OBJECTIVE: To investigate the long-term outcome and changes of the personality and psychopathological profile of opiate addicts after bilateral stereotactic nucleus accumbens (NAc) ablative surgery.
METHODS: 60 patients were followed up for 5 years and abstinent status and adverse events were evaluated. NAc lesion volumes and locations were obtained by postoperative MRI scans. The Chinese version of the Eysenck Personality Questionnaire (EPQ-RSC), the Symptom Checklist-90-Revised (SCL-90-R), the Beck Depression Inventory (BDI), the Yale-Brown Obsessive Compulsive Scale (Y-BOCS) and the World

A sample of 535 entrants to opioid dependence treatments across three treatment modalities were administered a structured interview to ascertain the prevalence of non-injecting heroin use. Ten per cent of participants had used heroin primarily by smoking/inhaling in the month preceding interview, and 9% had used heroin and other drugs exclusively by non-injecting routes. Non-injectors were younger (25.3 vs. 29.5 years), had higher levels of education (10.6 vs. 10.0 years), were more likely to be employed (33 vs. 18%) and had lower levels of recent crime (31 vs. 56%). They also had shorter hero

This study examined (a) associations of progression to daily cigarette smoking with lifetime psychopathology, regular smoking by family members, and psychopathology in family members, (b) the degree to which disorders precede or follow progression to daily smoking, and (c) whether daily smokers differ as a function of nicotine dependence. Among 941 participants interviewed at three time points, 722 (77%) had experimented with cigarette smoking, 391 of whom (54%) progressed to daily smoking. Lifetime psychiatric diagnoses were obtained at each assessment. Biological parents and full siblings we

In 189 opioid-dependent subjects on methadone maintenance treatment in Barcelona (Spain), we assessed the prevalence of co-occurrence substance use and non-substance-use disorders (dual diagnosis) by the Psychiatric Research Interview for Substance and Mental Disorders [PRISM], and the impact on quality of life (HRQoL) by the SF-12 (PCS-12 and MCS-12 scales). Rates of substance and non-substance-use diagnoses were 59% and 32%, respectively. Mean scores for PCS-12 and MCS-12 were 44.1+/-10.1 and 39.9+/-11.7, without differences by presence or absence of dual diagnosis. Heroin users on methadone

Baseline Addiction Severity Index (5th ed.; ASI-5) data of 2,142 substance abuse patients were analyzed with two nonparametric item response theory (NIRT) methods: Mokken scaling and conditional covariance techniques. Nine reliable and dimensionally homogeneous Recent Problem indexes emerged in the ASI-5's seven areas, including two each in the Employment/Support and Family/Social Relationships areas. Lifetime Problem indexes were derived for five of the areas--Medical, Drug, Alcohol, Legal, and Psychiatric--but not for the Employment/Support and Family/Social Relationships areas. Correlationa

BACKGROUND: Studies have implicated a wide variety of variables as being associated with an early age of first drink (AFD). AFD in turn has been associated with a variety of negative outcomes in adolescence and early adulthood. This study is designed to quantify the contributions of these antecedent variables to prediction of AFD; in particular it will carefully examine the involvement of variables in four areas (child characteristics, family demographics, family psychopathology, and child behavior problems).
METHODS: Using data from a multicenter study on alcoholism, we first investigated th

BACKGROUND: Intense and rapidly changing mood states are a major feature of borderline personality disorder (BPD); however, there have only been a few studies investigating affective processing in BPD, and in particular no neurofunctional correlates of abnormal emotional processing have been identified so far.
METHODS: Six female BPD patients without additional major psychiatric disorder and six age-matched female control subjects underwent functional magnetic resonance imaging (fMRI) to measure regional cerebral hemodynamic changes following brain activity when viewing 12 standardized emotio

This article considers self and self-concept in bipolar disorder. Bipolar disorder, defined on the basis of manic symptoms, is a highly debilitating psychopathology. It is heavily grounded in biology but symptom course is still very responsive to psychological and social forces in the lives of persons who have the disorder. This review assumes an overall view of the self that is typical of personality psychology: self as traits, self as goals and aspirations, and ongoing efforts to attain those goals. In this review, we will discuss two different facets of self and identity in bipolar disorder

BACKGROUND AND OBJECTIVES: Personality change is an important psychiatric complication following stroke linked to severe affective dysregulation and behavioral alterations.
METHODS: We investigated personality traits in 20 patients (age 45.37 ± 13.41 years) with subacute stroke submitted to rehabilitation training within 1-3 months after a first-onset stroke. All patients underwent psychological evaluation by using the Personality Inventory for DSM-5 for adults (PID-5), a specific instrument that enables traits (dimensions and facets) to be assessed by providing a personality profile, and the

A person's impulsive actions are characterized by a lack of sufficient planning or an assessment of possibly harmful consequences. An impulse control disorder can be diagnosed if such actions occur frequently, even in low-stress situations. In psychiatry there are a number of syndromes that are associated with an impulse control disorder. Neurobiological findings point to a reduced turnover of serotonin in the brain of persons with an impulse control disorder and aggressive behaviour. However, a reduced turnover of serotonin is also found in persons who can consume immoderate amounts of alcoho

CONTEXT: Epidemiologic information is important to inform etiological research and service delivery planning. However, current information on the epidemiology of alcohol use disorders in the United States is lacking.
OBJECTIVES: To present nationally representative findings on the prevalence, correlates, psychiatric comorbidity, and treatment of DSM-IV alcohol abuse and dependence.
DESIGN, SETTING, AND PARTICIPANTS: Face-to-face interviews with a representative US adult sample (N = 43 093).
MAIN OUTCOME MEASURES: Lifetime and 12-month DSM-IV alcohol abuse and dependence.
RESULTS: Prevalenc

Twenty-eight dysthymic patients (82.1% with personality disorders) were investigated with questionnaires for personality and depression before and after treatment. When in asymptomatic state, defined by clinical criteria and HAM-D score 6 or lower, the personality profile of 15 patients (group I) was significantly changed from the one before treatment (avoidant, passive-aggressive, borderline and schizotypal dimensions were lower, and narcissistic dimension higher [P < 0.01]). Thirteen patients (group II) had an unchanged profile. The first group showed significant state-trait dependence, espe

Behavioral disinhibition is a manifestation of impulsive behavior that is prominent in the psychopathology of various psychiatric disorders such as addiction, attention-deficit hyperactivity disorder, mania, and personality disorders. Impulsivity may be studied by measuring anticipatory responses made before the presentation of a food-predictive, brief light stimulus in a two-choice serial reaction time task. In such serial reaction time tasks, amphetamine has been shown to produce dose-dependent increases in premature responding in a manner dependent on dopamine D(2)-like receptor stimulation

Phobic disorders and other anxiety disorders belong to the most frequent psychiatric illnesses. Since they can be overlapped by other mental illnesses, they are often overlooked in practice. For example, comorbidity between anxiety and depression is very high. Also, personality disorders and a person's biography can lay the ground to anxiety. A typical complication of anxiety disorders is dependence of substances, predominantly of tranquilizers and alcohol. By means of a clinical case example, the topic of anxiety disorders is discussed practically and the treatment concept is detailed. Alread

This study, based on a nationally representative, epidemiologic sample (N = 43,093, response rate 81%), compared sociodemographic and family history correlates, antisocial personality disorder (ASPD) symptom patterns, and Axis I and Axis II comorbidity, among adults with DSM-IV ASPD who reported onset of conduct disorder (CD) in childhood (<age 10) versus adolescence (> or =age 10). Prevalence of each ASPD diagnostic criterion and comorbid lifetime disorder was estimated. Logistic regression was used to examine associations of childhood-onset CD with ASPD symptom patterns and comorbid disorder

OBJECTIVE: Sleep problems are a clinical and/or diagnostic feature for a broad array of mood, substance use, and anxiety disorders, including posttraumatic stress disorder (PTSD). Previous research by Leskin et al (Leskin GA, Woodward SH, Young HE, Sheikh J. Effects of comorbid diagnoses on sleep disturbance in PTSD. J Psychiat Res 2002;36:449-452) using the baseline National Comorbidity Survey (NCS) data found that persons with PTSD and panic disorder had a greater proportion of sleep problems than persons with other comorbid disorders. The current study extends Leskin et al's findings using

This longitudinal study investigated the scope and course of attention problems over a period of time from preteen (ages 7-12 years) to early teen years (ages 13-17 years). We compared symptoms in subjects with and without a family history (FH) of alcohol abuse or dependence from among families without evidence of antisocial personality disorder. Evaluations of attention problems for the offspring were based on the Child Behavior Checklist and a validated semistructured interview carried out with the mother. The findings indicate no higher risk for attention problems and attention-deficit hype
