Developmental morphology of the middle ear.

The development of the murine middle ear was monitored both qualitatively and morphometrically by scanning electron microscopy from the 19th gestational day to the adult stage. At birth, the middle ear was less well developed than the inner ear. The tympanic membrane (TM) was obscured by occlusion of the external auditory canal. Ciliated cells and secretory granules were present in the middle ear epithelium already 5 days after birth (DAB). Keratin debris was discerned on the external layer of the TM 9 DAB. By 12 DAB, mesenchymal tissue had resorbed from the middle ear cavity, except around th…

[Tympanoplasty on only hearing ears].

Tympanoplasty on only hearing ears was performed on 11 patients in Osaka University Hospital and Osaka Rosai Hospital during 1986 1994; this was 0.68% of all cases of middle ear surgery. The patients consisted of 5 with cholesteatoma, 5 with chronic otitis media and 1 with cholesterin granuloma. The mean age was 47.6 years (13-68 years) and the mean follow-up period was 2 years and 7 months (6 months-4 years). Postoperative hearing results was obtained in 9 patients (81.8%) according to the criteria of Otology Japan (1987), and all ears were dry at the final examination. Compared with the preo…

[Treatment of the deformed ear and jaw].

Lengthening of the mandible by gradual distraction and ear reconstruction using tissue expanders were performed on nine young patients (6-11 years old) with unilateral hemifacial microsomia. The technique holds promise for reconstruction of ear and mandible at the same time. This is essentially a pilot clinical project. A late follow-up of these patients is necessary.

[Lightening strike injuries of the ear].

Although lightning injuries are quite common, lightning-induced damage to the middle or inner ear and to the vestibular nerve has been reported in only a very few cases. Two case reports are presented: a 22-year-old woman and a 26-year-old man who experienced lightning injuries to several body parts including the ear. Pathophysiology, diagnostic criteria and treatment are discussed.

Otoplasty for correction of the prominent ear.

The oral and maxillofacial surgeon is aware of the need to evaluate and treat the entire facial complex. To gain maximum esthetic results, excessive ear prominence occasionally needs to be addressed. Excessive ear prominence can be the result of failure of scapha folding, conchal hypertrophy, conchal malposition, or a combination of these deformities. Management of this problem is based on the accurate diagnosis of the deformity and a thorough understanding of the basic techniques that address them. This article presents an overview of otoplasty for the treatment of prominent ears.

[Adenomatous neoplasia of the middle ear].

Adenomatous neoplasm occurring in the middle ear is rare. The clinical findings are non-specific and include otological symptoms as well as sometimes functional deficits of the lower cranial nerves. The final diagnosis will be made only after elaborate histological examinations because of the inconsistent radiological and clinical findings. DNA analysis yields some information on the prognosis of this tumour. We report on a case where a primary adenomatous neoplasm developed in the middle ear, and discuss our concept of therapy.

Carcinoid tumor of the middle ear.

Carcinoid tumor of the middle ear is a very rare benign neoplasm, which may be mistaken for a malignant tumor. We present two new cases together with a review of 28 previously reported cases. Nearly all patients had progressive hearing loss, most often of the conductive type. About half of the patients complained of tinnitus and fullness of the ear. The typical otoscopic picture was erythema or lateral bulging of the tympanic membrane, which was intact in most patients. The tumor was most often localized to the middle ear with varying degree of extensions into neighboring areas. It often encap…

[Revision of stapedectomy (110 ears)].

We have carried out a retrospective study of 110 ears in patients affected with otosclerosis who underwent revision of stapedectomy, from 1979 to 1991, because they did not achieve hearing improvement or still presented persistent dizziness after primary stapedectomy. In our series, the most common causes of surgical failure after the first surgery are: displacement of the prosthesis (36.3%), ossicular chain alterations (21.8%) and regrowth of otosclerotic bone in the oval window (19.9%). One year after surgery, hearing test was compared with preoperatory tonal audiometry, finding and improvem…

[Benign adenoma of the middle ear].

Benign adenoma of the middle ear is a tumor from glandular mucosa. In most of the cases, it is revealed between 40 and 60 year old, by a conductive hearing loss with a greyish mass behind intact tympanic membrane. Scanography demonstrated a no specific tumor, without bone destruction. The ossicular chain is usually respected. The histologics findings show two distinctive type of cells: glandular and neuroendocrine differentiation. The treatment is surgical. A regularly follow up is necessary to diagnostic recurrence.

Asymptomatic osteomas of the middle ear.

Osteomas of the middle ear are extremely rare lesions. Of the nine cases reported, eight presented with conductive hearing loss. We recently treated two patients whose osteomas, of the promontory and of the posteroinferior tympanic wall, were asymptomatic. We suggest that middle ear osteomas need not be removed when they do not impinge on the sound conduction system.

Dermoid cyst in the middle ear.

A case of a moderate essentially low tone unilateral conductive hearing loss without a previous history of secretory otitis media in a six-year-old female patient is described. The condition was found to be due to a dermoid cyst in the tympanic cavity, which was confirmed by pathological examination of the surgically removed specimen. This is the sixth documented case of dermoid cyst in the tympanic cavity.

Optimum management of the discharging ear.

Discharge from the ear can be the result of many disease processes. The ear may discharge blood, pus, cerebrospinal fluid (CSF) or wax. Keratosis obturans, stenosis of the external meatus and benign tumours of the external meatus all lead to wax build-up, which may cause recurrent attacks of otitis externa. Malignant tumours, such as basal cell carcinoma, squamous cell carcinoma and tumours of ceruminous gland origin may also present with discharge. Tumours should be excluded by submitting all material removed from the external canal for histological examination. Single or multiple abscesses (…

Merkel cell carcinoma of the ear.

All the consultants agree that, given this patient's history, a common skin tumor like squamous cell or basal cell carcinoma is unlikely. Melanoma or Merkel cell carcinoma belong in the differential. Interestingly, the consultants all suggest a biopsy of the lesion prior to other testing, and because this tumor is so accessible, a biopsy should not interfere with further testing or treatment. Drs. Weymuller and Marks would then proceed with a CT scan; Dr. Ridge favors an MRI scan. While a chest-ray is in order to rule out metastases, Dr. Weymuller also suggests immunocytochemistry. [Congenital cholesteatoma of the middle ear].

The AA. inform about 8 patients suffering from congenital cholesteatoma of the tympanomastoidal cleft. They underline the difficulties for achieving a correct diagnosis when the eardrum remains intact and remark the need to keep in mind this process for differential diagnosis of the conductive hypoacusis in childhood. As a matter of fact the AA. emphasize the few cases of recurrences after surgery, in contrast with that of relapses in child's acquired cholesteatoma.

Odontoma of the middle ear cleft.

Odontoma of the middle ear cleft is an unusual cause of conductive hearing loss. We report two cases and discuss the developmental mechanism by which this anomaly may occur. The differential diagnosis of calcified masses in the middle ear is also discussed.

[Primary adenocarcinoma of the middle ear].

One case of primary adenocarcinoma of the middle ear is reported, which developed from a chronic otitis media. Histological examination established the diagnosis of adenocarcinoma. Its clinical appearance was benign, being restricted to the middle ear and tympanic membrane and with no bone destruction or metastases. Two years after local surgery there was no evidence of recurrence.
