The ligaments of the proximal row of carpal bones and the triangular fibrocartilage (TFC) strongly influence the function and stability of the wrist. Injury to the ligaments may result in chronic wrist pain or instability. Wrist arthrography is valuable in the investigation of such damage when surgical intervention is considered and plain radiography is unrewarding. There are also several technical modifications of the standard radiocarpal arthrography available. Owing to the possibility of congenital perforations and degenerative changes in these ligaments the arthrographic findings should be…

Arthrography of the wrist joint as a dynamic examination aids in confirming lesions of interosseous ligaments of the triangular fibrocartilage complex and soft tissue. The results of arthrography in 185 wrists, classified according to age of patients and pathological disorders with or without trauma demonstrate mostly post-traumatic discus lesions in younger patients. Older patients have an increase in pathological findings with a similar distribution between ligaments and discus. Soft-tissue injuries or discus lesions can be confirmed or excluded before osteoplastic surgery for distal radius …

Articular cartilage plays a vital role in joint morphology. An understanding of articular cartilage anatomy and physiology will enable the p … efects can heal spontaneously, if the defect extends to subchondral bone. The reparative substance, fibrocartilage, is less durable and much less smooth. There are many techniques and procedures in which chondral or osteochondral defects can be filled. Promoting subchondral bleeding is the method most commonly used clinically. This allows pleuripotent cells to f …

Distal radioulnar joint instability is commonly seen after surgical fixation of a distal radial fracture, and surgeons' ability to reliably determine stability on examination is poor. Debate remains regarding whether to fix the ulnar styloid or reinsert the triangular fibrocartilaginous complex. Four surgeons with Level 5 expertise were asked to respond to questions surrounding this debate.

OBJECTIVE: The purpose of this study was to identify the location of lunate chondromalacia and to compare the difference in location according to presence or absence of triangular fibrocartilage complex (TFCC) foveal tear, which induces distal radioulnar joint (DRUJ) instability.
METHODS: We performed a retrospective study of 97 patients (102 wrists) who were diagnosed with and treated for idiopathic ulnar impaction syndrome (UIS) between 2014 and 2018. Subjects were divided into two groups according to presence or absence of TFCC foveal tear with DRUJ instability. Group I had UIS without a T…

Focal fibrocartilaginous dysplasia is an uncommon, benign bone lesion that causes deformity of the long bones in young children. It has most commonly been encountered in the proximal tibia, and very rarely in the long bones of the upper limb, that is, the proximal humerus, distal radius, ulna and proximal phalanx. Only one case of focal fibrocartilaginous dysplasia of the proximal humerus has been reported previously. The present study reports two such additional cases that were diagnosed in late childhood. The clinical presentation and radiographic findings are described with an emphasis on t

Distal radioulnar joint (DRUJ) instability is a complex pathology that typically occurs due to injury of the triangular fibrocartilage complex. In the acute setting, repair is often successful. However, in the chronic setting or after a prior failed repair, reconstructive methods aim to improve stability using an extrinsic radioulnar soft tissue link. Traditional surgical techniques are technically demanding and require a broad exposure, compromising extrinsic stabilizers. Modifications to these methods using arthroscopy facilitate preservation of ulnocarpal and DRUJ secondary stabilizers, whi…

Wrist arthroscopy is mainly used to assist fracture reduction and fixation and to diagnose and treat concomitant injuries mainly to the scapholunate (SL), lunotriquetral (LT) ligament and the triangular fibrocartilage complex (TFCC). Arthroscopy is beneficial in improving anatomical reduction of fracture steps and gaps in intra-articular distal radius fractures (DRFs). Yet, the literature that the functional outcome correlates with the use of arthroscopy, is limited. Non-surgical treatment and immobilization is recommended for Geissler grade I-III Sl-ligament injuries, while open reduction, li…

Cartilage-derived pluripotent cells reside in hyaline cartilage and fibrocartilage. These cells have the potential for multidirectional differentiation; can undergo adipogenesis, osteogenesis, and chondrogenesis; and have been classified as mesenchymal stem cells (MSCs) conforming to the minimal criteria of the International Society for Cellular Therapy. Cartilage tissue is prone to injury and is difficult to repair. As cartilage-derived pluripotent cells are the closest cell source to cartilage tissue, they are expected to have the strongest ability to differentiate into cartilage compared to…

The distal radioulnar joint, the triangular fibrocartilage complex (TFCC) and the wrist are stabilized by many ligaments allowing not only a wide range of motion but also providing sufficient stability. The complex arrangement of carpal ligaments and prosupination around the forearm joint enables multiaxial motion patterns. In the wrist, intra-articular ligaments can be differentiated from extra-articular capsular ligaments as well as intrinsic and extrinsic ligament courses. Imaging is essential for classification of dynamic and static instability patterns. This review article illustrates the…

Chondrocalcinosis refers to deposition of calcium pyrophosphate crystals within cartilage or fibrocartilage, as visualised on plain radiograph. Clinical features of chondrocalcinosis are various. The two common presentations of chondrocalcinosis are acute synovitis (pseudogout) and chronic arthritis, which can lead to a severe disability. Sporadic form of the disease is by far the most frequent. Aging is the main risk factor for the occurrence of sporadic chondrocalcinosis. Prevalence of chondrocalcinosis varies from 7 to 10% in people aged around 60 years old. A primary metabolic disorder or …

Ulnar-sided wrist pain has long been a diagnostic problem, partly because of the complex anatomy and many possible causes of pain in this region. This article discusses anatomy, pathophysiology, and imaging appearance of the more common causes, including tears of the triangular fibrocartilage complex, disorders of the distal radioulnar joint, tears of the lunotriquetral ligament, disorders of the extensor carpi ulnaris tendon, disorders of the pisotriquetral joint, the impingement and impaction syndromes, and ulnar wrist masses. Along with clinical history and physical examination, imaging is …

To assess tibialis posterior tendon (TPT) pathology, we investigated 27 feet with the accessory navicular bone and 22 normal feet by MRI. We found two major anatomical differences in the feet with the accessory navicular bone; the TPT directly inserted in the accessory navicular bone, without any continuity to the sole of the foot or with a slip, less than 1 mm in thickness, and there was a mass with the density of fibrocartilage tissue, between the tendon and the bone in 20/27 feet. These abnormalities were not detected in the control group. 3 patients in the study group were operated on and …

To investigate the pathogenesis of hypertrophy of the ligamentum flavum, 45 cases of lumbar canal stenosis were evaluated by computed tomography scan and pathologic and immunohistochemical studies. The ligamentum flavum along with the medial one-third of the superior facet was obtained en bloc to include the enthesis. Statistically significant differences in transverse area and thickness of the ligamentum flavum were evident compared to the control group (P < 0.01). Pathogenesis of the hypertrophied ligamentum flavum was classified into three major groups: 1) fibrocartilage change due to proli…

Lesions of the "discus articularis ulnae" (triangular fibrocartilage) are presented. Possible differences between the left and the right hand, as well as possible age and sex-linked differences were statistically analysed. In 22 wrist the disc was removed. The anatomical and pathological findings correlated with the arthrogram in all cases. Due probably to the advanced age of our study material disc lesions were found to be more frequent than in previous studies. The lesion was less frequent in men than in women. No difference could be shown between the left and the right hand. The arthrograph…
