Malta Consensus Statements on Paranasal Sinus Disorders in SCUBA Diving: Results of a Delphi Study.

OBJECTIVES: To establish expert consensus on the diagnostic approach, treatment strategies, and fitness-to-dive assessment for divers with paranasal sinus disease, addressing the current lack of specific guidelines for this frequent diving-related disorder. METHODS: A rapid, conference-based modified Delphi process was conducted with a panel of 14 Dutch otorhinolaryngologists, including four certified diving medicine specialists. Consensus was defined as more than 80% agreement. RESULTS: The panel achieved consensus on recommendations across four domains. For diagnosis, a comprehensive ENT e…

Potential for flexible lactate shuttling between astrocytes and neurons to mitigate against diving-induced hypoxia.

For most non-diving mammals, lack of O2 (hypoxia) has detrimental effects on brain function. Seals, however, display a series of systemic, cellular, and molecular adaptations that enable them to tolerate repeated episodes of severe hypoxia. One as yet unresolved question is whether seal neurons in part employ anaerobic metabolism during diving: the "reverse astrocyte-neuron lactate shuttle" (rANLS) hypothesis postulates that seal neurons, by shuttling lactate to the astrocytes, may be relieved (1) from the lactate burden and (2) from subsequent ROS-production as lactate is oxidized by astrocyt…

Continuous Glucose Monitoring During Recreational Diving in Type 1 Diabetes: Navigating Clinical and Technical Uncertainties.

Recreational diving with self-contained underwater breathing devices is gaining popularity worldwide as a sport and leisure activity. People living with type 1 diabetes mellitus (PLT1D) are no exception, although historically diabetes mellitus, especially insulin-treated, has been described as an absolute contra-indication for diving. However, based on observational data collected by the Divers Alert Network, the presence of background diabetes mellitus became only a relative contraindication for those without significant co-morbidities or long-term complications. Regarding diving activities a…

Diving into Relevance: How Deep Sea Researchers Articulate Societal Relevance within their Epistemic Living Spaces.

The deep sea is many things: a subject of both fascination and indifference; a realm of great unknowns and scientific inquiry; and a source of untapped potential and looming exploitation. Its multifaceted nature is also visible in scientific research on the deep sea. While traditional relevance narratives in the field of deep sea research centred around fundamental knowledge creation, calls for doing societally relevant research are intensifying. The field is currently in transition regarding its understanding of societally relevant research, as it has been criticised for failing to provide na

Contrasting offspring dependence periods and diving development rates in two closely related marine mammal species.

Understanding the ontogeny of diving behaviour in marine megafauna is crucial owing to its influence on foraging success, energy budgets, and mortality. We compared the ontogeny of diving behaviour in two closely related species-northern elephant seals (Mirounga angustirostris, n = 4) and southern elephant seals (Mirounga leonina, n = 9)-to shed light on the ecological processes underlying migration. Although both species have similar sizes and behaviours as adults, we discovered that juvenile northern elephant seals have superior diving development, reaching 260 m diving depth in just 30 days…

Chain of events analysis in diving accidents treated by the Royal Netherlands Navy 1966-2023.

INTRODUCTION: Diving injuries are influenced by a multitude of factors. Literature analysing the full chain of events in diving accidents influencing the occurrence of diving injuries is limited. A previously published 'chain of events analysis' (CEA) framework consists of five steps that may sequentially lead to a diving fatality. This study applied four of these steps to predominately non-lethal diving injuries and aims to determine the causes of diving injuries sustained by divers treated by the Diving Medical Centre of the Royal Netherlands Navy. METHODS: This retrospective cohort study w…

Diving deep into fish bornaviruses: Uncovering hidden diversity and transcriptional strategies through comprehensive data mining.

Recently, we discovered two novel orthobornaviruses in colubrid and viperid snakes using an in silico data-mining approach. Here, we present the results of a screening of more than 100,000 nucleic acid sequence datasets of fish samples from the Sequence Read Archive (SRA) for potential bornaviral sequences. We discovered the potentially complete genomes of seven bornavirids in datasets from osteichthyans and chondrichthyans. Four of these are likely to represent novel species within the genus Cultervirus, and we propose that one genome represents a novel genus within the family of Bornaviridae

Unilateral optic neuropathy from possible sphenoidal sinus barotrauma after recreational scuba diving: a case report.

A case report is presented of a 35-year-old woman who developed a progressive right optic neuropathy while surfacing from a series of four recreational dives on the Great Barrier Reef, Queensland, Australia. The patient reported severe sudden onset blurred vision in the right eye associated with a mild headache and epistaxis on surfacing from diving. The patient had her first medical review the day after returning from her trip. At this time visual acuity in the right eye was 20/80, with left eye 20/20. There was a relative afferent pupillary defect in the right eye. A high-resolution computed…

Decompression illness secondary to occupational diving: recommended management based current legistation and practice in Malaysia.

Occupational divers are exposed to hazards which contribute to the risk of developing decompression illnesses (DCI). DCI consists of Type I decompression sickness (DCS), Type II DCS and arterial gas embolism (AGE), developed from formation of bubbles in the tissues or circulation as a result of inadequate elimination of inert gas (nitrogen) after a dive. In Malaysia, DCI is one of the significant contributions to mortality and permanent residual morbidity in diving accidents. This is a case of a diver who suffered from Type II DCS with neurological complications due to an occupational diving a…

Gas exchange and pulmonary stress variations during SCUBA and breath-hold diving in open seawater.

Understanding of pulmonary gas exchange measurements in divers at sea is incomplete. In this study, arterial blood gases (ABGs) were measure … sound abnormalities. KEY POINTS: The understanding of lung-environment interactions during open-sea diving remains limited. We integrated underwater and surface arterial blood gases, lung ultrasound and endothelial glycocalyx markers (syndecan-1, heparan sulfate) to quantify gas-exchange perturbations and lung stress in SCUBA and breath-hold divers (BHDs …

Prediction of unpowered diving and floating for large-depth manned submersible based on geometric similarity.

Large-depth manned submersibles generally conduct diving and floating operations in an unpowered mode. On the premise of guaranteeing satisfactory diving and floating speed, this mode can significantly reduce energy consumption and prolong underwater operating time, which serves as one of the key foundations for the underwater operational performance of manned submersibles. Therefore, the study on prediction methods for unpowered diving and floating motion is of great engineering significance. At present, existing prediction methods are mainly established on the standard motion model, whose re…

Geographic Variation in Diving Behaviour and Skin Isotope Ratios of Northern Bottlenose Whales, Hyperoodon ampullatus.

Geographic variation in the diving behaviour of oceanic mammals is a major component of their biodiversity with far-reaching relevance for ecology and conservation. Here we compared high-resolution multi-sensor tag data from northern bottlenose whales in Jan Mayen, Norway (n = 15, 127.2 h) and the Gully, Scotian Shelf, Canada (n = 6, 45.8 h) to compare diving and foraging behaviours. K-means clustering identified short-shallow and long-deep dive types in both locations but showed a propensity for mid-depth dives in Jan Mayen. Foraging dives (defined by high roll variance corroborated by active…

Diving into Diversity: Hasleaberepwari (Bacillariophyceae, Naviculaceae), a new species of marine diatom from New Caledonia.

The current article introduces and describes Hasleaberepwari sp. nov., a new species of diatom discovered in the vicinity of Boulouparis, New Caledonia. Under light microscopy, H.berepwari sp. nov. strongly resembles Hasleapseudostrearia, but preliminary molecular barcoding conducted using partial 18S and rbcL genes suggested that it was a distinct species. This was confirmed first by scanning electron microscopy which showed the differences in stria densities between both species. A short-reads genome-skimming protocol applied on H.berepwari sp. nov. led us to obtain its complete mitochondria

Cave Diving Documents Spatial and Temporal Water Quality Variability in a Phreatic, Karst Cave System.

Karst aquifers have evolved secondary porosity features that facilitate heterogeneous recharge and groundwater flow dynamics. These dynamics … as wells or springs, which do not always capture the magnitude of water quality heterogeneity. Cave diving in phreatic caves can be used to explore this variability by using water quality sensors and discrete water chemistry samples to explore spatial and temporal water quality changes for improved and targeted water resource management. Our study uses c …
