Nystagmus, head-deviation, and hearing loss were provoked by Type I allergy induced in guinea pigs with passive sensitization and specific antigen challenge. Mast cell degranulation, edema of connective tissue of the endolymphatic sac and endolymphatic hydrops were histologically observed. These findings indicated that Type I allergy can be induced in the inner ear, and that symptoms analogous to Meniere's disease may be induced by this response.

There is a disturbing variation in the reported success rate in Endolymphatic Sac Surgery in the literature. Relatively long-term (24 months to 10 years) follow-ups are given in a series of 66 patients in this paper. Results reveal only a 55% long-term relief of vertigo in these patients. A pathophysiological interpretation is discussed of the clinical findings that in one out of three cases at the onset of Meniere's disease the cochlear and vestibular symptoms start simultaneously and that during the course of Meniere's disease most attacks show both cochlear and vestibular symptoms. It is concluded that the Meniere attack is not caused by a slowly expanding endolymphatic system that finally ruptures or leaks, but that the pathogenesis of the attack is different from that of the underlying endolymphatic hydrops.
