Intra-articular anterior inferior iliac spine avulsion with hip instability: A retrospective study.

BACKGROUND: Intraarticular anteroinferior iliac spine (AIIS) avulsion fractures combined with hip dislocation are infrequently reported in medical literature. This study aims to present the outcomes of surgical treatment for this rare type of injury. METHODS: We retrospectively analyzed six cases of AIIS avulsion fracture with hip dislocation treated surgically. All patients underwent emergency hip reduction under sedation followed by open reduction and internal fixation using 4 mm cannulated screws through the iliofemoral approach. RESULTS: After an average follow-up period of 50 months, al

Alveolar Bone Fracture: Pathognomonic Sign for Clinical Diagnosis.

AIM: Dental injuries, especially luxation and avulsion, are common. Dental trauma can cause alveolar bone fracture that can lead to tooth loss and malocclusion. Single tooth alveolar bone fractures are difficult to identify unless it protrudes through the overlying mucosa and can be visualized. Pain, malocclusion, and tooth mobility provide signs of suspected alveolar bone fractures. Integrity of the proximate alveolar bone should be examined for fractures where avulsion, luxation, or other tooth trauma is detected. Any suggestion of alveolar fractures should be further investigated with an ap

Isolated trochlea and triquetrum fractures in an immunocompromised adolescent after a ground-level fall.

Isolated fractures of the humeral trochlea and triquetral avulsion fractures are uncommon, and to our knowledge, there are no reported cases of these two injuries occurring simultaneously (deleted). Thorough physical examination and advanced imaging facilitate diagnosis and guide fixation. Immunosuppressive therapy, such as chronic prednisone and tacrolimus, has the potential to predispose patients of all ages to uncharacteristic injury patterns. These cases may require a more focused initial work-up. Surgeons should maintain a high index of suspicion for atypical patterns in immunosuppressed

Isolated Adult Tillaux Fracture Associated With Volkmann Fracture-A Unique Combination of Injuries: Report of Two Cases and Review of the Literature.

Avulsion fractures of the anterior inferior tibiofibular ligament from its tibial attachment, Tillaux fractures, are usually seen in adolescents during the interval of the distal tibial epiphyseal closure. However, this pattern of fracture is rare in adult patients, because the ligaments will usually fail before the bone fails. Avulsion fracture of the posterior inferior tibiofibular ligament from its tibial attachment, Volkmann fracture, is the posterolateral counterpart of a similar injury. In the present study, the cases of 2 adult patients with simultaneous Tillaux and Volkmann fractures a

All-Arthroscopic Refixation of an Osteochondral Femoral Avulsion of the Anterior Cruciate Ligament with a Laxity-Minimizing Suture.

PURPOSE: Torn anterior cruciate ligaments in children are rare injuries. The incidence of concomitant avulsion fracture in the skeletally immature patient is high. Reports of arthroscopic reconstruction in the literature are sparse. CASE PRESENTATION: We present the case of a femoral avulsion fracture of the anterior cruciate ligament in a seven-year-old girl. The patient underwent arthroscopic refixation with DynacordTM Suture and Suture Button. At a follow-up of 8 weeks, the patient had free range of motion and a stable knee, with a negative Lachman- and Pivot-Shift-test in particular. CON

Sequential avulsion of the anterior inferior iliac spine in an adolescent long jumper.

We present here an unusual case of a sequential avulsion fracture of the anterior inferior iliac spine (AIIS), occurring first in the right and then the left AIIS, during long jump attempts. The case of a 17 year old boy with pain around his right groin, which first occurred during a long jump attempt, is presented. Radiographic examination revealed an avulsion fracture of the right AIIS. After conservative treatment, full athletic activity was allowed 2 months after the injury. However, 4 months later the patient returned with a similar pain in the opposite side of his groin. Radiographs reve

Isolated avulsion of the tibial attachment of the posterior cruciate ligament of the knee.

Of fourteen isolated avulsion fracutres of the posterior tibial attachment of the posterior cruciate ligament, one was undisplaced and five were minimally displaced. These six were treated conservatively, but four of the five displaced avulsion fractures progressed to non-union and significant functional disability. Eight displaced avulsion injuries were treated by open reduction. In four knees the fragment was sutured back to its origin on the posterior superior aspect of the tibia, and three of the these united. In two knees the fragment was large enough to fix with a screw and union ensued.

Avulsion of the femoral attachment of the medial collateral ligament in the setting of knee multiligament injury: A case report.

RATIONALE: Medial collateral ligament (MCL) injury is a common sports injury. The damage mainly occurs in ligament fibers, but MCL avulsion fracture is extremely rare and only a few reports have been published. PATIENT CONCERNS: Herein, we present a healthy 21-year-old man with an avulsion fracture of the MCL of the right knee sustained during snowboarding. DIAGNOSIS: Clinical and radiographic findings confirmed the presence of an avulsion fracture at the proximal attachment of the MCL, combined with complete anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) rupture. Outcome of arthroscopic ACL avulsion fixation using adjustable Endobutton: a case series report of new surgical technique.

INTRODUCTION: Anterior cruciate ligament (ACL) avulsion fractures, also referred to as tibial spine fractures, are increasingly encountered in both pediatric and adult populations. Although multiple fixation techniques have been described, the optimal surgical approach - particularly for displaced and comminuted fractures - remains controversial. Arthroscopic fixation using an adjustable-loop cortical Endobutton has emerged as a minimally invasive and potentially reliable option. METHODS: This case series includes five consecutive male patients (aged 15-31) with displaced ACL avulsion fractur

Fracture of the lesser trochanter as a sign of undiagnosed tumor disease in adults.

Isolated avulsion fractures of the pelvic ring are rare and occur predominantly in adolescent athletes. Isolated fractures of the lesser trochanter are reported to be pathognomic for tumor diseases in adults. We present a case of a female patient with an isolated avulsion of the lesser trochanter after treatment by her chiropractor. After staging examination, we determine the diagnosis of a left-sided carcinoma of the mamma. Additional imaging shows multiple metastases in liver, spine and pelvis. Palliative therapy has started over the course of time. We suggest, on suspicion of a malignant me

Isolated fractures of the lesser trochanter in children.

In a retrospective study of 1126 children with fractures of the proximal third of the femur, three children were found to have isolated fractures of the lesser trochanter. This fracture occurred from a fall in one child and following sporting activities, without a history of injury, in the others. In the latter children, the clinical presentations were similar to those of children with transient synovitis of the hip or Perthes disease. In each child, plain radiographs showed an avulsion fracture of the bony portion of the lesser trochanter. Early and complete recovery followed symptomatic trea

Understanding acute apophyseal spinous process avulsion injuries.

Apophyseal spinous process avulsion injury was first described in 1941. Since then, there have been sparse additional reports in the literature. The authors report their second case, involving an elite adolescent tennis player. The patient underwent surgical excision of the avulsed spinous process 12 weeks after initial presentation and experienced complete resolution of back pain. The authors provide the first reported histopathological analysis of the avulsion fracture site in the literature for both of their cases. The avulsion injury of the interspinous ligament was characterized by hyperc

Late Metastasis of Renal Cell Carcinoma in Distal Femur Treated with Knee Joint Preservation: A Case Report.

INTRODUCTION: An avulsion fracture of the lesser trochanter of the femur in adults is rare and should be differentiated from neoplastic lesions. We present a rare case of avulsion fracture of the lesser trochanter of the femur with prodromal symptoms in an adult. CASE REPORT: A right-handed 40-year-old man with gradual-onset left hip joint pain resulting from baseball pitching consulted a neighborhood doctor. X-ray images did not show any obvious anomalies; however, the pain persisted. Two months after the onset of pain, he stumbled when getting into a car, and the pain worsened. He was then

Metrizamide examination of traumatic lumbar nerve root meningocele.

Demonstration of traumatic lumbosacral root avulsion is a rare clinical entity because the bony pelvis offers support and protection to the nerve roots against stretching and rupture. Major vehicular trauma producing unstable pelvic fractures allows lumbosacral root avulsion to occur, and the 18th well-documented case is reported. Delay in diagnosis of several months is common. Demonstration of traumatic meningoceles is important, and metrizamide offers better visualization of the root than does iodophendylate. Absence of the root in the meningocele is equated with avulsion.

Traumatic tympanic bulla fracture.

A Pekingese dog was presented for evaluation of head trauma with ventral head and neck swelling, puncture wounds, palpable mandibular fractures, and loss of menace, severe miosis, and loss of palpebral reflex of the right eye. Computed tomography confirmed multiple mandibular and zygomatic fractures, a right ear canal avulsion, and a complete right tympanic bulla fracture with ventral displacement. The tympanic bulla fracture was managed conservatively. Topical lubrication and antibiotic ointment was prescribed for the right eye. A subtotal hemimandibulectomy was performed to address the mandi

Open dorsomedial fracture-dislocation of the medial cuneiform with concomitant tibialis anterior tendon rupture: a case report and review of the literature.

Medial cuneiform fracture-dislocations are rare and typically occur in the setting of high-energy midfoot trauma. We report the case of a 30-year-old woman who sustained an open dorsomedial fracture-dislocation of the medial cuneiform with concomitant tibialis anterior tendon avulsion after a 6-m fall. She underwent emergent irrigation and debridement, open reduction, and internal fixation of the midfoot, and primary repair of the tibialis anterior tendon. Intraoperatively, a longitudinal split tear with avulsion from the medial cuneiform insertion was identified and repaired through a bone tu

Longitudinal tear of the inferior rectus muscle in orbital floor fracture.

We report a case of longitudinal avulsion of the inferior rectus muscle following orbital floor fracture and describe its clinical presentation, computed tomography (CT) features and management. A 53-year-old man felt vertical diplopia in all gaze immediately after the trauma. Orthoptic assessment showed left over right hypertropia of 20 prism diopters and left exotropia of 10 prism diopters in primary position. The left orbital floor fracture and the prolapse of orbital contents into the maxillary sinus were presented by CT. Exploration of the orbit was performed under general anesthesia. The

Strength Recovery from Nonsurgical Treatment of Apophyseal Avulsions of the Pelvis.

BACKGROUND: Strength deficits, strength recovery, and return to sports timing following nonoperative apophyseal avulsion fractures are relatively unknown, as is the effect of displacement of the avulsed fragment. This study aims to assess strength recovery following a pelvic apophyseal avulsion and determine whether a relationship between radiographic characteristics and strength exists. METHODS: Twenty-eight patients from a prospective pelvic apophyseal avulsion study database, were treated nonoperatively with physical therapy, and a minimum 3-month follow-up was included. Patients were cate

Shoulder Pain in a Pediatric Throwing Athlete.

Pediatric avulsion fractures most commonly occur at sites of secondary ossification and are often associated with chronic stress from repetitive movements. Because of a variety of risk factors, youth athletes place higher stress on ossification centers, and their activities may predispose them to injury. This case report describes a 12-year-old female softball player who presented with pain at the distal acromion, worsened by her overhead throwing motion. Further questioning revealed improper throwing mechanics placing extenuated stress on the shoulder. Plain radiograph imaging showed an avuls

The pathoanatomy of lateral ligamentous disruption in complex elbow instability.

We sought to determine the lateral soft-tissue injury pattern in a consecutive series of patients with elbow dislocation (10 cases) or fracture-dislocation (52 cases) that required open operative repair. Patients who were seen more than 3 months after injury or those in whom previous operative intervention had obscured the anatomy were excluded. There were 42 men and 19 women (mean age, 43 years; range, 13-82 years). One patient had bilateral injuries. The mean time to surgery was 15 days after injury, with a range from 1 to 76 days. There were associated fractures in 52 elbows: coronoid (39),
