The patient was a 52-year-old man. Tube drainage was performed to the right traumatic hemothorax. Then, 2,000 ml of blood flowed out and bleeding stopped for a time. Four hours later after injury, massive bleeding occurred again. So emergency thoracotomy was performed and we found a laceration in the main pulmonary artery and injury of intrathoracic artery. Pulmonary artery disruption from blunt chest trauma is rare. Further discussion was made by reviewing the related literature.

We experienced 25 cases of spontaneous hemopneumothorax, in which 9 were defined as occult cases on chest X-ray at the time of visit. All 9 cases visited our hospital within 12 hours after the onset. Of these, 5 were done chest computed tomography (CT) and 3 could be diagnosed as hemopneumothorax. Emergency surgery were necessary in 4 of 9. Chest CT and careful observation is thought to be essential in case of suspiciously combined hemothorax in pneumothorax cases.

Internal mammary artery (IMA) aneurysms are very rare, have a high risk of rupture, and can cause hemothorax. Here, we report the case of a 33-year-old man with metachronal and bilateral IMA aneurysms. He had Marfan syndrome diagnosed by genetic testing. We carried out endovascular repair with coil embolization. He has survived without additional treatment for 7 years. Endovascular repair of metachronal and bilateral IMA aneurysms is feasible even in a patient with Marfan syndrome.

Four days after uncomplicated implantation of a two-chamber pacemaker and a normal postoperative course, a patient was referred to our hospital with left-sided hemothorax and early hemorrhagic shock. Chest X-ray and CT scan were suspicious of a right ventricular lead perforation with additional pericardial and pleural injury. Immediate surgery was performed via a lateral thoracotomy and the perforation was repaired via direct suture. An epimyocardial ventricular lead was implanted simultaneously. The patient made an uneventful recovery.

Copper deficiency was produced in weanling C57BL/6J and Hap:(ICR) mice that were fed a milk-sucrose based diet supplemented with appropriate vitamins, minerals, and amino acids. Growth retardation, decreased hematocrit, lethargy, dyspnea, achromotrichia, and high mortality (in ICR mice) were apparent in copper-deficient mice. Cardiac enlargement, hemopericardium, hemothorax, and pleural effusion were observed at necropsy. Thrombosis of the atria, myocardial degeneration and necrosis, and sites of myocardial rupture were observed at histopathologic examination.

The Extended-Focused Assessment with Sonography for Trauma (E-FAST) allows clinicians to rapidly diagnose traumatic thoracoabdominal injuries at the bedside without ionizing radiation. Pulmonary arteriovenous fistula (PAVF) is a rare anomaly in the lung, and hemothorax or massive hemoptysis due to spontaneous rupture of the fistula sac is even rarer. The patient described here was a 47-year-old woman who presented with massive hemoptysis resulting from the rupture of her PAVF just after laparoscopic operation. To our knowledge, this may be the first case ever reported that the rupture of PAVF may be correlated with a laparoscopic operation. The patient survived without adverse events after emergency pulmonary lobectomy.

The hemothorax due to rupture of arteriovenous fistula is very rare and only 6 cases of this complication have been reported in Japanese literature. 44-year-old male complained of chest pain and dyspnea. A chest roentgenogram revealed right pleural effusion and an abnormal pulmonary shadow. By further examination including pulmonary angiography, the rupture of pulmonary arteriovenous fistula into right pleural cavity was diagnosed. Partial resection of the right lung containing arteriovenous fistula was successfully performed and postoperative course was uneventful.

Catheterization of central veins is a routine technique which is widely used in emergency department, operation theater, and intensive care units. Seldinger technique is widely used to place central venous and arterial catheters and is generally considered safe. The technique does have multiple potential risks. We describe a case of hemothorax following internal jugular venous cannulation. Measures which can be taken to prevent such complications are explained in detail as well as recommended steps to remedy errors should they occur.
