Cerebral amebic encephalitis due to Balamuthia mandrillaris is a rare yet typically fatal disease. As such, identification of the clinical characteristics, appropriate diagnostic workup and commencement of treatment is frequently delayed. Here, we present a case of a 4-year-old male with a B. mandrillaris cerebral abscess successfully treated with expedited neurosurgical resection and broad-spectrum antimicrobial therapy.

A 16-year-old with new-onset dilated cardiomyopathy underwent VAD placement, later complicated by low flow from outflow graft kinking. To expedite heart transplantation, TCV was calculated and compared with 141 normal patients pinpointing the upper weight threshold. He was transplanted 2 days later within the expanded weight range with no post-transplant complications.

Upper chest and shoulder varicosities, although uncommon compared with lower extremity varicosities, are a harbinger of venous obstruction and should expedite the use of contrast venography in patients undergoing cardiac device implantation. We present the first reported case, to our knowledge, of a patient having isolated left upper chest and shoulder varicosities prior to permanent pacemaker implantation.

In situ homeotropic alignment is achieved by photochromic trans- to cis-isomerization of an azo-dye doped in a nematic host. The augmented dipole moment of the cis-isomer formed under UV-irradiation expedites molecular assembly into crystalline aggregates. Subsequent deposition of the aggregates creates a roughened surface and induces an anchoring transition from the initial planar to a homeotropic alignment of the LCs.

Inverting serosa-to serosa staple applications may lead to intraluminal extrusion of staples. The possible clinical pathological consequences of such a migration are examined. Technical maneuvers designed to prevent errors and expedite safer handling of automatic stapling devices are presented. Delayed hemolytic transfusion reactions (DHTRs) occur secondary to slow, mild IgG-mediated processes against minor red blood cell antigens. Herein, we report the case of a rapidly fatal alloimmune anti-c IgM-mediated hemolysis, a rare, previously undescribed, pathophysiologic scenario. Early recognition of such phenomena can expedite supportive measures and optimize patient outcomes.

This review summarizes the most important features of different mycobacterial species, and addresses the issues of specimen collection and shipment, bacteriological diagnosis of tuberculosis and other mycobacterial infections, and drug susceptibility testing of different mycobacteria. Special emphasis is placed on the expected turnaround time for the various laboratory reports from different methods. The potential of new methods of today and tomorrow to expedite laboratory results are discussed.

We have presented a case of diffuse mixed lymphocytic and histiocytic lymphoma that appeared as persistent oral ulcerations in a pituitary dwarf with combined variable immunodeficiency. The difficulty in diagnosis is emphasized and the differential features of polymorphic reticulosis, Wegener's granulomatosis, malignant lymphoma, and idiopathic midline destructive disease have been reviewed. Awareness of these disease processes and their distinguishing features may expedite accurate diagnosis.

Immune-mediated necrotizing myopathy (IMNM) is an autoimmune myopathy typically characterized by a subacute-onset, rapidly progressive proximal predominant weakness, markedly elevated creatine kinase (CK) levels, and myopathological features of necrotic and regenerating fibers with minimal or no lymphocytic infiltration. BACKGROUND: To determine whether patients with no alarm signs who ask the endoscopist to shorten their waiting time due to test result anxiety, represent a risk category for a major organic pathology.
METHODS: At our open-access endoscopy service, we set up an expedite list for six months for outpatients who complained that the waiting time for gastroscopy was too long. Over this period we studied 373 gastroscopy patients. Dural Venous Sinus Thrombosis (DVST) is a sporadic cause of headache. DVST is a recherché complication of maxillary sinus infection. Maxillary sinusitis infection may spread directly to orbit via lamina papyracea and it is expedited by the presence veins of breschet. The authors present a clinical case of dural sinus thrombosis with protein S deficiency and also describe an effective management approach for DVST.

Evaluation of anogenital lesions in children is a challenging problem. The differential diagnosis includes a spectrum of traumatic, infectious, neoplastic, and genetic disorders. The possibility of sexual abuse should always be considered and investigated as indicated. A systematic work-up will expedite the correct diagnosis and limit unnecessary anxiety for parents, children, and physicians. Referral to a center that provides specialized service for the evaluation and treatment of these disorders may be indicated.
