[Diagnosis and therapy of cecal diverticulitis].

Acute pain in the lower right side of the abdomen caused by caecal diverticulitis is very rare and hardly to distinguish from acute appendicitis. Therefore it is usually diagnosed intraoperatively. We report our experience with 12 patients, who received surgical treatment from 1984 to 1991. The diagnosis was made not before operation in 11 cases. Because of a severe inflammation of the caecal wall we performed an ileocaecal resection in 10 cases, in 2 cases it was necessary to perform a right hemicolectomy because of additional diverticulitis of the ascending colon. The surgical resection of t

CT of diverticulitis. Diagnosis and treatment.

Diverticulitis can be detected accurately by computed tomography (CT) and staged according to an established system that is meaningful to both radiologists and surgeons. Diverticulitis limited to the wall of the sigmoid colon or small diverticular abscesses contained within the pericolic mesentery generally respond well to antibiotic therapy; larger abscesses must be drained. CT-guided catheter drainage is an effective method to treat these large diverticular abscesses. Overall management is simplified since one-stage sigmoid resection can be performed electively on a nonseptic patient. High-r

An Unusual Presentation of Acute Appendicitis Mimicking Sigmoid Diverticulitis: A Case Report.

Acute appendicitis typically causes right lower quadrant pain, but in elderly patients with comorbidities, it can present atypically, complicating diagnosis. This case highlights a rare presentation, mimicking sigmoid diverticulitis. A 70-year-old man with chronic heart failure, arrhythmia, and renal failure presented with two days of left lower quadrant pain. Examination showed tenderness, rebound, and guarding. Laboratory tests revealed a white blood cell count of 12,000/µL and creatinine of 2.5 mg/dL. C-reactive protein was elevated at 25 mg/L. Computed tomography revealed an inflamed appen

[Conservative treatment of simple, uncomplicated diverticulitis. Preliminary study for evaluating the effectiveness of a method of topical therapy: rectal perfusion].

Transit times and distribution modalities of an oily mixture used as a vehicle for drugs administered by rectal perfusion in topical therapy in patients with uncomplicated diverticulitis of the colon have been examined. X-ray study makes it possible to state that the technique is mainly indicated in descending colon and sigmoid localisations, segments in which the oily mixture distributes homogeneously and massively in all patients treated.

[Fallopian tube in colonic anastomosis after sigmoid resection].

In this case report, we present a rare case in which biopsies from a malignancy-suspect anastomosis after sigmoid resection revealed fimbrial tissue from the fallopian tube. The patient had severe diverticulitis with abscess and underwent resection with primary anastomosis. Persistent atypical endoscopic findings led to repeated biopsies, eventually showing tubal epithelium without malignancy, likely due to fimbrial adherence and incorporation into the anastomosis.

Oxidative imbalance and muscular alterations in diverticular disease.

BACKGROUND: It is still a matter of debate if neuromuscular alterations reflect a primary event in diverticular disease (DD). AIMS: This study aimed to assess colonic wall layers from both stenotic and non-stenotic complicated DD, bio-phenotypic alterations, inflammatory and oxidative status. METHODS: A systematic analysis of colonic specimens obtained from stenotic and non-stenotic DD specimens was conducted and compared with controls. Biological activity and qPCR analysis were performed on longitudinal and circular muscles. Western blot analysis was performed throughout colonic wall layers

[Diverticulosis. Anatomical aspects].

Sigmoid diverticula are pseudodiverticula, protrusions of the mucosa between the muscularis, so that they pouch out ball shaped under the serosa or in the fatty connective tissue of the sigmoid region. They occur in pairs, often symmetrically along the taenias. They increase with age; as their evidence depends on the actual contraction of the bowel, the incidence varied strongly with the method of investigation. The pseudo-diverticula can cause diverticulitis. The anatomical characteristics of the large bowel are the fortified longitudinal muscular bands, the taenias, and the haustras. By a co

[Colonic ileus. Treatment of colonic ileus, a 10-year patient material].

Morbidity and mortality have been evaluated in 238 patients with acute large bowel obstruction admitted over a ten year period. In two thirds, the cause of obstruction was a primary colonic carcinoma. Most of these patients (61%) had advanced disease and were over 70 years of age. Volvulus and cancer recurrences each accounted for 10%, and diverticulitis for eight percent. An overall mortality of 24% was found. Of 115 patients who had the obstructing lesion--primary carcinoma or diverticulitis--resected, a mortality of 8.7 was seen, higher for primary (14.9%) than for staged (4.4%) resections,

Perforated granulomatous colitis caused by Histoplasma capsulatum.

A 57-year-old man who presented with an acute abdomen and clinically was thought to have perforated colonic diverticulitis, was found to have transmural granulomatous inflammation and perforation of colon that was caused by Histoplasma capsulatum. Although involvement of any part of the gastrointestinal tract may occur with disseminated histoplasmosis, the complication of intestinal perforation requiring emergency surgery (particularly in the colon) is extremely rare and warrants this case report with discussion of the various clinicopathologic features of gastrointestinal histoplasmosis and t

Primary Anastomosis Versus Hartmann's Procedure in Acute Care Surgery: Evidence, Indications, and Barriers to Adoption.

Purpose of ReviewThis review examines the contemporary role of Hartmann's procedure (HP) in acute care surgery, focusing on the two principal disease processes in which it remains commonly utilized: complicated diverticulitis and traumatic colon injury. We review the evolving evidence comparing HP with primary anastomosis (PA), with or without diverting loop ileostomy (DLI), while emphasizing differences in patient populations, operative decision-making, and factors influencing adoption in clinical practice.Recent FindingsIncreasing evidence supports the safety and feasibility of PA in appropr

Abdominal Pain Syndromes: Diverticular Disease.

Diverticular disease is a common finding in Western countries, with a prevalence of up to 70% among individuals at 60 years of age. The term diverticular disease includes the historically recognized conditions of diverticulosis, uncomplcated diverticulitis, complicated diverticulitis, and diverticular bleeding; it also includes the increasingly recognized condition of symptomatic uncomplicated diverticular disease. The diagnosis of symptomatic diverticular disease requires a history and physical examination, laboratory evaluation, and imaging. In uncomplicated diverticulitis, antibiotic therap

The results of rapid source control laparotomy or open abdomen for acute diverticulitis.

INTRODUCTION: Rapid source control laparotomy (RSCL) for the management of non-traumatic intra-abdominal emergencies has increased over the past 25 years when it was advocated for trauma patients. Little data, however, support its widespread use. We hypothesize that the patients with RSCL will have poorer outcomes than those treated with primary fascial closure (PFC). METHODS: Patients operated for acute diverticulitis from 2014 to 2016 using The American College of Surgeons sponsored National Surgical Quality Improvement Program (NSQIP) data were reviewed. Two groups were identified: PFC, pa

Primary anastomosis and nonrestorative resection for perforated diverticulitis with peritonitis: meta-analysis of randomized trials.

AIM: The aim of this meta-analysis was to comparatively evaluate the outcomes of primary anastomosis (PRA) and nonrestorative resection (NRR) as emergency surgery and ostomy reversal in patients with perforated diverticulitis and peritonitis. METHODS: PubMed, MEDLINE via Ovid, Embase, CINAHL, Cochrane Library and Web of Science databases were systematically searched. Postoperative morbidity following emergency resection was the primary end-point. Quality assessment of the included studies was performed using the Cochrane Quality Assessment Tool including recruitment bias and crossover with in

CT colonography followed by elective surgery in patients with acute diverticulitis: a radiological-pathological correlation study.

PURPOSE: To perform a radiologic-pathologic correlation analysis of sigmoid colon in patients undergoing pre-operative CT Colonography (CTC) after an episode of acute diverticulitis (AD). METHODS: Fifty-nine consecutive patients (31/28 M/F; 58 ± 13 years) underwent CTC 55 ± 18 days after AD, 8 ± 4 weeks before surgery. Thirty-seven patients (63%) underwent conventional abdominal CT at time of AD. An experienced blinded radiologist retrospectively analyzed all images: disease severity was graded according to the Ambrosetti classification on conventional CT and according to the diverticular dis
