Carotid artery stenting under flow arrest for the management of intraluminal thrombus: technical case report.

OBJECTIVE: The management of intraluminal carotid thrombus is not well defined. Medical and surgical management carry a relatively high risk of thromboembolism. Although endovascular options also carry the risk of thromboembolism from manipulation of the lesion, successful carotid stent placement using various methods of flow restriction has been reported anecdotally. We describe a technique to arrest antegrade flow before placement of carotid stents in the setting of symptomatic intraluminal thrombus. CLINICAL PRESENTATION: A 49-year-old woman presented with expressive dysphasia, right-upper

Endovascular stenting of supra-aortic lesions using a transcarotid retrograde approach and flow reversal: A multicenter case series.

OBJECTIVE: Endovascular treatment has largely replaced open reconstruction of proximal brachiocephalic and left common carotid ostial arterial stenoses. The objective of this study was to report the technical feasibility and safety of a flow-based embolic protection system in stenting of single and tandem stenotic lesions of supra-aortic arch vessels. METHODS: All cases used flow-based neuroprotection by the ENROUTE Transcarotid Neuroprotection System (Silk Road Medical, Sunnyvale, Calif). Case specifics, such as the stents used, the details of flow-based neuroprotection, the order in which l

Choices of Stent and Cerebral Protection in the Ongoing ACST-2 Trial: A Descriptive Study.

OBJECTIVE/BACKGROUND: Several plaque and lesion characteristics have been associated with an increased risk for procedural stroke during or shortly after carotid artery stenting (CAS). While technical advancements in stent design and cerebral protection devices (CPD) may help reduce the procedural stroke risk, and anatomy remains important, tailoring stenting procedures according to plaque and lesion characteristics might be a useful strategy in reducing stroke associated with CAS. In this descriptive report of the ongoing Asymptomatic Carotid Surgery Trial-2 (ACST-2), it was assessed whether

The effect of kinking and residual plaque ulcers after carotid artery stenting: differences between closed stent design and double-layer stents.

BACKGROUND: Carotid stenting (CAS) is an established alternative to carotid endarterectomy for asymptomatic and symptomatic stenosis. However, morphological features possibly appearing after CAS, such as carotid tortuosity/kinking and plaque ulcers underneath the stent (PuUS) remain under investigated. This study evaluates the impact of these factors on postoperative neurological risks, according to the type of stent implanted, i.e. closed stent design (CSD) versus double-layer stents (DLS). METHODS: A single center retrospective evaluation of all consecutive CAS from 2004 to 2022 was perform

The Transcaval AngioVac Aspiration of a Left Subclavian Artery Intimal Sarcoma Masquerading as Arterial Thrombosis: A Case Report.

Background and Clinical Significance: Intimal sarcoma is a rare, aggressive malignancy of the large arteries that grows within the vessel lumen and frequently mimics thromboembolic disease, leading to diagnostic delay. Flow restoration and tissue biopsy are both difficult if the tumour occupies a technically demanding arterial segment, which is further aggravated if conventional large-bore arterial access is precluded by peripheral arterial disease. Case Presentation: A 66-year-old woman with multiple cardiovascular risk factors presented with subacute left upper-limb ischaemia and painless di

Renal function and survival after renal artery stent revascularization may be influenced by embolic debris.

UNLABELLED: Four years of follow-up renal function and survival data were obtained on 544 patients who underwent 714 successful renal artery stenosis (RAS) stent revascularizations. The mean serum creatinine (SCr) was unchanged at 4 years (1.6+/-1.0 mg/dl versus 1.6+/-0.9 mg/dl). The 2-year paired comparison analysis revealed no change in the reciprocal of the SCr (1/SCr), when compared with baseline or 1-year follow-up values. Simple linear regression analysis revealed flat post-procedure trend line slopes within all patient subsets, which implied renal functional stabilization. The 4-year mo

Transcervical carotid stenting with internal carotid artery flow reversal: feasibility and preliminary results.

OBJECTIVE: Transfemoral carotid artery stenting (CAS), with or without distal protection, is associated with risk for cerebral and peripheral embolism and access site complications. To establish cerebral protection before crossing the carotid lesion and to avert transfemoral access complications, the present study was undertaken to evaluate a transcervical approach for CAS with carotid flow reversal for cerebral protection. METHODS: Fifty patients underwent CAS through a transcervical approach. All patients with symptoms had greater than 60% internal carotid artery (ICA) stenosis, and all pat

Antithrombotic Therapy after Structural Heart Disease Intervention.

BACKGROUND: Structural heart disease interventions, including transcatheter aortic valve implantation (TAVI), transcatheter edge-to-edge repair (TEER), patent foramen ovale (PFO) closure, and left atrial appendage closure (LAAC), have transformed the management of valvular and congenital heart disease. However, antithrombotic therapy following these procedures remains a major challenge, as clinicians must balance the prevention of thromboembolic events with the risk of bleeding. Evidence from randomized trials and registries has provided guidance, yet optimal strategies are still evolving. ME

Outcomes of Innominate Artery Revascularization Through Endovascular, Hybrid, or Open Approach.

BACKGROUND: Atherosclerotic disease of the innominate artery (IA) is rare and can lead to cerebral, upper extremity, and vertebral steal symptoms. Nonocclusive lesions can be treated with endovascular interventions, often with a hybrid approach while performing a right carotid endarterectomy (RCEA). Calcified IA lesions have a high risk of embolization to bilateral cerebral hemispheres. Occlusive lesions may require treatment through a median sternotomy and bypass. The purpose of our study is to review our short-term and long-term outcomes of IA revascularization. METHODS: Our operative datab

Infective Endocarditis by Biofilm-Producing Methicillin-Resistant Staphylococcus aureus-Pathogenesis, Diagnosis, and Management.

Infective endocarditis (IE) is a life-threatening condition with increasing global incidence, primarily caused by Staphylococcus aureus, especially methicillin-resistant strains (MRSA). Biofilm formation by S. aureus is a critical factor in pathogenesis, contributing to antimicrobial resistance and complicating the treatment of infections involving prosthetic valves and cardiovascular devices. Biofilms provide a protective matrix for MRSA, shielding it from antibiotics and host immune defenses, leading to persistent infections and increased complications, particularly in cases involving prosth

Technical note on endovascular treatment of concomitant carotid occlusion in large vessel occlusion stroke: The "single-cross" technique.

BACKGROUND: Emergent large vessel occlusive (ELVO) stroke secondary to underlying carotid occlusive disease is frequently encountered in endovascular ischemic stroke therapy and trials. Up to 29% of all cerebral vascular accidents are attributed to severe carotid occlusive disease, and recent interventional trials have demonstrated this occurrence in 18.6-32.2% of ELVO stroke. We present a novel technique using the stent retriever guide wire to expedite angioplasty and/or stent placement for associated carotid occlusive disease during mechanical thrombectomy of ELVO stroke. This technique util
