The search of 'novel' mtDNA mutations in hypertrophic cardiomyopathy: MITOMAPping as a risk factor.

MITOMAP is by far the most frequently cited Web resource that is referred to in substantiating novelty of an mtDNA mutation. This database, as is now known, has quite an incomplete coverage of the mtDNA mutations from the literature. This circumstance has seduced many scholars of medical genetics in the past to claim novelty of rather 'worn-out' mtDNA mutations. What is, however, really novel in the field is that researchers take advantage of this situation and deliberately suppress information from other sources, as it appears to have occurred in two recently published cases of hypertrophic c

Regulation of genetic and other health information in a comparative perspective.

In the present article, the authors provide a general overview of the academic and legal debate on the regulation of access to and use of genetic information by non-medical actors. Their aim is to give some insight in the academic views on the need to introduce specific genetics legislation and on the balance that might be struck between the various interests concerned. Furthermore, by analyzing relevant legislation and policy measures in the US and in Europe, they identify the issues that are deemed relevant in considering and, eventually, introducing regulative measures with respect to genet

Disease-causing mutations or functional polymorphisms?

Molecular genetics has infiltrated all medical disciplines and particularly the field of neuromuscular diseases. Scientists use genetic techniques who may not always aware of their restrictions and potential pitfalls. Particularly the genetic distinction between disease-causing mutations and "benign" polymorphisms can be difficult because the current standard criteria are not sufficient. Here a recently developed mathematical algorithm is given which helps to calculate the number of required healthy controls depending on the prevalence of a genetic alteration in patients. Furthermore, the gene

Fragile X syndrome.

Fragile X syndrome is the most common form of inherited mental retardation. It is seen in people of all nationalities and in all areas of the world. Fragile X syndrome can be a devastating condition, as many boys are severely retarded and require multiple services. Treatment involves behaviour management techniques, appropriate school placement, community support for the family, and careful medical follow-up often including psychopharmacology. The genetics of fragile X syndrome is now understood, prenatal testing is available, and the disorder is preventable through appropriate genetic counsel

Nevoid basal cell carcinoma syndrome: review of the literature and report of a case.

Any patient who has multiple cystic lesions of the jaws should be evaluated for the presence of other signs of nevoid basal cell carcinoma syndrome. The workup should include consultations with specialists in neurology, radiology, dermatology, medical genetics, and any other specialty areas where signs of symptoms may appear. It is not unusual for the oral surgeon to be the first practitioner to see such a patient, because initial complaints often refer to pain and swelling for an infected jaw cyst. The workup of this case and its treatment are typical for a patient with nevoid basal cell carc

[The Anders Annual Prize for young researchers 1992. Identification of genes in human diseases].

The localisation and identification of the genes for human diseases are most important aspects of human gene mapping. The identification of a disease gene immediately defines the function of the respective chromosomal region. In other words, the identification and characterisation of morbid mutations provide a short-cut to the definition of specific functions of certain parts of the human genome. Professor Leena Peltonen Palotie has been awarded the Anders Jahre prize for young scientists for her basic research into hereditary diseases, particularly the Marfan syndrome and aspartyl-glucosaminu

Checklist for gene/disease-specific variation database curators to enable ethical data management.

Databases with variant and phenotype information are essential for advancing research and improving the health and welfare of individuals. These resources require data to be collected, curated, and shared among relevant specialties to maximize impact. The increasing generation of data which must be shared both nationally and globally for maximal effect presents important ethical and privacy concerns. Database curators need to ensure that their work conform to acceptable ethical standards. A Working Group of the Human Variome Project had the task of updating and streamlining ethical guidelines

Tragedy and Grenzsituationen in genetic prediction.

Philosophical anthropologies that emphasise the role of the emotions can be used to expand existing notions of moral agency and learning in situations of great moral complexity. In this article we tell the story of one patient facing the tough decision of whether to be tested for Huntington's disease or not. We then interpret her story from two different but compatible philosophical entry points: Aristotle's conception of Greek tragedy and Karl Jaspers' notion of Grenzsituationen (boundary situations). We continue by indicating some ways in which these two positions may be used for reflecting

Attitudes Towards Non-directiveness Among Medical Geneticists in Germany and Switzerland.

The principle of non-directiveness remains an important tenet in genetics. However, the concept has encountered growing criticism over the last two decades. There is an ongoing discussion about its appropriateness for specific situations in genetics, especially in light of recent significant advancements in genetic medicine. Despite the debate surrounding non-directiveness, there is a notable lack of up-to-date international research empirically investigating the issue from the perspective of those who actually do genetic counselling. Addressing this gap, our article delves into the viewpoints

Genetic Landscape of a Cohort of 120 Patients with Diminished Ovarian Reserve: Correlation with Infertility.

Diminished ovarian reserve (DOR) and primary ovarian insufficiency (POI) are major causes of female infertility. We recently found a monogenic etiology in 29.3% of POI, leading to personalized medicine. The genetic landscape of DOR is unknown. A prospective study (2018-2023) of an international cohort of 120 patients with unexplained DOR was performed using a large custom targeted next-generation sequencing panel including all known POI-causing genes. The diagnostic yield, based on the American College of Medical Genetics, was 24, 2%. Genes belong to different pathways: metabolism and mitochon

Evaluation of Genetic Testing in a Cohort of Diverse Pediatric Patients in the United States with Congenital Cataracts.

The aim of this study was to evaluate the diagnostic yield from prior genetic testing in a 20-year cohort of pediatric patients with congenital cataracts. A retrospective review of patients with congenital cataracts who underwent genetic testing was completed from 2003-2022. The diagnostic yield of the test was determined by variant classification and inheritance pattern. Variants from initial testing underwent reclassification in accordance with ACMG-AMP (American College of Medical Genetics and Genomics-American Association of Molecular Pathology) 2015 or 2020 ACMG CNV guidelines. Prenatal case report of Barth syndrome caused by novel TAFAZZIN mutation: Clinical characteristics of fetal dilated cardiomyopathy with ascites.

Barth syndrome (BTHS) is a rare X-linked recessive genetic disease, which appears in infancy with myocardial and skeletal muscle diseases, neutropenia, growth retardation, and other clinical features. TAFAZZIN is the pathogenic gene of BTHS, which encodes the tafazzin protein of the inner membrane of the mitochondria, a phosphatidyltransferase involved in cardiolipin remodeling and functional maturation. At present, BTHS has been widely reported, but prenatal cases are rare. We report a 24+4-week fetus with clinical manifestations including left ventricular insufficiency and ascites. After ind

Splicing impact of deep exonic missense variants in CAPN3 explored systematically by minigene functional assay.

Improving the accuracy of variant interpretation during diagnostic sequencing is a major goal for genomic medicine. To explore an often-overlooked splicing effect of missense variants, we developed the functional assay ("minigene") for the majority of exons of CAPN3, the gene responsible for limb girdle muscular dystrophy. By systematically screening 21 missense variants distributed along the gene, we found that eight clinically relevant missense variants located at a certain distance from the exon-intron borders (deep exonic missense variants) disrupted normal splicing of CAPN3 exons. Several

Risk management adherence following genetic testing for hereditary cancer syndromes: a Singaporean experience.

Assessing adherence behavior among mutation carriers to cancer risk management guidelines is important for both service improvement and cost-effectiveness analyses, but such real-world data is often lacking. The present study aims to report adherence rates among mutation carriers in a recently established cancer genetics program in Singapore. We conducted a medical chart review of mutation carriers who had attended for genetic counseling and gathered data regarding risk management behavior, including cancer surveillance and/or risk-reducing surgery, and cancers subsequently detected. Of the 52

Pathogenesis, diagnosis and therapeutic strategies in WHIM syndrome immunodeficiency.

21 introductionWHIM syndrome is a rare combined primary immunodeficiency disorder caused by autosomal dominant gain-of-function mutations in the chemokine receptor CXCR4. It is the only Mendelian condition known to be caused by mutation of a chemokine or chemokine receptor. As such, it provides a scientific opportunity to understand chemokine-dependent immunoregulation in humans and a medical opportunity to develop mechanism-based treatment and cure strategies.22 areas coveredThis review covers the clinical features, genetics, immunopathogenesis and clinical management of WHIM syndrome. Primary Open-Angle Glaucoma among Black and Latinx Populations: Exploring Racial Disparities in the United States.

Primary open-angle glaucoma (POAG) is the leading cause of irreversible blindness in the United States. It disproportionately affects Black and Latinx populations. Among Black people, the disease is associated with an earlier onset, faster progression, and significantly higher rates of bilateral blindness. Latinx individuals also face higher prevalence and frequent underdiagnosis compared to non-Hispanic White individuals. Racial and ethnic differences in treatment response, ranging from medication efficacy to surgical outcomes, further contribute to these disparities. These disparities can be

Cystic fibrosis carrier screening in Australia: comparing sequencing and targeted panels across diverse ancestries.

BACKGROUND: Targeted cystic fibrosis (CF) carrier screening panels may lack sensitivity in non-European ancestry groups. This study aims to evaluate the sensitivity of various panels in Australian CF carriers identified through sequencing. METHODS: The following panels were evaluated in 869 CF carriers: Asuragen, Elucigene, Devyser, American College of Medical Genetics and Genomics and Victorian Clinical Genetics Services. Ancestry-specific CF carrier frequencies from population databases and Bayesian analysis were used to estimate post-test residual carrier risks. RESULTS: When variants wit

Modifying mosquitoes to suppress disease transmission: Is the long wait over?

For more than 50 years it has been a dream of medical entomologists and public health workers to control diseases like malaria and dengue fever by modifying, through genetics and other methods, the arthropods that transmit them to humans. A brief synopsis of the history of these efforts as applied to mosquitoes is presented; none proved to be effective in reducing disease prevalence. Only in the last few years have novel approaches been developed or proposed that indicate the long wait may be over. Three recent developments are particularly promising: CRISPR-Cas9 driven genetic modification, s
