This report demonstrates a case of undifferentiated carcinoma of the duodenal ampulla. A 74-year male experienced jaundice lasting for 3 weeks. An upper gastrointestinal series demonstrated a polypoid, ovoid filling defect in the second portion of the duodenum, and duodenoscopy disclosed a protruding mass involving the orifice of the papilla of Vater. Cholangiography demonstrated obstruction due to compression in the terminal common bile duct. Pylorus-preserving pancreatoduodenectomy was performed on the diagnosis of ampullary carcinoma. BACKGROUND: Although duodenal rupture is usually diagnosed during the course of surgery for other injuries, a small portion of such injuries occur in isolation. In such cases, the significance of the clinical and diagnostic findings may not be appreciated for an extended period. The primary determinant of mortality in duodenal rupture is the presence of associated injuries, but delay in diagnosis is often a secondary factor.
METHODS: A retrospective case review of 8 patients with isolated duodenal rupture that was diagnosed more than 24 hours following the injury.

A Quality of Life questionnaire was administered to 200 duodenal ulcer patients taking daily H2 antagonists and compared with the responses given by 50 newly diagnosed duodenal ulcer patients. Assessment involved 17 questions which attempted to measure ulcer symptoms, emotional and social function. Age, sex and percentage of smokers were identical in the two groups. The Plenk method in the author's modification was successfully applied in resection of the stomach "for exclusion" in 22 patients. Two cases of systemic mastocytosis are presented. Both patients had peptic ulceration. Greatly raised values on investigation of gastric acid secretion in patients with therapy-resistant peptic ulceration and normal serum gastrin should raise the suspicion of systemic mastocytosis.

Gastrointestinal tuberculosis is still rampant in underdeveloped countries and can mimic other gastrointestinal (GI) disorders. Herein we report four cases of isolated proximal duodenal tuberculosis, without involvement of other parts of the GI tract. The clinical presentation in three of them resembled that of peptic ulcer disease, and one had features of gastric outlet obstruction. On investigation, one of these patients had a pyeloduodenal fistula. The limitations of clinical evaluation, radiology, and endoscopy are stressed, and the value of surgical biopsy is highlighted.
