Rumination, Hopelessness, Behavioural Avoidance and Psychopathology Symptoms After Bereavement: Serial Mediation Analyses.

Bereavement can precipitate severe mental health problems, including major depressive disorder and prolonged grief disorder. Rumination is a risk factor of post-loss mental health problems, and as such, a better understanding of its working mechanisms may inform clinical practice. Rumination is theorized to take up time and increase feelings of hopelessness, leading to inactivity and social withdrawal, which in turn fuels post-loss psychopathology. Yet, these ideas have not been tested comprehensively. Therefore, we aimed to fill this gap in knowledge. A sample of bereaved adults (87% women) c…

Ruminative coping as avoidance: a reinterpretation of its function in adjustment to bereavement.

The paper argues for a reconceptualization of ruminative coping with the death of a loved one as an avoidant rather than a confrontational strategy. Ruminative coping has been characterized within the bereavement field as persistent, repetitive and passive focus on negative emotions and symptoms. It has been theoretically described and empirically shown to be a maladaptive process, being conceptually related to complicated/chronic/prolonged grief. Furthermore, it has been contrasted with denial and suppression processes--which, too, have been understood to be maladaptive and associated with ma…

Factors affecting the emotional reactions of patient relatives who receive news of death: a prospective observational study.

BACKGROUND: Reporting the death of relatives to a family member is a very stressful task for physicians. Grief reactions differ from person to person. METHODS: Demographic data of 100 patients who died after staying in ICU for more than three days were recorded. For each patient, one of the family members filled a form which contained their own age, gender, education level, marital status, number of children, degree of relationship, psychiatric treatment status, living in the same house as the patient, and whether they had ever visited the ICU before. Grief reactions were evaluated in five di…

Trajectories of insomnia following bereavement.

BACKGROUND: Insomnia symptoms are common following bereavement and may exacerbate severe and protracted grief reactions, such as prolonged grief disorder (PGD). However, typical trajectories of insomnia symptoms and risk factors for having a more chronic insomnia trajectory following bereavement are yet unknown. METHOD: In the current investigation, 220 recently bereaved (≤6 months post-loss) participants, completed questionnaires assessing sociodemographic and loss-related characteristics, rumination, experiential avoidance and symptoms of (prolonged) grief and depression, on three time-poin…

The impact of supportive intervention after second trimester termination of pregnancy for fetal abnormality.

The reactions of women who had had a termination of pregnancy for fetal abnormality in the second trimester have been studied retrospectively using a semi-structured questionnaire. The severity of the grief reaction was measured and the outcome at 6 months was compared with the findings from a previous study in South Wales which had led to the introduction of skilled support from genetic fieldworkers and formal genetic counselling after the termination. Of the 69 women interviewed, 55 (80 per cent) experienced an acute grief reaction and 17 (25 per cent) had not resolved their grief 6 months a…

Supporting kids in loss: A randomised controlled trial of a co-produced unguided internet cognitive behaviour therapy intervention for grieving children.

Bereavement is a common experience in childhood. Unguided internet cognitive behaviour therapy (CBT) was tested in a sample of 96 grieving children aged 6-12 years (M = 8.51, SD = 3.62). The intervention was intended for use by children who had experienced either bereavement or non-bereavement loss. Children were not required to have an elevated level of prolonged grief to enter the trial. The primary outcome of anxiety and secondary outcomes of well-being, and symptoms of depression, posttraumatic stress disorder (PTSD), and prolonged grief were examined at pre- and post-intervention and one …

METHODS: An observational multi-centre study was conducted with 61 caregivers of VS patients hospitalized in specialized units in Italy. The Anxiety and Depression Short Scale, the Prolonged Grief 12 and Family Strain Questionnaire were used to measure caregivers' burden and the Coping Orientations to Problem Experiences to identify the coping strategies used by caregivers. A hierarchical cluster analysis was carried out to group the data…

Effects and User-Reported Experiences of a Self-Management Mobile Health App for Grieving Adolescents: Randomized Controlled Trial.

BACKGROUND: Adolescents who experience the loss of a family member are at increased risk of adverse mental health outcomes, yet many face barriers or may be reluctant to access in-person or group-based support. mHealth (mobile health) interventions can help address these barriers by offering flexible, accessible, and low-threshold support. OBJECTIVE: This study evaluated the short- and long-term mental health effects of Alba - Youth in Grief, a preventive self-management mobile app for bereaved adolescents. The primary outcome was symptoms of prolonged grief, while secondary outcomes included…

The Association between Therapeutic Alliance and Parental Health Outcomes following a Child's Death in the Pediatric Intensive Care Unit.

Therapeutic alliance reflects the strength and quality of the physician-patient/family relationship. We investigated the association between therapeutic alliance and bereaved parents' mental health and perceived overall health following their child's death in a pediatric intensive care unit (PICU). Bereaved parents were surveyed 6 months after their child's death in a PICU affiliated with the Collaborative Pediatric Critical Care Research Network. Parents were evaluated for complicated grief, depression, and post-traumatic stress using the Inventory of Complicated Grief (ICG), the Patient Heal…

Psychologic implications of AIDS.

Eleven patients with AIDS were seen by a psychiatric consultation service over a four-year period. Psychologic issues identified in these patients included changes in body image, feelings of helplessness and isolation, sexual concerns and the grief process. Management should be aimed toward maintaining a nonjudgmental stance, providing liaison with other caregivers and securing appropriate information and grief counseling for patients, with provision for the patient's loved ones.

Motor Neurone Disease family carers' experiences of caring, palliative care and bereavement: an exploratory qualitative study.

BACKGROUND: Motor Neurone Disease (MND) is a neurodegenerative disease with a sudden onset, a rapid progression, a profile of complex disabilities and fatal consequences. Caring for a person with MND is an unremitting commitment, yet little research has examined the experiences and needs of carers for palliative care and bereavement care. AIM: This study explored the experiences of MND family carers, both during their time as carers and following bereavement. Particular attention was paid to the carers' prolonged grief status and to the implications for service delivery, including palliative …

Prospective patterns of resilience and maladjustment during widowhood.

Using prospective longitudinal data on an older sample beginning prior to the death of a spouse, G. A. Bonanno et al. (2002) distinguished 5 unique trajectories of bereavement outcome: common grief, chronic grief, chronic depression, depression followed by improvement, and resilience. These trajectories having been identified, the aims of the current study were to examine differences in how respondents in each group reacted to and processed the loss. Specific hypotheses were tested regarding differences in coping, meaning making, context, and representations of the lost relationship. Results s…

[Perinatal bereavement. Psychopathology and counseling].

Perinatal death entails bereavement for the family and confusion for the care-givers. In the light of a brief theoretical study of grief, the present article reviews the difficulties and complications of parental grief and the propositions for support and counselling. The most recent works in the subject as well as topics for future research are emphasized. Perinatal bereavement is followed by severe complications: depression, psychosis for mothers, mainly following the next live birth; family disagreement; disturbances, running away, drug, suicide among already born or future children, especi…
