Slippage of Tightrope Button in Syndesmotic Fixation of Weber C Malleolar Fractures: A Case Series.

BACKGROUND: Tightrope fixation is an emerging technique for syndesmotic fixation with promising results. However, our case series highlights the slippage of Tightrope buttons as a complication of suture button syndesmotic fixation of Weber C malleolar fractures using limited contact dynamic compression (LCDCP) plates. METHODS: We report a series of cases from our database in which slippage of the Tightrope button through the LCDCP holes in Weber C malleolar fractures was noted. We measured the medial clear space (MCS), tibiofibular clear space (TFCS), and distal tibiofibular overlap (DTFO) an

Autologous grafts of double-strut fibular cortical bone plate to treat the fractures and defects of distal femur: a case report and review of literature.

We reported a 23-year-old man who was involved in a high-speed motorcycle accident. He sustained a closed fracture at the right distal femur. The primary fracture happened on February 2008. He underwent open reduction and internal fixation with cloverleaf plate. And one hundred days after the surgery, the proximal screws were pulled-out, but the bone union was not achieved. Treatment consisted of exchanging the cloverleaf plate with a locking compression plate and using an auto-iliac bone graft to fill the nonunion gap. In July 2009, the patient had a sharp pain in the right lower limb. The X-

Posterolateral transfibular approach to tibial plateau fractures: technique, results, and rationale.

We describe a posterolateral transfibular neck approach to the proximal tibia. This approach was developed as an alternative to the anterolateral approach to the tibial plateau for the treatment of two fracture subtypes: depressed and split depressed fractures in which the comminution and depression are located in the posterior half of the lateral tibial condyle. These fractures have proved particularly difficult to reduce and adequately internally fix through an anterior or anterolateral approach. The approach described in this article exposes the posterolateral aspect of the tibial plateau b

Immediate reconstruction of bone and skin defects of the humerus with free fibular graft and muscle flap.

The following Case Report describes the immediate reconstruction of bone and skin defects of the humerus with a free fibular graft and muscle flap. A 33-year-old white female sustained a gunshot wound to the left upper extremity with a 3.08 caliber high-powered rifle. The entry wound was on the lateral aspect of her mid-arm. The exit wound on the anteromedial aspect of the upper extremity measured approximately 10 x 15 cm. A segmental loss of the middle of the humerus and a defect of the radial nerve were noted. Within 48 hours after injury, reconstructive surgery was begun. A free vascularize

Technique Tip: Novel Use of a Targeting Drill Guide for Syndesmotic Screw Placement.

UNLABELLED: Ankle fractures are among the most common traumatic injuries encountered by orthopaedic surgeons, but obtaining anatomic syndesmosis fixation can be difficult. Previous authors have described the centroidal axis of the syndesmosis. Our group has developed a novel technique of aligning the tibia and fibula along their anatomic centroidal axis using a targeting guide, which has showed good results in 1 patient at 1-year follow-up. LEVELS OF EVIDENCE: Level V: Expert opinion.

Increasing Burden of Lower-Extremity Fractures in the Middle East and North Africa (MENA): A 30-Year Epidemiological Analysis.

BACKGROUND: Lower-extremity fractures (LEFs) account for >30% of all skeletal injuries, contributing to the global health and economic burden. Fracture epidemiology in the Middle East and North Africa (MENA) region has been studied little. Health factors and disease epidemiology differ greatly among populations in MENA despite cultural, political, and economic similarities among the region's countries. This study examined the epidemiology of LEFs and the need for rehabilitation in MENA from 1990 to 2019. METHODS: We examined the epidemiology of fractures of the pelvis, hip, femur, patella, ti

Delayed Treatment of a Traumatic Lumbosacral Dislocation With Associated Injuries: A Case Report.

CASE: A 41-year-old woman sustained a degloving injury over her lumbosacral and perineal region with fractures of her right tibia and fibula. After diversion colostomy and osteosynthesis for the fractures at a primary center, a missed grade 2 lumbosacral dislocation was diagnosed at a tertiary center and the degloving injury was treated with debridement and skin grafting. After 5 months, the dislocation had progressed to grade 4 and she underwent delayed posterior lumbosacral reduction, interbody fusion, and L4-S1 fixation, with superior gluteal artery perforator flap and subsequent colostomy

Fibula flap in upper extremity segmental/critical size bone defects fixed with locking plates. Single-institution observational cohort.

INTRODUCTION: Management of bone defects in the upper extremity can vary depending on the size and location of the defect. Large defects may require complex reconstruction techniques. Vascularized bone grafts, mainly free vascularized fibula flap (FVFF), have many advantages in the treatment of bone or osteocutaneous defects. However, complications such as graft fracture are the most common, with a free fibula flap for bone defects in the upper extremity. This study aimed to describe the results and complications associated with the treatment of posttraumatic bone defects in the upper extremit

High fibular fracture in association with triplane fracture: reexamining this unique pediatric fracture pattern.

A case of a high fibular fracture in association with a triplane fracture in a 12-year-old boy is presented and this unique fracture pattern examined. Whether from blunt trauma or an external rotation mechanism, it appears that trans-syndesmotic fixation is not needed to satisfactorily repair this injury. Because of the rarity of the condition, there is limited previously published information describing this particular injury, although surgeons should be aware of it and the underlying mechanism of the fracture.

Effects of Fibular Plate Fixation and Deep Posterior Tibiotalar Ligament Repair on Ankle Stability in a Weber B Fracture Model with Complete Deltoid Ligament Sectioning.

BACKGROUND: Weber B (supination external rotation) ankle fractures with complete deltoid ligament rupture (SER4b) are typically considered unstable and treated with plate fixation of the fibula. Recent studies have suggested a significant ankle-stabilizing role of the deep posterior tibiotalar ligament (dPTTL), which questions current practice. To our knowledge, there are few studies on the biomechanical effects of plate fixation and none on dPTTL repair. Therefore, this study aims to evaluate the effects of fibular plate fixation and dPTTL repair on ankle stability in a Weber B fracture model

Vascularised Fibula Transfer for Post-traumatic Critical Tibial Bone Defects: A Systematic Review.

INTRODUCTION: Managing post-traumatic critical bone defects in the tibia remains challenging. Vascularised free fibula grafts (VFFG) are an attractive option due to their versatility and ability to integrate with host bone. However, they are challenging, and their role compared to alternative techniques, such as bone transport and Masquelet, remains unclear. This study aims to assess the safety and effectiveness of free fibula flaps in reconstructing critical tibia defects following trauma. MATERIALS AND METHODS: Five databases were searched for English-language studies from inception until A

Anatomy of the Sural Nerve in the Posterolateral Approach to the Ankle: A Cadaveric Study.

Sural nerve injury may occur during the posterolateral approach to the ankle during fracture fixation. We aimed to map its location in a posterolateral approach in cadaveric specimens. A posterolateral approach was used in 28 cadaver legs with the incision made halfway between the medial border of the fibula and the lateral border of Achilles tendon, extending proximally from the tip of the lateral malleolus. The sural nerve was identified and the distance from the distal tip of the incision to where it crossed the incision proximally was measured. The mean distance was 3.4 ± 1.2 (range 0.5-7.

[Use of a vascularized fibula for spinal reconstruction in neurofibromatosis].

We report a case of reconstruction of a dysplasic thoracic spine with vascularized fibula in Recklinghausen's disease. We present the case of a thirteen-year-old lady with neurofibromatosis type 1 who developed a dystrophic thoracic kyphoscoliosis. A T9 compression due to a severe scaloping of the thoracic spine caused an important deformation and a medullar compression (with dorsal pain, right sciatica and a pyramidal syndrome). The surgery consisted in medullar liberation by a T9-T10 corporectomy and an osteosynthesis with arthrodesis T3-L3. A free vascularized fibula bone graft, with an end

Staged Treatment for Unstable Open Fracture-Dislocation of the Ankle: A Case Report.

Unstable fracture-dislocation of the ankle is a common lower-extremity injury. Treatment is challenging when the fracture-dislocation is open and cannot be treated with conventional open reduction and internal fixation (ORIF). Immediate ORIF may not be possible for severe, unstable ankle injuries, such as those with ischemic foot because of a poor blood supply caused by soft-tissue injury, or open fracture-dislocation of the ankle with a deltoid ligament rupture. We describe a staged treatment for unstable open fracture-dislocation of the ankle with a deltoid ligament rupture. The first stage

Multiple fractures and impaired bone metabolism in Wolfram syndrome: a case report.

Wolfram Syndrome (WS) is a rare and lethal disease characterized by optic atrophy, diabetes mellitus, diabetes insipidus, and hearing loss. To date, osteoporotic related fractures have not been reported in affected patients. Here, we describe the case of a man affected by WS complicated by several bone fragility fractures. A 50-year-old Caucasian man was hospitalized because of tibia and fibula fractures. His clinical features included diabetes mellitus, diabetes insipidus, optic atrophy and deafness that were consistent with an unrecognized WS diagnosis, which was confirmed by the identificat…
