Incidence and risk factors for the development of radiographic arthrosis after traumatic elbow injuries.

PURPOSE: Radiographic arthrosis is a common sequela of elbow trauma. Few studies have addressed risk factors for radiographic arthrosis after elbow injury, especially in the long term. Data from multiple long-term follow-up studies of patients with surgically treated elbow fractures provided us with an opportunity to assess risk factors for long-term radiographic arthrosis after elbow injury. METHODS: During a 5-year period, we obtained radiographs during a research-specific evaluation of 139 patients (81 men and 58 women) 10 or more years (median, 19.5 y; range, 10-34 y) after surgical treat…

Prevention and management of early treatment failures in elbow injuries.

The appropriate management of early treatment failures in patients with elbow injuries requires the identification of the cause of failure. In this work, six types of elbow injury are considered: (i) identification of early failed reduction of a dislocation or fracture-dislocation of the elbow should lead to a repeat reduction procedure, testing for elbow instability, and ligament repair, followed by the use of a hinged external fixator to allow early mobilisation. Differentiating an isolated dislocation from a dislocation combined with a fracture of the coracoid process is crucial. Re-implant

Elbow injuries.

The elbow is a commonly injured joint, yet physicians may be less comfortable treating injuries to the elbow compared with knee and shoulder injuries. Common injuries involving the elbow are tendinosis, instability, tendon ruptures, osteochondritis dissecans, and fractures. Tendinosis is a common overuse injury and may occur on the lateral, medial, or infrequently, the posterior side of the elbow. Injury to the medial or lateral ulnar collateral ligaments may result in instability. Repetitive trauma from overuse is the most common etiologic factor in athletes. Distal biceps and triceps tendon

Long-term outcome of displaced radial neck fractures in adulthood: 16-21 year follow-up of 5 patients treated with radial head excision.

BACKGROUND: There have been no reports on the long-term outcome of radial neck Mason type IIIb fractures in adults. METHODS: 3 women and 2 men, aged 46 (22-69) years when they sustained a radial neck Mason type IIIb fracture, were evaluated after an average of 18 (16-21) years. All had been treated with radial head excision. RESULTS: 3 individuals had no subjective elbow complaints while 2 reported occasional weakness. None had severe elbow complaints. The maximum elbow-to-elbow difference in range of motion was a deficit of mean 10 degrees in extension in the injured elbow. Mean deficits in

Surgical treatment for posterior distal humeral shear fractures.

BACKGROUND: Posterior shear fractures of the distal humerus are rare and have a complex, controversial injury mechanism. This study aims to investigate the morphological characteristics, injury mechanisms, and treatment outcomes of these fractures. METHODS: From March 2021 to October 2024, 13 patients with posterior shear fractures of the distal humerus were treated with open reduction and internal fixation. Among them, 7 were male and 6 were female, with a mean age of 40 years (range: 25-66 years). Five cases involved the left side, and 8 involved the right side. All fractures were closed. M

Ununited Fracture Medial Condyle in a Child: A Case Report and Review of Literature.

INTRODUCTION: Fractures of the medial condyle of elbow in children are rare. Diagnosis is frequently missed and many authors reported poor outcome for these fractures when treatment was delayed. CASE REPORT: Here, we present a case of 3-month-old ununited fracture of the medial condyle in an 8-year-old child, treated by osteosynthesis that produced good result. The patient presented to us with valgus instability of elbow and restricted range of motion. Open reduction, freshening of margins, and fixation of the ununited fragment with K-wires were done. Postoperatively, the patient regained fun

Three-dimensional printing designed customized plate in the treatment of coronal fracture of distal humerus in teenager: A case report.

RATIONALE: Because of the complicated anatomy and considerable change in size and morphology with age in teenagers, the appropriate internal fixator of coronal shear fracture of distal humerus is difficult to choose, and therefore, the fixation of this kind of fracture is difficult and controversial. Furthermore, distal humeral fractures in teenagers often involve the epiphysis, the rigid fixation of fracture and the simultaneous minimally invasive and protection of the epiphysis are contradictory. Coronal shear fractures of the distal humerus in teenagers are great challenge for orthopedic su

Radial head fractures.

Radial head fractures are the most common adult elbow fracture. Our ability to treat these fractures has improved with increased understanding of elbow biomechanics. Based on fracture type, possible treatment includes nonoperative management, open reduction and internal fixation, radial head resection, and replacement arthroplasty. Management and operative technique are discussed.

Complications of type I coronoid fractures in competitive athletes: report of two cases and review of the literature.

Coronoid fractures have been classified into three types. Elbow instability is a well-recognized complication of type III and less so in type II fractures. Type I fractures have generally been considered to heal uneventfully, with early range of motion. We present two cases of type I fractures in athletes that failed to improve with conservative management. One developed loose body formation and the other a fibrous nonunion with mechanical blockage of elbow flexion. Both patients responded to arthroscopic intervention and returned to full athletic activities. These cases illustrate potential c

Fractures of the radial head in adults with the complication "cubitus valgus".

Cubitus valgus is a well-known complication of fracture of the bones of the elbow in childhood, but it is an uncommon deformity in adults. Three adult patients are presented, in each of whom cubitus valgus developed as a complication of a fracture and resection of the radial head. The importance of reestablishing the integrity of the ligaments of the elbow following severe injuries to the elbow is stressed.

Articular fractures of the distal humerus.

Distal humeral fractures represent 2% of all adult elbow fractures. Injury mechanisms include high-energy trauma with skin involvement, and low energy trauma in osteoporotic bone. Treatment goals are anatomical restoration in young, high-demand patients and quick recovery of activities of daily living in the elderly. Complete fractures are relatively easy to diagnose, but partial intra-articular fractures are not. The clinical diagnosis must take into account potential complications such as open injuries and ulnar nerve trauma. Standard X-rays with additional distraction series in the operatin…

Ipsilateral simultaneous fracture of the trochlea involving the lateral end clavicle and distal end radius: a rare combination and a unique mechanism of injury.

Isolated trochlea fracture in adults is a rare surgical entity as compared to its capitellar counterpart. It has been only mentioned sporadically in the literature as case reports. Fracture of the trochlea is accompanied by other elbow injuries like elbow dislocation, capitellum fracture, ulnar fracture and extraarticular condylar fracture. Here we report a unique case of isolated displaced trochlea fracture associated with fractures of the lateral end clavicle and the distal end radius. We propose a unique mechanism for this rare combination of injuries: typical triad of injury, i.e. fracture

Osteochondral flap avulsion fracture in a child with forearm compartment syndrome.

Traumatic elbow dislocations account for approximately 3% to 6% of childhood upper extremity injuries, and 41% of these dislocations are concomitant injuries around the elbow. Most of these injuries are associated with a fall on an outstretched hand. Compartment syndrome is one recognized complication after elbow dislocation and distal humeral and radial neck fracture. Compartment syndrome in the forearm is a devastating complication in children with forearm, elbow, and supracondylar fractures. Compartment syndrome occurs as the result of hypoxic damage caused by interruption of the circulatio

Treatment of radial head and neck fractures: in favor of anatomical reconstruction.

Radial head fractures represent the most common elbow fractures. Undisplaced fractures usually occur in isolation and can be treated nonsurgically. Displaced fractures should be treated surgically. Simple two-part fractures can easily be handled by osteosynthesis, but comminuted fractures pose a major problem for reconstruction. As the radial head is an important stabilizer of the elbow joint-especially in the context of concomitant ligamentous injuries-its resection may lead to pain, limited range of motion, and instability. Therefore, radial head resection is not recommended for the acute si…

Arthroscopic Reduction and Fixation of Coronoid Fracture with Screws and Modified Pull Out Configuration.

The fracture of the coronoid process can compromise elbow stability. The surgical treatment aims to achieve adequate reduction and stability of the fragment. This article describes how arthroscopic assistance and the use of screws and sutures achieve fragment fixation. Our main objective is to describe the effectiveness of arthroscopy in obtaining reduction and fixation of isolated coronoid process fractures. This technique allows for optimal fragment stability and early rehabilitation.

Lateral Humeral Condyle Fracture in Childhood: Results of a New Surgical Technique.

Fractures of the lateral condyle of the humerus are one of the most common fractures in children, accounting for between 10% and 20% of fractures involving the elbow, with a peak incidence at 6 years of age. Treatment is often surgical for displaced fractures > 2 mm, according to Milch and Jakob classification. There is no consensus in the literature about the appropriate surgical management of these fractures. Objectives: The aim of this study is to describe, propose, and evaluate outcomes and complications of the surgical technique of reduction and osteosynthesis using trans-bone suture with

Radial head-capitellum view in elbow trauma: clinical application and radiographic-anatomic correlation.

The radial head-capitellum view, first reported 2 years ago, has proved useful in the evaluation of elbow trauma. Minimally displaced or nondisplaced fractures of the radial head, capitellum, and coronoid process are better demonstrated on this special projection than on traditional radiographs of the elbow. To test the accuracy of this view, 50 patients with trauma to the elbow were examined and routine anteroposterior, lateral, and external oblique radiographs were compared with this special angled projection. In 10 patients, the conventional studies either were negative or failed to show th

Kinematics and stability of the fractured and implant-reconstructed radial head.

Controversy exists as to the optimal management of radial head fractures. Biomechanical studies have been conducted to quantify elbow stability for simulated wedge fractures, head excision, and head replacement, with and without the integrity of the collateral ligaments. Our in vitro studies have demonstrated that in the ligamentously intact elbow, kinematics and stability are slightly altered with simulated depressed wedge fractures up to 120 degrees of the radial head, markedly altered with head resection, and improved after radial head replacement. Radial head excision decreases elbow stabi

Combined simple elbow dislocation and forearm joint injuries. A systematic review of the literature with injury patterns and current treatment rationale.

INTRODUCTION: Recent studies have allowed a great improvement in understanding elbow and forearm traumatic injuries. Simple elbow dislocations combined with forearm injuries have been occasionally reported in the literature. The aim of this study was to detect the possible patterns of simple elbow dislocations and forearm fracture-dislocations, providing a classification scheme to guide surgical treatment. MATERIALS AND METHODS: PubMed search was performed to find combination of simple elbow dislocation and forearm joint injury, between 2000 and 2020, including clinical studies and case repor
