The subarachnoid shunt for Menière's disease: sixteen years' experience.

One hundred forty patients have been treated at the Baptist Memorial Hospital in Memphis, Tennessee for Menière's disease over a 16-year period, using an endolymphatic sac-subarachnoid shunt. Emphasis was placed on surgical methods, including compression of the sigmoid sinus with bone wax during surgery, facial nerve identification, and coverage of the sac with vein tissue. Satisfactory follow-up was accomplished in 66% of patients. Results were reported according to the criteria of the American Academy of Ophthalmology and Otolaryngology (1972). Control of attacks was accomplished in 83% of c…

A Three-Year Follow-Up of a Patient With Large Vestibular Aqueduct Syndrome Who Underwent Bilateral Endolymphatic Duct Blockage Surgery.

Large vestibular aqueduct syndrome (LVAS) is a congenital malformation characterized by an abnormally large vestibular aqueduct, diagnosed primarily via CT scans. Patients with LVAS often experience progressive hearing loss and recurrent vertigo, with treatment strategies mirroring those for Meniere's disease. Traditional surgical interventions such as endolymphatic sac decompression (ESD) are common; however, the efficacy of endolymphatic duct blockage (EDB) as an alternative still remains under investigation. We present the case of a female in her 20s who was diagnosed with Meniere's disease…

Correlation between hearing loss and scala media area in guinea pigs with long-standing endolymphatic hydrops.

HYPOTHESIS: Histologic analysis of the hydropic and normal guinea pig cochleae was undertaken to assess a potential correlation between the … animal model. METHODS: Nine albino guinea pigs were each subjected to surgical obstruction of the endolymphatic sac and duct of the right ear. The left ears remained as internal histologic controls. Hearing was assessed from 2 kHz to 32 kHz by auditory brain stem response testing for 16 to 25 weeks after surgery. Histologic morphometry after the animals were kill …

Triple Canal Occlusion for the Treatment of Intractable Menière's Disease.

OBJECTIVE: Menière's disease can be a debilitating condition but in most cases the symptoms are controlled by lifestyle changes and medical management. However, some patients remain symptomatic despite medical treatment and have the option of more invasive surgical treatments. Surgical intervention for Menière's includes a range of interventions from grommet insertion, intratympanic steroids/Gentamicin, endolymphatic sac decompression, labyrinthectomy, and vestibular neurectomy. A recently described technique involves the occlusion of all three semi-circular canals as an alternative in intract…

[The etiology of Menière's attacks and their consequent physiologic aspects].

On the basis of theoretical considerations and experimental studies, it can be shown that Menière's attacks result from rupture of the membranous labyrinth with diffusion of potassium into the perilymph and sodium into the endolymph.

SOX9 and SOX10 control fluid homeostasis in the inner ear for hearing through independent and cooperative mechanisms.

The in vivo mechanisms underlying dominant syndromes caused by mutations in SRY-Box Transcription Factor 9 (SOX9) and SOX10 (SOXE) transcription factors, when they either are expressed alone or are coexpressed, are ill-defined. We created a mouse model for the campomelic dysplasia SOX9Y440X mutation, which truncates the transactivation domain but leaves DNA binding and dimerization intact. Here, we find that SOX9Y440X causes deafness via distinct mechanisms in the endolymphatic sac (ES)/duct and cochlea. By contrast, conditional heterozygous Sox9-null mice are normal. During the ES development…

Vestibular nerve section in a child with intractable Menière's disease.

Clinical presentation of Menière's disease in children is not as typical as in adults. The triad of vertigo, tinnitus and deafness are not usually elicited, diagnosis often being made after years of follow up and batteries of investigation. A case of Menière's disease in a 3-year-old boy is presented. The diagnosis was only obvious at the age of 8 when the triad of vertigo, deafness and tinnitus were present. His disease progressed despite a trial of intratympanic gentamicin injections and endolymphatic sac decompression. Vestibular nerve section was subsequently performed for his intractable …

Possible involvement of free radicals in lipopolysaccharide-induced labyrinthitis in the guinea pig: a morphological and functional investigation.

Intratympanic injection of bacterial lipopolysaccharide impaired caloric responses and caused severe and widespread morphological damage to vestibular end organs and the endolymphatic sac in the guinea pig. These effects could be blocked with N-nitro-L-arginine methylester, a competitive inhibitor of nitric oxide synthase, with superoxide dismutase, an O2 scavenger, with dexamethasone, and with ebselen, a scavenger of peroxynitrite. These observations indicate that enhanced nitric oxide and superoxide production, resulting in formation of peroxynitrite, is probably an important factor responsi…

Mondini dysplasia--late complications.

Mondini's deformity is the second most common malformation seen in congenital genetic deafness. It is characterized by bony and membranous abnormalities of the inner ear, with a wide range of morphological and functional abnormalities. The importance of the Mondini malformation is that the patient is at an increased risk of developing meningitis or bilateral total hearing loss (or both) at an early age. Six cases are presented which illustrate the various sequelae of the Mondini malformation including meningitis, vertigo, and auditory deterioration. Decompression of the endolymphatic sac was u…

Distal Renal Tubular Acidosis With Sensorineural Deafness in a Saudi Female: A Case Report of an ATP6V1B1 Mutation in a Consanguineous Family.

Distal renal tubular acidosis (dRTA) with sensorineural deafness is a rare entity inherited in an autosomal recessive manner caused by mutations in the ATP6V1B1 gene leading to defective acidification function in the distal nephron, cochlea, and endolymphatic sac. We report the case of an 11-year-old Saudi girl with dRTA and congenital sensorineural hearing loss. Genetic testing revealed a homozygous mutation in the ATP6V1B1 gene (c.1037C>G; p.P346R). Both parents were heterozygous carriers. This case highlights the clinical and genetic features of dRTA in a consanguineous family and underscor…

A Case of Post-Traumatic Meniere's Disease.

There are only a few reports of post-traumatic Meniere's disease and there is few literature that contains detailed data associated with the disease. We report a case of post-traumatic Meniere's disease. He suffered from tinnitus, fluctuating sensorineural hearing loss, and recurrent vertigo. Symptomatic medical treatment was not helpful and neither was soft tissue plugging around the oval and round windows during exploratory tympanotomy. Three months after soft tissue plugging, endolymphatic sac decompression surgery was performed. The patient's symptoms improved markedly thereafter. The clin…

The vein of the vestibular aqueduct with potential pathologic perspectives.

HYPOTHESIS: Pathologic changes around the vein of the vestibular aqueduct (VVA) may cause obstruction to the flow of blood toward the sigmoid sinus. Furthermore, a distal obstruction of this vessel may be responsible for a development of a retrograde flow of blood with concomitant drainage of endolymphatic sac (ES) substances to the inner ear. BACKGROUND: The VVA is responsible for the venous drainage of the vestibular apparatus and endolymphatic duct and ES. Previous studies have linked the VVA to Ménière's disease. The aim of the present article was a 3-dimensional perspective study of the …

Sac decompression for refractory luetic vertigo.

Luetic hydrops can occur in both congenital and acquired lues and otologic manifestations may be indistinguishable from Meniere's disease. Eight endolymphatic sac decompression procedures were performed on 6 patients with refractory luetic vertigo. Of those, 3 had congenital and 3 acquired lues. All cases had received extensive anti-luetic medical therapy without any discernible improvement in the vertiginous attacks. In our 6 patients with luetic hydrops the main complaint was severe refractory vertigo, indeed, disabling in most cases. Postoperatively, all cases remained free from these sever…
