Understanding Health Care Utilization and Mortality After Emergency General Surgery in Patients With Underlying Liver Disease.

BACKGROUND: Mortality and complications are not well defined nationally for emergency general surgery (EGS) patients presenting with underlying all-cause liver disease (LD). STUDY DESIGN: We analyzed the 2012-2014 National Inpatient Sample for adults (aged ≥ 18 years) with a primary EGS diagnosis. Underlying LD included International Classification of Diseases, Ninth Revision, Clinical Modification codes for alcoholic and viral hepatitis, malignancy, congenital etiologies, and cirrhosis. The primary outcome was mortality; secondary outcomes included complications, operative intervention, and …

Antithrombotic prescriptions for many general surgery patients significantly increases the likelihood of post-operative bleeding complications.

BACKGROUND: The investigators hypothesized that despite cessation recommendations for peri-operative antithrombotic management in the elective setting, the use of these drugs is likely associated with increased odds for bleeding complications. METHODS: Humana® insurance claims (22 million covered lives) from 2007 through 2017 were analyzed. Only patients undergoing elective general surgery procedures were included. Patient Safety Indicator 09 (PSI-09) coding for post-operative hemorrhage and hematoma were analyzed. RESULTS: Despite the existence of peri-operative management guidelines, patie…

Development of a Risk Stratification Scoring System to Predict General Surgical Complications in Hand Surgery Patients.

PURPOSE: Avoidance of postoperative complications is important to both patients and surgeons. In an attempt to optimize postoperative outcomes, a risk stratification scoring system has been created to aid in optimizing risk factors for general surgical complications in hand surgery patients. METHODS: Patients were identified who underwent hand procedures as part of the American College of Surgeons National Surgical Quality Improvement Program. Independent risk factors associated with postoperative complications within 30 days of surgery were identified and used to develop a point-scoring syst

The feasibility of sugammadex for general anesthesia and facial nerve monitoring in patients undergoing parotid surgery.

The use of neuromuscular blocking agent (NMBA) during anesthesia may interfere with facial nerve monitoring (FNM) during parotid surgery. Sugammadex has been reported to be an effective and safe reversal of rocuronium-induced neuromuscular block (NMB) during surgery. This study investigated the feasibility and clinical effectiveness of sugammadex for NMB reversal during FNM in Parotid surgery. Fifty patients undergoing parotid surgery were randomized allocated into conventional anesthesia group (Group C, n = 25) and sugammadex group (Group S, n = 25). Group C did not receive any NMBA. Group S

Acoustic neuroma surgery for preservation of hearing: technique and experience in the Chinese PLA General Hospital.

CONCLUSIONS: Good hearing preservation can be achieved during acoustic neuroma surgery assisted by real-time dynamic auditory monitoring and ear endoscopic techniques. Preservation of the arachnoid and its blood supply are important for hearing preservation, and injury to the internal auditory artery is the most important cause of hearing loss. OBJECTIVE: To explore techniques to preserve hearing during acoustic neuroma resection. METHODS: This was a retrospective case review in a hospital setting. From July 2003 to July 2007, intraoperative auditory monitoring using auditory brainstem respo

Effectiveness of combined thoracic epidural and light general anaesthesia in patients undergoing non-cardiac thoracic surgery.

BACKGROUND: The specialty of thoracic anaesthesia has emerged as a scientifically based discipline just 30 years back. At the start of the 20th century empyema and tuberculosis were the main indications for the thoracic surgery. Later on with the introduction of antibiotics lung malignancies were more commonly operated. Recent resurgence of tuberculosis and associated medical illnesses put these patients in high risk for surgery and anaesthesia, necessitating introduction of more skillful approach. The objective of this study was to evaluate the effectiveness of combined thoracic epidural anae

Propofol for maintenance of general anesthesia: a technique to limit blood loss during endoscopic sinus surgery.

PURPOSE: Most cases of endoscopic sinus surgery are amenable to techniques using local anesthesia with monitored sedation. However, it is frequently the preference of the patient to have surgery under general anesthesia. One major drawback of general anesthesia is the increased bleeding encountered which can interfere with optimal visualization of the intranasal anatomy. In this study, an analysis was made to see if technique of general anesthesia has an impact on estimated blood loss in patients undergoing endoscopic sinus surgery. METHODS: Twenty-five patients undergoing outpatient endoscop

[Controlled hypotension by sodium-nitroprusside in general anaesthesia for difficult intraocular surgery (preliminary report) (author's transl)].

General anaesthesia was performed in 75 patients with difficult intraocular procedures in which the standard relaxation and hyperventilation was supplemented by controlled short-term hypotension by sodium-nitroprusside. This resulted in a significant decrease of the "vis a tergo"--e.g. the threatening prolaps of intraocular tissue. Preliminary experience revealed that intraoperative complications in these difficult situations are reduced. As this facilitates also the task of the anaesthesist this broadens our indications for complex intraocular surgery particularly in younger patients.

Patterns of total and ionized calcium and other electrolytes in plasma during and after general surgery.

Thirteen patients were studied during two hours of anaesthesia for abdmonial operations and for an hour post-operatively. Serial venous blood samples were taken for determination of plasma total and ionized calcium, acid-base variables, Na, K, Mg, PO4 total proteins and parathyroid hormone. One group of patients received 5 per cent dextrose/water and the other 5 per cent dextrose in Ringer's solution, at 250 ml per hour. Total and ionized calcium levels decreased toward the end of operation and an hour later, associated with respiratory alkalosis. Parathyroid hormone increased at the end of op

Anesthetic Outcomes: Postoperative Nausea and Vomiting in Laparoscopic Gynecological Surgery Patients Who Had General Anesthesia With an Enhanced Recovery After Surgery (ERAS) Technique.

PURPOSE: Postoperative nausea and vomiting (PONV) is a common occurrence that leads to patient discomfort and prolonged length of stay. The average rate of PONV is approximately 30% in the general population and can be as high as 70% to 80% in those at high risk. Enhanced recovery after surgery (ERAS) protocols have been created from analysis of current evidence-based research aimed to decrease postoperative complications and improve patient outcomes following surgical interventions. DESIGN: This retrospective pre-experimental cohort study used convenience sampling of 1,200 past surgical pati
