Anterior dislocation of the proximal interphalangeal joint is a complex injury involving specific pathomechanics. Progression of this injury to a chronic stage is associated with considerable morbidity in the form of pain, instability, and loss of motion in the involved joint. Surgical reconstruction is complicated and results of delayed intervention are compromised. Two cases of anteriorly dislocated interphalangeal joints, unrecognized as such early and allowed to progress to a chronic stage, presented to us as boutonniere deformities. Descriptions of clinical presentation, surgical findings

A 28-year-old female recreational athlete presented with middle finger proximal interphalangeal joint pain, stiffness, and ulnar deviation deformity 2 years after internal fixation of a middle phalanx base fracture-dislocation. Radiographs revealed posttraumatic changes on both sides of the proximal interphalangeal joint. Having failed nonsurgical measures, she elected to proceed with surgical reconstruction. Intraoperatively, we confirmed substantial articular damage on both sides of the joint. We proceeded with hemi-hamate autograft for 80% of the middle phalanx base. We used a cobalt chrome

OBJECTIVES: To explore the effects of different test positions on quantitative muscle strength of wrist and finger flexor muscle groups and to establish a standardized muscle strength test protocol for each muscle group.
METHODS: Forty healthy subjects (12 males and 28 females) were recruited. A portable digital quantitative muscle strength tester, Micro FET2TM, was used to measure the flexor muscle strength of each finger and the wrist joint at the 30° extension, 0° neutral, and 30° flexion, respectively. Palmar abduction strength of the thumb was measured at 30° and 60°, respectively. Ten s

PURPOSE: Dorsal fracture-dislocations of the proximal interphalangeal (PIP) joint can lead to joint incongruity from loss of the buttress function of the middle phalanx volar base. Hemi-hamate arthroplasty can reconstruct the volar articular surface of the middle phalangeal base where repair is not possible. We compared the anatomy of the hamate graft with the middle phalanx base.
METHODS: Forty unique skeletal specimens (40 hamates, 160 middle phalanges) were sampled. Anatomical features relevant to hemi-hamate reconstruction were measured, including the articular surface areas, the axial ri

BACKGROUND: A novel opposition tendon transfer, previously described by Snow and Fink in adults but not in children, can provide better functional results and cosmetic appearance than the currently preferred Huber transfer.
METHODS: Thirty-one children were born either without a thumb (17) or with a hypoplastic thumb (14). Thirty-eight thumbs were classified according to the Blauth classification as stages II, IIIA, IIIB, IV, and V respectively. The opposition tendon transfer was performed between the ages of 4 and 10, using the expendable flexor digitorum superficialis (FDS) tendon from the

This study presents a novel algorithm to measure joint space widths (JSWs) in patients with rheumatoid arthritis (RA) using radiographic hand images. Radiographic images were first preprocessed, and then phalangeal regions corresponding to the bone structures of each finger were extracted using step-wedge functions. Phalangeal branch paths were also extracted. Each of the five extracted phalangeal branch paths matched the bone structures of each finger exactly and ran through the center of each finger. The algorithm automatically detected 14 joints, which were identified as sharp changes in gr

Proximal interphalangeal joint arthroplasty has resulted in good outcomes in patients treated for osteoarthritis, posttraumatic arthritis, and rheumatoid arthritis. Most hand surgeons complete arthroplasties of the proximal interphalangeal joint through a dorsal approach. However, for the past 7 years, we have had positive results with a volar approach. We describe this technique, which avoids injury to the extensor tendon and allows for a more simplified approach to postoperative therapy compared with the therapy regimen required after the dorsal approach.

Locking of a finger is a common condition usually caused by tightness of the fibrous sheaths around the flexor tendons. A case of a locking finger with an unusual traumatic cause and perioperative findings is presented. Different causes of trigger fingers, especially traumatic, reported in the literature are briefly discussed, and the importance of differing between traumatic and nontraumatic causes for locking phenomena is emphasized.

INTRODUCTION: Utensils with built-up handles are often recommended to minimize the required finger joint angles for functional grasping to reduce pain and help individuals with Rheumatoid Arthritis (RA) overcome participation barriers. However, there is a paucity of data describing the impact of built-up handles on range of motion (ROM) requirements of the hand. Therefore, the purpose of this study was to evaluate how built-up handles of varying thickness affect hand ROM in individuals with RA.
METHODS: Thirty-five individuals with RA were instructed to grasp a standard handle spoon, a 1.00 i…

BACKGROUND: The difficulties in surgical treatment of pilon fractures of the finger include fragment reconstitution and posthealing stiffness. In adults, external fixation with traction and early active range of motion (AROM)/passive range of motion (PROM) during healing is considered necessary for avoiding joint stiffness and attaining realignment. The authors present a unique approach to pediatric pilon fractures that uses open reduction and multivector external fixation with delayed AROM/PROM. Initial immobilization and significant traction allowed for joint realignment and prevented noncom
