergoing Ambulatory Surgery: A Case Report.

The perioperative management of patients with hypokalemic periodic paralysis is challenging due to the risk of postoperative muscle weakness. We describe a patient with hypokalemic periodic paralysis who presented for wisdom teeth extraction. Our perioperative goals included minimizing exposure to weakness triggers such as hypokalemia, pain, anxiety, hypothermia, and neuromuscular blockade. The patient tolerated the procedure under deep sedation with intravenous agents, local anesthesia, and a natural airway with spontaneous ventilation. He experienced mild upper extremity weakness postoperati…

Emergent Airway Management of an Uncooperative Child with a Large Retropharyngeal and Posterior Mediastinal Abscess.

Retropharyngeal abscesses are deep neck space infections that can lead to life-threatening airway emergencies and other catastrophic complications. Retropharyngeal abscesses demand prompt diagnosis and early establishment of a definitive airway when there is airway compromise. This can be difficult in an uncooperative patient. We present the case of a 12-year-old girl with mediastinitis and tracheal compression and anterior displacement from a large retropharyngeal and posterior mediastinal abscess secondary to traumatic esophageal perforation, who received successful awake nasal fiberoptic in

A case of a short-coupled variant of torsades de Pointes with electrical storm.

This case report describes a short-coupled variant of Torsades de Pointes with a characteristic ECG pattern consisting of a prominent J wave in leads V3-V6, in which an electrical storm was evoked with autonomic receptor stimulation and a blockade test. The patient's frequent VF attacks were triggered by short-coupled premature ventricular contractions with a right bundle branch block morphology and left-axis deviation, and were suppressed by deep sedation followed by a combination therapy using verapamil and mexiletine. Interestingly, with the use of those drugs, the prominent J wave diminish…

Long-time microdialysis in a patient with meningoencephalitis.

As part of invasive neuromonitoring, long-time microdialysis was performed in a 32-year-old patient suffering from meningoencephalitis. Cerebral magnetic resonance imaging (MRI) revealed marked global brain oedema. An intracranial pressure (ICP) probe, brain tissue oxygen pressure measurement (pTiO(2)), and intraparenchymal microdialysis were used for intensive neuromonitoring. Despite high ICP, only intensive care therapy was applied for 16 days using deep propofol sedation, hyperventilation, and intermittent mannitol therapy. One year later, the condition of the patient was excellent. Intrac

The graded effect of propofol in electrophysiology-guided navigation during deep brain stimulation surgery.

Propofol is widely used for general anesthesia (GA) during deep brain stimulation (DBS) surgery targeting the subthalamic nucleus (STN) in Parkinson's disease (PD), yet its effects on intra-operative spatial navigation, critical for electrode placement, remain contentious. We performed multimodal analysis on 583 microelectrode recordings (MER) from PD patients undergoing DBS surgery under local anesthesia (LA) and GA. Deep sedation interfered with the identification of the dorsal STN border, and propofol dosages >4 mg/kg/h resulted in deeper final electrodes. While firing rate (FR) and burst i…
