[Unusual case of successful hand replantation with bad functional outcome: a case report].

INTRODUCTION: Traumatic hand amputation is a very severe and rare injury. Replantation and revascularization surgery requires the existence of specialized centres and well-educated teams of microsurgeons. In places where there are no such specialized centres for this kind of surgery, traumatic hand amputation is a major concern and quite a challenge, and a successfully performed replantation is a great accomplishment. CASE OUTLINE: We report a case of a successfully survived replanted hand in a 35-year-old male patient with unfortunately bad end result of the treatment. The patient with the a

A manual therapy intervention improves symptoms in patients with carpal tunnel syndrome: a pilot study.

In carpal tunnel syndrome (CTS), manual therapy interventions (MTI) reduce tissue adhesion and increase wrist mobility. We evaluated the efficacy of a MTI in relieving CTS signs and symptoms. Twenty-two CTS patients (pts) (41 hands) were treated with a MTI, consisting in 6 treatments (2/week for 3 weeks) of soft tissues of wrist and hands and of carpal bones. Pts were assessed for hand sensitivity, paresthesia, hand strength, hand and forearm pain, night awakening; Phalen test, thenar eminence hypotrophy and Boston Carpal Tunnel Questionnaire (BCTQ) Symptom Severity Scale (SSS) and Functional

Radial shift of the ulnar fingers: a new technique for special cases of longitudinal central deficiency.

Treatment of central longitudinal deficiencies is well defined, with different techniques established for the classical clefts. However, none of these techniques is easily applied to the treatment of very deep clefts accompanied by a significant divergence of the metacarpal bones. In such cases, the results of current techniques are disappointing. We propose a new technique of "Translocation in the Radial direction of the Ulnar Finger(s)" (TRUF) by intra-carpal osteotomy. The results are illustrated in three clinical cases. The TRUF operation allowed closing of the cleft, alignment of the meta

Type I congenital dyserythropoietic anaemia with myelopoietic abnormalities and hand malformations.

Type I dyserythropoietic anaemia was diagnosed in an infant, who presented with respiratory distress and hepatosplenomegaly soon after birth. Anaemia became manifest during the neonatal period. The case clearly proves the congenital nature of the disease. Abnormalities of the myelopoietic series indicate that it might be a stem cell disease and the presence of skeletal anomalies of the hands suggests a genetic relationship to some cases of Fanconi and Diamond anaemia. No serum lipid or vitamin E deficiency was present as in type II congenital dyserythropoietic anaemia. Serial serum ferritin de

Treatment of pathologic fractures.

Pathologic fractures occur in bone weakened by a disease process. In the hand, the most common cause of pathologic fracture is a solitary enchondroma, although many other less common causes exist, including bone cysts, benign and malignant tumors, and other rarer entities. Arriving at a correct diagnosis is the key to successful treatment. If the fracture seems the result of a benign process, the fracture can be allowed to heal prior to definitive treatment of the underlying lesion. Fractures associated with aggressive or malignant lesions require more urgent treatment, although the overall pr

Unusual coalition of the carpal bones with simian crease: a case report.

We present a case of scapholunate coalition with absent fifth metacarpal on one hand, and scapholunate triquetral coalition on the other hand. The patient was asymptomatic, though he was aware of the deformity of the little and ring fingers on the right hand. No treatment was offered as the deformity was not incapacitating and the syndactyly did not interfere with hand function.

Volar Plating of Scaphoid Fractures: A Retrospective Case Series.

BACKGROUND: The purpose of this study was to evaluate the rate of union of scaphoid fractures managed with volar plating and assess postoperative complications. METHODS: Retrospective consecutive case series of 28 patients with scaphoid fractures, 9 acute and 19 chronic nonunions, undergoing surgical fixation with volar scaphoid plating by a single surgeon between 2013 and 2019. Patients were followed up for a minimum of 3 months with scaphoid bony union being confirmed on radiograph or computed tomography. Postoperative complications and need for plate removal were recorded. RESULTS: Overal

[Congenital partial gigantism of the upper extremities in children].

Angio-and phlebography, as well as pathomorphological studies were made on epiphyseal cartilaginous plate, diaphyseal bone tissue and skin of patients with partial gigantism of upper extremities. Significant developmental anomalies were found in the formation of palmar arches and digital arteries, as well as hypoplasia and aplasia of deep brachial and antebrachial veins. Light and electron microscopy revealed epiphyseal plate dystrophy and dysplasia, tubular bone osteoporosis, subcutaneous fat hyperplasia. The data obtained suggest a pathogenetic relationship between the vascular formation and

Recurrent Metacarpal Enchondroma Treated With Strut Allograft: 14-year Follow-up.

Enchondroma of the hand is a common lesion with a recurrence rate of up to 13.3% after curettage and bone grafting. Pathologic fractures often occur. Although allograft bone chips are widely used in the surgical treatment of enchondroma, the use of structural allograft bone has not been reported before. This case report presents a recurrent enchondroma of the fifth metacarpal with pathologic fracture in a 13-year-old girl who had 2 previous interventions and 2 more interventions for other enchondromas in the same hand. These interventions consisted of curettage and autogenous iliac crest bone

[Langerhans cell histiocytosis].

Langerhans cell histiocytosis is a non-neoplastic granulomalike lesion of unknown origin; it is characterized by proliferation of reticulohistiocytic structures, polynuclear eosinophils, neutrophils, lymphocytes, plasma cells, multinucleate giant cells and Langerhans cells. In recent years, the older terms "eosinophilic granuloma" and "histiocytosis X" have been replaced by the more precise expression "Langerhans cell histiocytosis". The granuloma can be solitary and multifocal, with or without soft-tissue involvement. A specific entity is Hand-Schüller-Christian disease. Characteristic lesion

[Dieterich's Disease--the rare aseptic necrosis of the metacarpal head].

The rare diagnosis of the Dieterich's disease at the head of the metacarpal III is presented on the case of a 19-year-old female patient. While aseptic bone necroses at the hand skeleton in handsurgical special departments are not rare and diagnostics and choice of the right treatment are not difficult, there are quite rare findings, which are not commonly known. Dieterich's disease finds precipitation in the literature in a handful of articles, mostly case reports. Dieterich saw among 8 patients 7 women, with a distribution of 7 concerned metacarpal bone III and one metacarpal bone IV. A pref

Finger reconstruction using induced membrane technique and ulnar pedicled forearm flap: a case report.

INTRODUCTION: Injuries referred to as "rollover hands" are associated with multiple and complex lesions of the dorsal aspect of the hand. We present a case of a multitissular reconstruction following a severe injury of the dorsum of the fingers in a 45-year-old woman. MATERIALS AND METHODS: The bone loss reconstruction was performed in two stages using the Masquelet induced membrane technique. In the first stage, a cement spacer was inserted into the phalanx bone defects. For the second stage, the membrane induced by the foreign-body reaction was opened, the spacer was removed, and an autolog

Complex Regional Pain Syndrome-like Pattern in a Case of Pancoast Tumor.

We present the case of a 52-year-old male who was recently diagnosed case of a Pancoast tumor and presented to the pulmonary outpatient department with a complaint of pain in the shoulder and chest region which was burning type, associated with shoulder abduction weakness and poor hand grip. Subsequently, he was referred for a 99m-Tc bone scan for metastatic workup, which showed increased uptake in all the joints and long bones of the ipsilateral upper limb. This case highlights the importance of considering nontraumatic cause of pattern similar to complex regional pain syndrome.

[Dorsal trapezoid-scaphoid luxation. A case report].

Dorsal trapezoscaphoid dislocations are characterised by posterolateral translation of the trapezium and the distal part of the first finger in relation to the rest of the wrist and hand. The authors found only 7 cases in the literature and they analyse the principal features in the light of a new personal case caused by violent trauma. It was easily reduced orthopaedically, but because of the instability, double pinning was performed to the adjacent bones and plaster immobilisation was maintained for six weeks. An excellent functional and radiological result was obtained and remained stable a

Humeral head osteonecrosis caused by electrical injury: a case report.

Various musculoskeletal abnormalities caused by electrical injury are described. Such abnormalities usually include fractures or dislocation of adjacent bones and joints. Osteonecrosis is a noteworthy, but less common, consequence of electric shock. The case is discussed of a 52-year-old woman who had received an electric shock (220-V alternating household current) to the right hand and developed osteonecrosis in the ipsilateral humeral head, most likely caused by bone "melting." An osteonecrotic lesion may therefore develop in a joint at a distance from the point of electrical contact, and th

[Clinico-roentgenologic semeiotics of chondroectodermal dysplasia (Ellis-van Creveld syndrome)].

The paper is devoted to the description of a rare hereditary systemic skeletal disease--chondroectodermal dysplasia (CED). The clinical symptoms of CED are divided into 4 groups. On the basis of 2 cases, symptoms of the affection of the locomotor system in patients with Ellis-van Creveld syndrome are analyzed. An x-ray picture of hand and foot lesions is characterized not only by change in the shape, size, number and synostosis of some bones but also by marked reorganization of osseous tissue in the epimetaphysial regions. X-ray examination was shown to be the chief method for investigation of

Limited wrist arthrodeses. I. The triscaphoid joint.

Limited wrist arthrodesis is the surgical fusion of selected bones of the wrist. In each case the extent of the fusion is determined by the extent of the disease process. Limited wrist arthrodesis of the joints between the scaphoid, trapezium, and trapezoid is indicated in localized degenerative arthritis, radial hand dislocations, and certain instability patterns following ligament rupture (rotary subluxation of the scaphoid). Fusion is easily obtained utilizing a local bone graft donor site. Fusion of the three-bone unit leaves approximately 80% of flexion extension and 66% of radial and uln
