Physical disability in Late Antiquity Milan: slipped capital femoral epiphysis with severe secondary joint disease in the Basilica of San Dionigi.

The paper presents the skeletal remains of an adult male of 30-40 years with bone lesions and deformity on the left hip, recovered during the archaeological excavation below the former Basilica of San Dionigi, dated to Late Antiquity (3rd - 5th century AD) Milan. Biological profile and paleopathological analysis were performed following standard references and the bones underwent radiological examination. Differential diagnosis included congenital anomaly, active rickets, infectious diseases, femoral neck trauma, Legg-Carvé-Perthes disease, Slipped Capital Femoral Epiphysis (SFCE), osteogenesi

Patients' delay is the major cause for late diagnosis of slipped capital femoral epiphysis.

Early diagnosis of slipped capital femoral epiphysis (SCFE) is essential in order to reduce slip severity and subsequent risk of sequelae. The aims of this study were to evaluate patients' and doctors' delay in SCFE diagnosis and to identify possible factors leading to delay. We performed a retrospective review of medical charts and a personal interview with 54 consecutive patients admitted with a diagnosis of stable SCFE at three hospitals in Sweden between 2001 and 2009. Data on symptom duration, symptomatology, medical visits and type of medical contacts were retrieved. Slip angle (Southwic

Slipped Capital Femoral Epiphysis: Early Intervention and Referral.

Slipped capital femoral epiphysis is the most common hip pathology in children aged 8-15 years old. Research has shown that when a nonorthopedic provider evaluates this patient population, there can be a significant delay in the appropriate treatment, which may have serious consequences for the prognosis of the patient. The delays are often caused by the practitioner's inability to put the clinical picture into focus with regard to how these patients typically present.. This article presents the demographics, clinical presentation, differential diagnosis, radiological and physical examination

Pediatric Hip and Pelvis.

The most common pediatric orthopedic conditions of the hip and pelvis involve abnormal architecture of the joint leading to pain and dysfunction. Developmental dysplasia of the hip and femoroacetabular impingement are 2 common and distinct forms of structural pathology in the pediatric hip. The authors also discuss 2 of the more common, and often questioned, pediatric hip disorders-slipped capital femoral epiphysis and Legg-Calvé-Perthes disease. Future investigations are aimed at identifying risk factors to provide pediatric orthopedists tools to risk stratify their patients and understand wh

[Epiphyseolysis of the femoral head: new aspects of diagnostics and therapy].

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is the most common hip disease in adolescents and is always surgically treated with the aim to avoid further slippage and to reduce the risk of degenerative arthritis at young age. OBJECTIVES: A summary of the etiology, pathogenesis, clinical features, radiographic imaging and current therapy concepts is given. MATERIAL AND METHODS: A selective review of the literature was performed. RESULTS: With an increasing body mass index in adolescents the incidence of SCFE also increases. The diagnostic routine is comprised of a clinical examinatio

A treatment algorithm for stable slipped capital femoral epiphysis deformity.

The authors' current experience in the surgical treatment strategy of stable slipped capital femoral epiphysis deformities was reviewed. From this, a treatment algorithm was developed that could be utilized as a guide in the evaluation and treatment of future patients with slipped capital femoral epiphysis. The clinical parameters of patients' histories of symptoms, physical examinations, and radiographic assessments of slip severity were used in formulating the algorithm. The intent was to prepare a comprehensive algorithm providing necessary alternate treatment pathways for the variable slip

[Traumatic distal humerus epiphysiolysis in a newborn child].

Traumatic distal humerus epiphysiolysis (TDHE) is a rare injury in infants with an incidence of about 1:35,000 births. It is primarily a birth injury, but it is also seen in cases of battered child syndrome. Because of its rare occurrence and the diagnostic difficulties, the lesion may be overlooked or misdiagnosed on initial presentation. The diagnosis is made on the basis of a high index of suspicion, clinical signs and symptoms and awareness of radiological changes. Traumatic humeral dislocation will produce the same radiological picture as TDHE and is consequently a common, incorrect initi

MRI morphometry, cartilage damage and impaired function in the follow-up after slipped capital femoral epiphysis.

OBJECTIVE: To assess rotation deficits, asphericity of the femoral head and localisation of cartilage damage in the follow-up after slipped capital femoral epiphysis (SCFE). MATERIALS AND METHODS: Magnetic resonance imaging studies were obtained in adult patients with a history of SCFE. A total of 35 hips after SCFE in 26 patients (mean age 24.1 +/- 6.5, mean follow-up 11.9 +/- 6.1 years) were evaluated. The control group comprised 20 healthy hips from 10 young adults with an average age of 23.9 +/- 3.7 years. The MR protocol included a T1-weighted sequence with a 3D volumetric interpolated b

Atypical slipped capital femoral epiphysis after radiotherapy and chemotherapy.

Atypical slipped capital femoral epiphysis after radiotherapy and chemotherapy is uncommon. Only 32 cases have been reported in the literature. Because patients may have slippage at atypical ages, we report two cases of slipped capital femoral epiphysis in children and review the 32 cases previously reported to heighten clinicians' awareness of this condition in patients who have received radiation and chemotherapy for pelvic tumors. The controversy over prophylactic pinning of the uninvolved hip in radiotherapy-associated slipped capital femoral epiphysis is unresolved. It may be justifiable

Hip arthroscopy in the pediatric population.

Hip arthroscopy has obvious advantages over arthrotomy in the pediatric population. Hip arthroscopy, used as a diagnostic or therapeutic tool, is significantly less invasive than arthrotomy and allows for quicker recovery and return to activities. In addition, arthroscopy avoids dislocation of the femoral head and the corresponding risk of osteonecrosis. Current indications for hip arthroscopy in pediatric patients include septic arthritis, labral disorders, slipped capital femoral epiphysis, and Legg-Calvé-Perthes disease. The role of hip arthroscopy in the pediatric population will continue

Three-dimensionally corrective external fixator system for proximal femoral osteotomy.

The authors introduce a technique of proximal femoral osteotomy using three-dimensionally corrective external fixator systems for treatment of Legg-Calvé-Perthes disease and slipped capital femoral epiphysis, and describe the results of 47 hips in 44 children. Postoperative casting immobilization was not performed. All osteotomies gained the correction angle as planned. All osteotomies healed with no nonunions, malunions, device failures, or avascular necrosis. Complications occurred in 7 (15%) of 47 hips. Pin tract infection occurred in six (13%) hips; one of these (2%) needed debridement. Th

Bilateral slipped capital femoral epiphysis occuring in an adult with acromegalic gigantism.

The etiology of slipped capital femoral epiphysis is still unknown. Traumatic, endocrine, toxic, and mechanical causes have all been hypothesized. It is well documented that the highest incidence occurs during the adolescent growth spurt, suggesting the role of an endocrine abnormality. We report a case that supports this hypothesis.

Chondrolysis after slipped capital femoral epiphysis.

Chondrolysis, or acute cartilage necrosis, is a controversial and perplexing complication of slipped capital femoral epiphysis (SCFE) in adolescents. It usually leads to deformity, pain, limping, and limitation of motion in the affected extremity. Although several theories have been proposed, no agreement has been reached as to the etiology or the pathogenesis of chondrolysis. Furthermore, no treatment program has been completely successful, and the general prognosis and natural history of this condition are not clear. An overview of the latest knowledge of the matter is presented.

Bone endoscopy: direct visual confirmation of cannulated screw placement in slipped capital femoral epiphysis.

Intraosseous endoscopic examination of the femoral neck and head was performed during the course of percutaneous screw fixation in 12 patients with slipped capital femoral epiphysis (SCFE). Visualization was satisfactory in 13 hips of these patients. We were able to distinguish trabecular bone and physeal cartilage endoscopically. Evidence of articular penetration was documented endoscopically in two patients. One patient had been referred for persistent hip pain and chondrolysis 6 months after screw insertion. Chronic joint penetration was observed endoscopically at the time of revision opera

Femoral neck fracture secondary to in situ pinning of slipped capital femoral epiphysis: a previously unreported complication.

Two patients developed femoral neck fractures as a complication of in situ pinning for slipped capital femoral epiphysis (SCFE) by surgeons who each used a cannulated screw system. Both patients exhibited recurrent hip pain, femoral neck fracture, and coxa vara after asymptomatic postoperative intervals of 2 and 6 months, respectively. The fracture in one patient healed with weight relief alone: the other persisted, requiring a vascularized pedicle bone graft. Bone biopsy at surgery disclosed avascular necrosis (AVN). Based on preliminary studies of heat production during reaming, we speculate
