[Methodical and critical analysis of the determination of target volumes in case of hypopharyngeal and epiglottic tumors and the adaption of the dose distribution to their dimensions - Part 2 (author's transl)].

Seventy-five patients with a hypopharynx or epiglottis carcinoma were treated between 1954 and 1974 at the Radiologic Hospital of the University of Heidelberg. The clinical study of these cases provides a survey of the extension and formation of metastases of these tumors. This allows to establish empirically justified target volumes for hypopharyngeal and epiglottic tumors.

Treatment of cancer of the base of the tongue and glosso-epiglottic region: a multicenter Italian survey. GLOCC Group. Gruppo di Lavoro in Oncologia Cervico Cefalica.

BACKGROUND: The current treatment options for cancer of the base of the tongue and glosso-epiglottic region are surgery, radiotherapy, or a combination of both modalities. Comparisons between different modalities are not common in the literature, and a real standard of treatment has not yet been established. The purpose of our study was to evaluate the results of treatment in a large series of patients from 18 Italian institutions in relation to the main treatment adopted. METHODS: The present study is a retrospective survey. The series was divided into a combined surgery group and a radiothe

[Methodical and critical analysis of the determination of target volumes in case of hypopharyngeal and epiglottic tumors and the adaptation of the dose distribution to their dimensions.--Part 1 (author's transl)].

After having discussed the problem of optimization with regard to the dose distribution in space, the authors enter into the particulars of the classification of hypopharyngeal and epiglottic tumors according to the TNM system. Taking into consideration the communications of literature and the cases of the Radiologic Hospital of the University of Heidelberg, the authors describe the clinical manifestations of these tumors, their histology, their distribution to age and sex, their symptomatology and time of anamnesis as well as the therapeutic results.

Refractory and Relapsing Laryngeal Edema Possibly Associated With Chronic Tonsillitis and Mycoplasma Infection, Requiring Reintubation and Tracheostomy.

Relapsing epiglottitis has rarely been reported, and its etiology is not well established. A 44-year-old previously healthy Japanese man presented with a quickly progressing choking sensation. He had been experiencing refractory and relapsing laryngeal edema and probably acute epiglottitis (three episodes within 2 weeks), with rash and elevated pancreatic amylase. The patient required immediate intubation. After the initial extubation, he required reintubation and a subsequent tracheostomy. Antibiotics, glucocorticoid, and antihistamines were administered, and he finally recovered with the tra…

Beware of thermal epiglottis! A case report describing 'teapot syndrome'.

BACKGROUND: The type of scalding injury known as 'teapot syndrome', where hot liquid is grabbed by the child with the aim of ingestion and falls over a child causing burns on the face, upper thorax and arms, is known to cause peri-oral and facial oedema. Thermal epiglottitis following scalds to face, neck and thorax is rare and can occur even in absence of ingestion of a damaging agent or intraoral burns, Awareness of the possibility of thermal epiglottitis, also in scald burns, is imperative to ensure prompt airway protection. CASE PRESENTATION: We report the case of a child with thermal epi…

[Investigation on invasion of preepiglottic space by supraglottic carcinoma].

This study, based on analysis of 100 whole-organ serial sections of the larynx, concluded that preepiglottic space invasion is involved in 60% of supraglottic carcinoma. The preepiglottic space is involved either by invasion through the adjacent structures of the epiglottis, or more frequently by destroying the epiglottic cartilage and the thyro-epiglottic ligament. The preepiglottic space is involved mainly by epiglottic and epiglottic-ventricular carcinomas, accounting for 95% of carcinomas which invade the space. The involvement of preepiglottic space by supraglottic carcinoma is intimately

Upper airway obstruction in a young adult.

Acute epiglottitis is a potentially rapidly fatal illness. Although commonly thought of as a disease affecting children, it is increasingly being diagnosed in adults. We report a case of upper airway obstruction due to epiglottitis in a 37-year-old female. The need for early diagnosis because of the risk of a fatal outcome is highlighted.

[Laser surgery of supraglottic laryngeal carcinoma].

The limited excision addresses small superficial lesions affecting free edge of the epiglottis, the aryepiglottic fold or the superior edge of the arytenoid. The median supraglottic laryngectomy excluding the pre-epiglottic space addresses small superficial T1 lesions of the endolaryngeal epiglottis. The incision line extends to the pre-epiglottic space but does not aim at removing the pre-epiglottic space entirely. The pharyngo-epiglottic folds, aryepiglottic folds, and ventricular folds are preserved. The median supraglottic laryngectomy including the pre-epiglottic space addresses T1 and T2

Pregame Sore Throat, Postgame Intensive Care Unit.

A collegiate football athlete presented, on game day, with an acute onset of sore throat. He was afebrile, speaking in full sentences, without signs of respiratory distress. His examination was negative for lymphadenopathy or tonsillar enlargement or exudate. Twelve hours after initial presentation, he developed acute epiglottitis. He underwent urgent fiberoptic intubation and was empirically treated with broad-spectrum antibiotics and corticosteroids. Currently, there are no published reports of acute epiglottitis in athletes. Consequently, there is no evidence to guide return to play decisio…

Intra- and interobserver reliability estimates for identification and grading of upper respiratory tract abnormalities recorded in horses at rest and during overground endoscopy.

BACKGROUND: Previous studies support good intra- and interobserver agreements for endoscopic evaluation of various upper respiratory tract (URT) diseases in horses. However, these studies mainly assessed resting endoscopic examination videos and/or focussed on a single URT abnormality. OBJECTIVES: To estimate intra- and interobserver agreement for identification and grading of all URT abnormalities from resting and overground endoscopy (OGE) videos of Thoroughbreds. STUDY DESIGN: Blinded, fully crossed design. METHODS: Resting and OGE URT videos for n = 43 Thoroughbreds were retrospectively

The role of the epiglottis in the swallow process after a partial or total glossectomy due to a neoplasm.

Repeatable epiglottic movement patterns were recorded during a videofluoroscopic swallow evaluation of 95 patients who had undergone a total or partial glossectomy due to a neoplasm. Because no epiglottic function assessment was performed preoperatively, for the purpose of this study it was assumed that epiglottic mobility was "normal" during this time and that all abnormalities found afterward resulted from the growth of the neoplasm and the glossectomy. It was noted that in the early postoperative period, absence of epiglottic movement was accompanied by aspiration and made swallowing incomp

[Acute respiratory insufficiency in children].

Acute respiratory distress in children is often a consequence of asthma. Other causes are subglottic laryngitis, epiglottitis, aspiration of a foreign body, acute bacterial pneumonia or pneumothorax. History and physical examination should differentiate between the various diseases. Asthma is characterized by recurrent symptoms and signs, while this is not the case with the other causes of acute breathlessness described. An asthma exacerbation is often preceded by one or more prodromes. In case of aspiration of a foreign body, like a peanut, immediate action is needed to prevent irreversible d…

A review of Haemophilus influenzae infections in Cambridge 1975-1981.

Invasive Haemophilus influenzae infections diagnosed in Cambridge between January 1975 and December 1981 are reviewed. Altogether, 81 sites in 77 patients were infected. Of these patients, 41 had meningitis, 20 epiglottitis, 8 bone or joint infections (2 with concurrent meningitis), 4 cellulitis, 5 pneumonia (2 with concurrent epiglottitis) and 3 septicaemia in the absence of documented localised infection. Most patients (88%) were children less than 5 years of age. Only 8 adults with such infections were identified. Of these, 6 had an identifiable predisposing condition. The incidence of meni…

Epiglottis Collapse in Individuals With Obstructive Sleep Apnea.

OBJECTIVE: Obstructive sleep apnea (OSA) is a common sleep disorder characterized by repeated upper airway collapse during sleep. The epiglottis, often overlooked in conventional assessments, has emerged as a site of significant obstruction in OSA. This review aims to highlight current knowledge of the epidemiology, pathophysiology, diagnosis, and treatment of epiglottic collapse in adults with OSA. DATA SOURCES: PubMed, Embase, Web of Science, and Cochrane literature search. METHODS: We conducted a detailed review of publications related to epiglottic collapse, specifically on study design,

Adult supraglottitis. A prospective analysis.

Epiglottitis in pediatric patients is an infection caused by Haemophilus influenzae type b, which can lead rapidly to sepsis and an asphyxial death. In an effort to study the cause and clinical course of adult epiglottitis, eight serially hospitalized adult patients with supraglottitis over a ten-month period were prospectively evaluated, including a daily laryngeal examination. Although multiple anatomic sites in the larynx and oropharynx were inflamed, the epiglottis was often not the most involved area and was actually normal in one patient. Bacterial cultures were harvested from blood, the…

Infective Uvulitis in a Child.

Infective uvulitis is a rare condition in children. In this report, we describe the case of a 4-year old-patient who presented a group A Streptococcus pharyngitis with uvulitis. No signs of epiglottitis were detected at nasal fibroscopy. She recovered rapidly with intravenous antibiotic therapy and 2 days of corticosteroid. Uvulitis is usually caused by group A Streptococcus or Haemophilus influentiae, but also other bacteria can be detected. Uvulitis can be isolated, or it can occur with epiglottitis and become an emergency.

Unusual infections caused by Pasteurella multocida.

Three cases of infection with Pasteurella multocida included acute epiglottitis, septic arthritis, and pleuropulmonary infection. Human infections with this microorganism are commonly related either to animal contact (eg, wound infections) or to the presence of underlying chronic respiratory tract disease. Although septic arthritis and pleuropulmonary infections have previously been attributed to this pathogen, to our knowledge, this is the first reported case of acute epiglottitis caused by P. multocida.
