Naturalistic follow-up in bipolar patients after successful electroconvulsive therapy.

BACKGROUND: Electroconvulsive therapy (ECT) is an effective treatment for acute depression, mania and mixed states. We evaluated the long-term outcome of patients with bipolar depression or mixed state, responsive to ECT. METHODS: this observational follow-up study was conducted in 70 patients with Bipolar Disorder: 36 patients met DSM-IV-TR criteria for a major depressive episode (MDE) and 34 for a mixed episode (MXE). During the follow-up after ECT, the relapse rates and the duration of response and remission periods were recorded. RESULTS: the mean duration of the follow-up was 57 weeks. …

Grey Matter changes in treatment-resistant depression during electroconvulsive therapy.

INTRODUCTION: 20-30% of depressed patients experience Treatment Resistant Depression (TRD). Electroconvulsive Therapy (ECT) remains the treatment of choice for TRD. However, the exact mechanism of ECT remains unclear. We aim to assess grey matter changes in patients with TRD undergoing bilateral ECT treatment at different points during and after treatment. METHODS: Patients are recruited at the University Hospital of Toulouse. Eligibility criteria include a diagnosis of TRD and an age between 50 and 70 years old. Patients received clinical assessments (Hamilton Depression Rating Scale) and st…

Electroconvulsive Therapy without Consent: The Influence of Human Rights Law.

The use of electroconvulsive therapy (ECT) is highly regulated across Australia. Its use on those under compulsory mental health treatment orders remains controversial and the United Nations Special Rapporteur on Torture and Other Cruel and Inhuman Treatment or Punishment has called for a ban on its nonconsensual use. Mental health tribunals must consider whether or not the person concerned has capacity to consent to ECT and there have been different understandings of just what capacity means in this regard. This column discusses the influence of human rights law and a recent decision by Justi…

Factors Associated With Delirium Following Electroconvulsive Therapy: A Systematic Review.

OBJECTIVES: Delirium following electroconvulsive therapy (ECT) has been a clinical challenge, which, however, has not been investigated through a systematic literature review. The objective of this study was to systematically synthesize available evidence regarding factors associated with post-ECT delirium. METHODS: We conducted a systematic literature search for any type of original investigations that reported risk factors of post-ECT delirium, using PubMed. RESULTS: The literature search identified 43 relevant articles. One study found an association between catatonic feature and increase…

Electroconvulsive therapy electrode placement for bipolar state-related targeted engagement.

BACKGROUND: Electroconvulsive therapy (ECT) is an effective treatment for all bipolar states. However, ECT remains underutilized, likely stemming from stigma and the risk of neurocognitive impairment. Neuroimaging research has identified state-specific areas of aberrant brain activity that may serve as targets for therapeutic brain stimulation. Electrode placement determines the geometry of the electric field and can be either non-focal (bitemporal) or more focal (right unilateral or bifrontal). Previous research has shown that electrode placement can impact clinical and cognitive outcomes ind…

P11 promoter methylation predicts the antidepressant effect of electroconvulsive therapy.

Although electroconvulsive therapy (ECT) is among the most effective treatment options for pharmacoresistant major depressive disorder (MDD), some patients still remain refractory to standard ECT practise. Thus, there is a need for markers reliably predicting ECT non/response. In our study, we have taken a novel translational approach for discovering potential biomarkers for the prediction of ECT response. Our hypothesis was that the promoter methylation of p11, a multifunctional protein involved in both depressive-like states and antidepressant treatment responses, is differently regulated in…

Do Antiepileptic Drugs Used During Electroconvulsive Therapy Impact Treatment Process?

PURPOSE: Whether the antiepileptic agents used by patients before the treatment should be discontinued after the initiation of the electroconvulsive therapy (ECT), which is one of the most effective treatment options in psychiatry, is a dilemma frequently faced by clinicians. Recently, there has been an increased interest in this subject. This study aimed to determine the impact of antiepileptic agents on ECT process in the event of continued use in the course of the therapy. METHODS: From among the patients on an ECT therapy in the Psychiatry Clinic of Hacettepe University Hospital since 201…

Characteristics of Successful Transitions From Inpatient to Outpatient Electroconvulsive Therapy.

OBJECTIVES: The purpose of this study was to identify clinical and psychosocial factors involved in transitioning hospitalized patients receiving electroconvulsive therapy (ECT) from the inpatient to the outpatient setting and to propose an algorithm to guide clinicians with this process. METHODS: A retrospective chart review was completed for adult patients discharged from a psychiatric hospital from 2002 to 2012 who had an acute course of ECT that was initiated in the hospital and completed as an outpatient. We reviewed demographic and clinical information and outcomes, including ECT treatm…

Peripheral blood E2F1 mRNA in depression and following electroconvulsive therapy.

The E2F transcription factors are a group of proteins that bind to the promotor region of the adenovirus E2 gene. E2F1, the first family member to be cloned, is linked to functions including cell proliferation and apoptosis, DNA repair, cell senescence and metabolism. We recently performed a deep sequencing study of micro-RNA changes in whole blood following ECT. Two micro-RNAs (miR-126-3p and miR-106a-5p) were identified and gene targeting analysis identified E2F1 as a shared target of these miRNAs. To our knowledge, no studies have examined E2F1 mRNA levels in patients with depression. Perip

[Remission of a complex periodic catatonic syndrome under electroconvulsive therapy].

This article is reporting about a spontaneous occurred catatonic syndrome in a 52 years old female patients with no prior psychiatric illnes … on. After applying widespread somatic diagnostics, which has excluded a neurodegenerative disease a electroconvulsive therapy was applied. During this treatment the patient showed a recurrence of her catatonic symptoms but they remitted if there was a too long period between the convulsive treatments. After establishing a sufficient period between the convulsive treatments …

Nonconvulsive Status Epilepticus After Electroconvulsive Therapy: A Review of Literature.

BACKGROUND: The clinical presentation and risk factors of nonconvulsive status epilepticus (NCSE) in the context of electroconvulsive therapy (ECT) are poorly understood, and guidance regarding diagnosis and management remains scarce. In this article, we identify case reports of ECT-induced NCSE from literature, and discuss the presentation, diagnosis, and management of these cases in the context of what is known about NCSE from the neurology literature. METHODS: A literature search on PubMed for case reports of NCSE after ECT. RESULTS: We identified 13 cases for this review. Diagnosis in al…

Self-assessed remission rates after electroconvulsive therapy of depressive disorders.

BACKGROUND: Electroconvulsive therapy (ECT) effectively treats severe depression, but not all patients remit. The aim of the study was to identify clinical factors that associate with ECT-induced remission in a community setting. METHODS: Depressed patients who underwent ECT in 2011-2014 were identified from the Swedish National Quality Register for ECT. Remission was defined as self-rated Montgomery-Åsberg Depression Rating Scale scores of 0-10 after ECT. Other registers provided data on previous antidepressant use, comorbidities, and demographics. RESULTS: Of 1671 patients fulfilling the i…

Acute myocardial infarction following electroconvulsive therapy in a Schizophrenic patient.

Deaths due to acute coronary insufficiency have been reported during or following electroconvulsive therapy (ECT). Various mechanisms related to ECT related deaths also include cerebral, and respiratory complications; however, cardiovascular complication remains the most common. Here, we describe a 55 year old male with a longstanding psychiatric illness with multiple antipsychotics under regular follow-up and required repeated ECT therapy to have the disease symptoms under remission. This time he developed acute onset chest pain immediately post ECT shock and diagnosed as Acute anterior wall …
