A 21-year-old man, with personal history of asthma and no usual medication, was referred to gastroenterology appointment due to dysphagia for solids and previous episodes of food impaction in the last 5 months. He also reported nocturnal heartburn without any other warning signs. Upper gastroscopy revealed a peptic stenosis in the distal esophagus. Histopathologic examination showed hyperplasia and numerous intraepithelial eosinophils, without dysplasia or malignancy. Therapy with a double-dose proton pump inhibitor (PPI) was started.

A 83-year-old woman with previous history of gallstone was hospitalized with intermittent melena of 1-week duration. Gastroscopy showed a protuberant mass with thick pedicle and superficial ulcer measuring 3.0 × 4.0-cm in the lower gastric body (Fig. 1. A). Biopsy of the ulcer indicated chronic inflammation without evidence of malignancy. Abdominal contrast-enhanced computed tomography demonstrated a well-defined and mixed density mass with heterogeneous enhancement in the gastric body. The final diagnosis was gastric angiolipoma.

We report a case of splenic rupture in an 83-year-old woman presenting with symptoms concerning for upper gastrointestinal malignancy. CT imaging revealed marked gastric thickening and a 33×33 mm splenic metastasis, with subsequent gastroscopy demonstrating a large lesion at the gastro-oesophageal junction. While awaiting further investigation, the patient developed acute onset left upper quadrant pain, peritonism and hypovolaemic shock. Urgent CT imaging demonstrated a ruptured spleen with extensive haemoperitoneum, necessitating emergency splenectomy.

A 28 month-old boy was hospitalized with pallor and weight stagnation. He had macrocytic anaemia and pancytopenia due to cobalamin deficiency and a rare homozygous mutation in the intrinsic factor gene. His sister showed similar symptoms at the age of 15 months. The heterozygous father had no symptoms, but did have a low cobalamin level. Gastroscopy with biopsies showed no pathology. All were given monthly cyanocobalamin injections which, however, caused leg cramps. Replacement with monthly hydroxocobalamin was successful.

Ten patients with chronic duodenal ulcer underwent posterior truncal vagotomy combined with anterior seromyotomy of lesser curvature of the stomach (Taylor's procedure). Perioperative acid secretory studies (Horlland test) and gastric emptying tests were performed. Postoperative recovery was uneventful in all patients. The acid secretory studies showed decreased BAO, MAO and PAO (P < 0.01). The postoperative gastric emptying was significantly delayed, and the patients are all symptom free. Follow up gastroscopy revealed ulcer healing in 7 patients.

A previously healthy 18-year-old man with a 9-month history of upper abdominal pain and significant weight loss underwent gastroscopy, revealing an ulcerative lesion in the duodenal bulb. Enrolled in a clinical trial, both 18 F-FDG and 68 Ga-FAPI-04 PET/CT demonstrated similar metabolic activity, though 68 Ga-FAPI-04 offered superior lesion delineation. Pathology confirmed synovial sarcoma. This case highlights the potential utility of 68 Ga-FAPI-04 as a radiopharmaceutical for evaluating rare gastrointestinal synovial sarcomas.

A 11-year-old child diagnosed as chronic tonsillitis and adenoid hypertrophy underwent adeno-tonsillectomy under general anesthesia. After surgery, patient complained with abdominal discomfort, paleness and vomiting, which presented as the old black contents. Complete blood count showed: 45.2 g/L, hemoglobin of red blood cells 2.57 x 10(12)/L, An emergency gastroscopy confirmed gastric ulcer with hemorrage. Hemorrage was controlled and complete blood count results restored to normal after supportive therapy. Gastric ulcer completely recovered two weeks after discharge.

A male patient developed hematemesis and chest pain after severe vomiting. Gastroscopy showed a linear hematoma from the esophageal entrance to the cardia. Enhanced CT of the esophagus revealed a high-density shadow in the middle of the esophagus. Severe vomiting can lead to esophageal injury and esophageal hematoma. As this condition mimics critical symptoms of myocardial infarction and aortic dissection, clinicians must remain vigilant. This case emphasizes the importance of recognizing acute submucosal bleeding in the digestive tract, aiding in clinical diagnosis and treatment.

BACKGROUND: No known case of neuroendocrine tumour (NET) with schwannoma has been reported.
CASE SUMMARY: A 63-year-old female presented to our hospital with nausea and vomiting. Upper gastrointestinal endoscopy revealed a mass in the descending part of the duodenum. Using ultrasound gastroscopy, we found that the tumour originated from the submucosa and showed low echo. We removed the tumour by electrocoagulation and sent it for pathological biopsy.
CONCLUSION: Immunohistochemical results showed that the mass was a rare NET with neurilemmoma.

Epithelioid sarcoma (ES) of the small bowel is a rare gastrointestinal tumour. We report a case of gastrointestinal bleeding secondary to small bowel ES in a 55-year-old gentleman. After gastroscopy and colonoscopy failed to identify the source of bleeding, we proceeded with computed tomography angiogram of the mesentery, which revealed intraluminal blood clot in the distal jejunum with features of obstruction. This is a rare cause of obscure gastrointestinal bleeding and emphasises the need for additional evaluation in the presence of negative endoscopic findings.

A seven-year-old boy was admitted with haematemesis. The boy was originally born at 27 weeks' gestational age. His neonatal period had been complicated by sepsis, for which he was treated with antibiotics through an umbilical venous catheter (UVC). A gastroscopy showed grade III oesophageal varices with bleeding. He was examined thoroughly for other causes of portal hypertension, but none were found. Portal hypertension caused by UVC in the neonatal period is a rare but very serious complication to neonatal UVC.

A 7 month old West Highland White Terrier was presented with anorexia and vomitus since 4 days. On physical examination the dog was depressed, febrile and showed abdominal pain. During abdominal ultrasonographic examination a pylorogastric intussusception was suspected and this was confirmed subsequently by gastroscopy. The invaginated pylorus could be repositioned without complications using the endoscope. The dog recovered completely and without recurrence. A pylorogastric intussusception is a rarely diagnosed cause of an acute abdomen that can be resolved with a minimal invasive procedure.
