Maturational changes in ventriculoatrial conduction in the intact canine heart.

This study reports on the changes in ventriculoatrial (VA) conduction that occur with maturation. Programmed atrial and ventricular premature extra-stimulation (coupled to a fixed paced cycle length) and rapid atrial pacing were performed in three groups of dogs: Group I = 8 neonates aged 5 to 14 days, Group II = 9 young dogs aged 6 to 9 weeks and Group III = 10 adult dogs. High right atrial, His bundle and right ventricular electrograms were recorded. There were no differences in the AH intervals at rest. In all but five animals, atrioventricular conduction was limited by the atrial functiona

[Prognostic significance of the intraventricular blocks during acute myocardial infarction (author's transl)].

An intraventricular conduction defect complicated 39 out of 489 cases of acute myocardial infarctions. The defect, with the exception of one case, always developed in infarctions in the anterior wall of the heart. Compared with the other acute anterior myocardial infarctions in which the mortality in the Coronary Care Unit was 15%, infarctions complicated by intraventricular conduction defects have on the whole, presented a worse clinical course and a higher mortality rate (56%). It must be stressed that clinical course and mortality rate have been very different in the various conduction defe

Cardiac sympatho-vagal balance and ventricular arrhythmia.

A hallmark of cardiovascular disease is cardiac autonomic dysregulation. The phenotype of impaired parasympathetic responsiveness and sympathetic hyperactivity in experimental animal models is also well documented in large scale human studies in the setting of heart failure and myocardial infarction, and is predictive of morbidity and mortality. Despite advances in emergency revascularisation strategies for myocardial infarction, device therapy for heart failure and secondary prevention pharmacotherapies, mortality from malignant ventricular arrhythmia remains high. Patients at highest risk or

Normal and abnormal circadian profiles of heart autonomic balance, evaluated by time-related common indicator of heart rate variability.

OBJECTIVE: Heart rate variability (HRV) is an universally accepted method for assessing the heart autonomic balance (HAB). At the same time HRV is a highly specific method, but with a low sensitivity to the cardiovascular diseases (CVD). We found that HRV indices may be applied to obtain differentiated characteristics of the HAB for individual CVD. METHODS: We assessed the changes taking place in the circadian characteristic of the autonomic balance, which has a definite profile with and without the principal CVD. We proposed adequate time-related autonomic balance indicator (TRABI) for evalu

Atrioventricular nodal function in progressive systemic sclerosis: electrophysiological and morphological findings.

Electrophysiological data consistent with atrioventricular nodal dysfunction were obtained in 10 out of 19 patients with progressive systemic sclerosis (scleroderma). Conducting system studies were carried out in a further seven patients with progressive systemic sclerosis. In each the proximal portion of the atrioventricular node was consistently found to be smaller and more slender. It is postulated that there is a relation between proximal atrioventricular nodal structural alterations and impaired atrioventricular nodal function.

Experimental creation of complete A-H block.

A new technique to block conduction from both atria to the atrio-ventricular node (A-V node) was devised through anatomical considerations of the junctional area. To assess the validity of the technique, a series of experiments were performed in mongrel dogs. Under cardiopulmonary bypass, the interatrial septum was incised along the tendon of Todaro, followed by cryosurgical ablation of the approach to the A-V node. Postoperative electrophysiological studies revealed complete conduction block between both atria and the A-V node. A spontaneous A-V nodal rhythm was produced. Initial activation w

Association Between New-Onset Type 2 Diabetes and Cardiac Conduction Diseases: A Prospective Cohort Study.

BACKGROUND: Cardiac conduction diseases can lead to life-threatening outcomes. However, the evidence on risk factors for conduction disease that is needed to underpin prevention strategies is limited. The present study aimed to determine the association between type 2 diabetes and cardiac conduction diseases. METHODS AND RESULTS: This study included 101 080 participants free of prevalent diabetes and cardiac conduction diseases at baseline from the Kailuan Study. All participants were monitored biennially until December 31, 2020. During follow-up, 14 397 participants were diagnosed as having

Association between anti-SSA autoantibodies and conduction disturbances in heart failure.

BACKGROUND: Conduction disturbances play an important role in the occurrence and development of heart failure (HF). Studies suggest autoantibodies may attack the conduction system. However, whether autoantibodies are associated with conduction disturbances in patients with HF is unclear. OBJECTIVE: The purpose of this study was to assess whether anti-SSA, anti-Ro/Sjögren syndrome-related antigen A antibodies known for congenital atrioventricular block (AVB), is associated with conduction disturbances in patients with HF. METHODS: This retrospective observational study used data from patients

Three-dimensional noncontact mapping defines two zones of slow conduction in the circuit of typical atrial flutter.

The cavotricuspid isthmus (CTI) is a slow conduction area in the circuit of typical atrial flutter. However, conventional methods are limited by the inaccuracy of measurements of distance on the surface of the heart. The aim of the study was to define the conduction properties of the atrial flutter circuit along the tricuspid annulus by using a three-dimensional noncontact mapping system. In 34 atrial flutter patients (30 men, 4 women; mean age 54 +/- 14; 27 counter-clockwise, 4 clockwise, and 3 both), a noncontact multielectrode array was used to reconstruct electrograms in the right atrium.

Stimulation and propagation of activation in conduction tissue: Implications for left bundle branch area pacing.

BACKGROUND: Characterizing wavefront generation and impulse conduction in left bundle (LB) has implications for left bundle branch area pacing (LBBAP). OBJECTIVES: The purpose of this study was to describe the pacing characteristics of LB and to study the role of pacing pulse width (PW) in overcoming left bundle branch block. METHODS: Twenty fresh ovine heart slabs containing well-developed and easily identifiable tissues of the conduction system were used for the study. LB stimulation, activation, and propagation were studied under baseline conditions, simulated conduction slowing, conducti

Voltage-gated sodium channels in the mammalian heart.

Mammalian species express nine functional voltage-gated Na(+) channels. Three of them, the cardiac-specific isoform Nav1.5 and the neuronal isoforms Nav1.8 and Nav1.9, are relatively resistant to the neurotoxin tetrodotoxin (TTX; IC50 ≥ 1 μM). The other six isoforms are highly sensitive to TTX with IC50 values in the nanomolar range. These isoforms are expressed in the central nervous system (Nav1.1, Nav1.2, Nav1.3, Nav1.6), in the skeletal muscle (Nav1.4), and in the peripheral nervous system (Nav1.6, Nav1.7). The isoform Nav1.5, encoded by the SCN5A gene, is responsible for the upstroke of t

Arrhythmia substrates accessible from the aortic root and immediate sub-aortic areas: Mapping and ablation.

The aortic root overlies the left ventricular ostium and the interatrial septum. Muscle sleeves supporting the aortic sinuses and extensions to the inter-leaflet triangle are sources for outflow tract arrhythmias. The sinuses of Valsalva, in addition, provide access to the inter-atrial septum and left ventricular infra-aortic regions, where substrates for arrhythmias reside in both idiopathic arrhythmias and structural heart disease. This review summarizes the anatomical relationship of the aortic root to the various arrhythmic substrates and discusses approaches to ablation.
