Do endoflip assessments of anal sphincter distensibility provide more information on patients with fecal incontinence than high-resolution anal manometry?

BACKGROUND: Anal manometry is the standard technique for evaluating anal sphincter function. However, the functional lumen imaging probe (EndoFLIP(®) ) can be used to measure sphincter distensibility during volume-controlled distensions. Our aims were (i) to assess anal distensibility in patients with fecal incontinence (FI) and in healthy subjects using the EndoFLIP(®) and (ii) to compare the results with anal pressures measured by 3D high-resolution manometry (3D-HRM) to determine whether the EndoFLIP(®) was more sensitive and specific for diagnosing FI than 3D-HRM. METHODS: EndoFLIP(®) and…

Prospective clinical audit of two neuromodulatory treatments for fecal incontinence: sacral nerve stimulation (SNS) and percutaneous tibial nerve stimulation (PTNS).

BACKGROUND AND PURPOSE: Two types of neuromodulation are currently practised for the treatment of fecal incontinence (FI): sacral nerve stimulation (SNS) and percutaneous tibial nerve stimulation (PTNS). This study compares these therapies, as no data exist to prospectively assess their relative efficacy and costs. METHODS: The subjects of this study were two distinct cohorts undergoing SNS (between 2003 and 2008) or PTNS (2008-onwards) for FI. Clinical outcomes assessed at 3 months included incontinence scores and the number of weekly incontinence episodes. The direct medical costs for each …

Effects of transcutaneous tibial nerve stimulation on anorectal physiology in fecal incontinence: a double-blind placebo-controlled cross-over evaluation.

BACKGROUND: Transcutaneous electrical tibial nerve stimulation (TENS) is of growing interest for the treatment of fecal incontinence (FI), but its mechanism of action remains uninvestigated. We aimed to further assess the anorectal response to TENS in a dynamic model. METHODS: We performed a placebo-controlled, randomized, double-blinded crossover study in 19 patients suffering from FI to assess the effects of TENS on anorectal function. Anorectal physiology and perception were recorded through two sequences of rectal isobaric distension using an electronic barostat device to measure anal and…

Comparative study between partial and complete lateral anal sphincterotomy in treatment of chronic anal fissure and its effect on fecal incontinence.

BACKGROUND: Chronic anal fissure is a painful condition that can greatly affect quality of life. When conservative treatment fails, lateral internal anal sphincterotomy is performed to promote healing. The procedure may involve partial or complete sphincter division, which can influence the risk of fecal incontinence. This study aimed to compare the outcomes of tailoring the length of the lateral internal anal sphincter with either the whole length or the fissure apex and their effect on fecal continence in the treatment of chronic anal fissure. METHODS: This comparative, prospective, observa…

Daily Experiences of Urinary and Fecal Incontinence in Young Adults with Spina Bifida: Preliminary Results from an Ecological Momentary Assessment Study.

BACKGROUND: Urinary (UI) and fecal (FI) incontinence are prevalent secondary chronic conditions among young adults with spina bifida (YASB). UI and FI decrease daily functioning for YASB, but no research has prospectively examined characteristics of UI and FI among YASB. We used ecological momentary assessment (EMA) over 30 days to describe the prevalence, episode-specific characteristics and negativity associated with UI and FI among a cohort of YASB. METHOD: Data were collected as part of a larger 30-day EMA study prospectively examining the daily prevalence and context of UI and FI in adul

Comparative outcomes of sphincteroplasty and sacral neuromodulation in postmenopausal women with late-onset fecal incontinence following obstetric trauma: a retrospective study.

Fecal incontinence (FI) is a debilitating condition that commonly affects postmenopausal women, particularly those with a history of obstetric trauma. To date, there are few studies directly comparing SP and SNM, which present many discrepancies in the inclusion criteria, such as age, etiology, and elapsed time from the onset of the condition. This retrospective study aims to compare the clinical outcomes of the two procedures most widely performed in the management of FI: sphincteroplasty and sacral neuromodulation (SNM). The inclusion criteria were established to ensure a representative samp…

Electroacupuncture Improves Anorectal Function in Neurogenic Fecal Incontinence Rats, Related to Reversing Anal Sphincter Atrophy and Enhancing Muscle Satellite Cell Activity.

BACKGROUND: Neurogenic fecal incontinence (NFI) is a disorder of anal dysfunction caused by nerve damage. Electroacupuncture (EA), grounded in Traditional Chinese Medicine (TCM) theory, has demonstrated efficacy in treating various degenerative disorders and is extensively utilized in the clinical management of neurogenic FI. The proliferation and differentiation of muscle satellite cells (SCs) are crucial in the onset and progression of this condition. However, the specific mechanism of electroacupuncture in treating neurogenic fecal incontinence is not yet clear. This study sought to determi…
