Extradural abscess can be a complication of frontal sinusitis. The chronicity of this condition can give rise to extreme frontal hyperostosis which can be an important radiological sign in a condition which may be clinically obscure.

Two cases are reported of intracranial complications of sinusitis, unusual nowadays. They were caused by osteomyelitis of the frontal bone following chronic frontal sinusitis. These cases were distinguished by complete destruction of the posterior wall of the sinus involved: in one of the cases there was an acute central neurological disturbance caused by a cerebral abscess; in the other patient, who came for consultation because of a Pott's Puffy tumour, a sizeable epidural abscess was found.

A 17 year-old boy suffered a minor head injury after a somersault in which his knee hit the forehead. He had a swelling over the right frontal sinus with unbroken skin. A computed tomography revealed a fracture of the anterior wall of the frontal sinus with a displaced bone fragment. The posterior wall was undamaged, and no intracranial injury was evident. The nasofrontal duct was intact. The fragment was repositioned surgically without complications. The frontal sinus is considered resistant to low-energy trauma. This dogma, however, has exceptions.
