Scrotal pyoderma gangrenosum associated with dermatomyositis.

Pyoderma gangrenosum (PG) of the scrotum is extremely rare. We report a case of scrotal PG associated with dermatomyositis. Initial healing was slow despite the use of oral prednisolone and azathioprine. Healing was accelerated by the local use of a potent topical corticosteroid. PG should be considered in the differential diagnosis of ulcerating lesions of the genitalia.

von Willebrand factor in juvenile dermatomyositis.

OBJECTIVE: To evaluate whether plasma von Willebrand factor (vWF) levels are a useful indicator of disease activity in juvenile dermatomyositis (DM). METHODS: Serum vWF levels were prospectively measured in 15 patients with juvenile DM and were compared to serum muscle enzyme levels, muscle strength, and presence of extramuscular manifestations. RESULTS: 6/15 active disease periods were accompanied by an increase in vWF; 9 were not. Elevated vWF levels did not relate to the presence of active skin disease or calcinosis. vWF was not consistently related to muscle strength, CPK, or aldolase in…

Juvenile dermatomyositis: a Roy nursing perspective.

With current medical treatment, juvenile dermatomyositis (JDMS) is rarely fatal. However, for most families, it is a chronic illness that interrupts life for both the affected child and the rest of the family. This article provides an overview of the disease, presents a case history of a 9-year-old affected girl, and discusses nursing care using the Roy Adaptation Model and nursing diagnoses.

Juvenile dermatomyositis associated with partial lipoatrophy.

A 26-year-old woman developed partial lipoatrophy 12 years after juvenile dermatomyositis was diagnosed. Renal function was normal, but she had other features typically associated with partial lipoatrophy, including hepatomegaly, acanthosis nigricans, hypertrichosis, and hyperinsulinemia.

Polymyositis-dermatomyositis and non-Hodgkin's lymphoma.

An association between polymyositis-dermatomyositis (PM/DM) and malignancy has been reported. However, controversy exists regarding a direct causal relationship between the two diseases. We describe a patient who had had PM and subsequently developed non-Hodgkin's lymphoma, and discuss the possibility of their causal relation.

[Juvenile dermatomyositis. Clinical and epidemiologic experience].

27 cases with a diagnosis of Juvenile Dermatomyositis were studied from a total of 1307 patients who suffered from connective tissue diseases; 19 of which met the diagnosis criteria of Bohan and Peters. Females were the most frequently affected. 52.63% of the cases presented the first symptoms between 5 and 9 years of age. Skin lesions, debility in inferior extremities and fever were the most frequent motives of consultation. The Aldolase and LDH were the muscular enzymes whose values increased in the majority of the cases. The electromyography was more sensible than the muscular biopsy in the…

Polymyositis/dermatomyositis associated with dermatitis herpetiformis.

We describe 2 patients with dermatitis herpetiformis who developed polymyositis/dermatomyositis. On HLA typing, both patients were found to be HLA-B8, DR3 positive. The concurrence of these two relatively rare diseases, both associated with immunologic abnormalities, further supports the role of autoimmunity in their pathogenesis and indicates a possible common genetic basis. It also suggests that myositis may be more common in patients with dermatitis herpetiformis than in the general population.

C4 complement allotypes in juvenile dermatomyositis.

Twenty probands with juvenile dermatomyositis and their relatives were studied to determine the inherited segregation patterns of class I, II, and III HLA region markers including C4A, C4B, Bf, and C2 complement polymorphisms. The extended haplotype B8, DR3, C4A*Q0, C4B*1, C2*C, and Bf*S was present in 13 of the 20 probands. Three other probands also carried a haplotype with a null allele for C4A and two further probands carried a null allele for C4B; only two probands had no detectable C4 null allele. These data confirm previous studies showing high frequencies of B8 and DR3 in patients with …

[Plasma separation in severe juvenile dermatomyositis].

Because a seven year old boy with severe dermatomyositis failed to respond to a six week treatment course with prednisone (60 mg/m2) and azathioprine (2 mg/kg/d), five plasma separations were performed. Already after the second plasma separation an impressive reduction of skin ulcerations and an increase of muscle strength were noticed. So far there has been no relapse in spite of rapid reduction in the prednisone dosage.

Recurrent photosensitive dermatitis preceding juvenile dermatomyositis.

The classic features of childhood dermatomyositis include muscle weakness, elevated muscle enzymes, and characteristic abnormal muscle biopsy and electromyography. Paramount to the diagnosis are cutaneous dermatoses that include a heliotrope rash and Gottron's papules. Rarely, a photo-sensitive dermatosis may occur. A recurrent photoexacerbated dermatoses can be an initial sign of occult childhood dermatomyositis.

[Pediatric dermatomyositis. Apropos of 28 cases].

The authors report 28 cases of juvenile dermatomyositis. Bohan and Peter criteria were fulfilled in all cases. All children received a treatment with prednisone as soon as diagnosis was established. Five patients (18%) died during this treatment. The remaining children were followed for at least 2 years after prednisone treatment was discontinued. An acute onset was found in 4 of the 5 children who died and in 6 of 23 children still alive. Swallowing disorders were present in all children who died and in 11 of the children still alive. Ten children recovered fully and 13 presented with sequell…

Gynecologic carcinoma associated with dermatomyositis-polymyositis.

Dermatomyositis-polymyositis was associated with gynecologic carcinoma in ten patients treated at the Mayo Clinic between 1952 and 1982. Only two patients did not manifest the typical skin changes of acute dermatomyositis. In all ten cases, the onset of dermatomyositis-polymyositis preceded evidence of carcinoma. The intervals between initial symptoms of dermatomyositis-polymyositis and subsequent diagnosis of carcinoma ranged from three months to six years. Usually, the carcinoma became evident within two years. The carcinomas were ovarian in five cases, cervical in three, endometrial in one,…

Cutaneous vasculitis in adult polymyositis/dermatomyositis.

Seven (9.2%) of 76 patients with adult-onset polymyositis/dermatomyositis (PM/DM) seen over an 11-year period had cutaneous vasculitis. This was manifest by dermal and/or subcutaneous nodules in 4, periungual infarcts in 3 and digital ulceration in 2. When these 7 patients were compared to the remaining 69, a significant association was noted between cutaneous vasculitis and DM (p = .025); only 1 of 31 patients with primary PM and none of 18 with overlap syndromes had vasculitis. Furthermore, 2 (28.6%) of those with vasculitis had an associated malignancy compared to only 4 (5.8%) of those wit…

Penicillamine-induced dermatomyositis. A case history.

A 69-year-old woman with classical rheumatoid arthritis developed a severe dermato-myopathy during treatment with penicillamine. Remission o … side effects have been well reviewed by others (1, 2). To our knowledge only a few earlier cases of dermatomyositis as a complication to penicillamine treatment of rheumatoid arthritis have been reported (3, 4, 5). We describe here another case.

Steroid-induced acanthosis nigricans in dermatomyositis.

Acanthosis nigricans of the groin, axillae, and neck developed in a patient on long-term steroid therapy for dermatomyositis after eight years. The dermatosis resolved spontaneously ten years after the steroid use had been discontinued. Review of three personal cases and the literature suggests that acanthosis nigricans may have a hormonal basis.
