Methylene Blue in the Treatment of Neuropsychiatric Disorders.

Methylene blue is a long-established drug with complex pharmacology and multiple clinical indications. Its diverse mechanisms of action are most likely responsible for the large variety of its clinical effects. Of interest to psychiatrists, methylene blue has antidepressant, anxiolytic, and neuroprotective properties documented by both animal and human studies. Its stabilizing effect on mitochondrial function and dose-dependent effect on the generation of reactive oxygen species are of significant heuristic value. For these reasons, methylene blue holds promise as a proof-of-concept treatment

A Multicenter Study Into Causes of Severe Acute Liver Injury.

The differential diagnosis of an increase in alanine aminotransferase (ALT) level and/or aspartate aminotransferase (AST) level of ≥1000 IU/L often is stated to include 3 main etiologies: ischemic hepatitis, acute viral hepatitis (typically hepatitis A and hepatitis B), and drug-induced (more specifically, acetaminophen/paracetamol) liver injury (DILI).1 Unfortunately, there are a paucity of studies examining the most common causes of acute liver injury (ALI) and those that have been published have been small,2 single-center,2 or examined less severe increases in ALT or AST levels.3,4 We condu

Quantitative characterization of optic nerve atrophy in patients with multiple sclerosis.

BACKGROUND: Optic neuritis (ON) is one of the most common presentations of multiple sclerosis (MS). Magnetic resonance imaging (MRI) of the optic nerves is challenging because of retrobulbar motion, orbital fat and susceptibility artifacts from maxillary sinuses; therefore, axonal loss is investigated with the surrogate measure of a single heuristically defined point along the nerve as opposed to volumetric investigation. OBJECTIVE: The objective of this paper is to derive optic nerve volumetrics along the entire nerve length in patients with MS and healthy controls in vivo using high-resolut

Positive activities as protective factors against mental health conditions.

Applying Nolen-Hoeksema and Watkins's (2011) transdiagnostic risk factor heuristic to our work on positive activities (i.e., practices that characterize naturally happy people, like expressing gratitude and practicing generosity), we propose that such activities may serve as protective factors that mitigate proximal risk factors both directly and by intervening with the mechanisms that give rise to them. First, we discuss theoretical and empirical support for the importance of well-being and the mechanisms that explain how positive activities promote well-being (by boosting positive emotions,

Optimizing the precision of case fatality ratio estimates under the surveillance pyramid approach.

In the management of emerging infectious disease epidemics, precise and accurate estimation of severity indices, such as the probability of death after developing symptoms-the symptomatic case fatality ratio (sCFR)-is essential. Estimation of the sCFR may require merging data gathered through different surveillance systems and surveys. Since different surveillance strategies provide different levels of precision and accuracy, there is need for a theory to help investigators select the strategy that maximizes these properties. Here, we study the precision of sCFR estimators that combine data fr

Assessment of structured socioeconomic effects on health.

Social epidemiologists study effects of variables such as education or income on health outcomes. Because other factors may influence both the exposure and the outcome, adjustments are commonly made in an effort to estimate the "independent" effect of exposure. The validity of common adjustment strategies when estimating the outcome distribution under hypothetical interventions of the exposure is potentially compromised by structured relations between covariates, observed and unobserved. These considerations of covariate structure may be particularly important for the study of "distal" socioec

Multimodal Intelligent Monitoring of Parkinson Disease: Scoping Review of Progress and Translational Challenges.

BACKGROUND: Parkinson disease (PD) is a progressive neurodegenerative disorder with a rapidly growing global prevalence. Current clinical assessments, such as the Unified Parkinson Disease Rating Scale, are limited by subjectivity and episodic application, creating a need for continuous, objective monitoring solutions. While previous reviews have often focused on single technologies, there is a growing trend toward integrating multiple data sources to provide a more holistic view of PD. OBJECTIVE: This scoping review synthesizes progress in multimodal intelligent monitoring systems for PD, fo

Virtual Reality App for Treating Eating Behavior in Eating Disorders: Development and Usability Study.

BACKGROUND: Anorexia nervosa is one of the more severe eating disorders, which is characterized by reduced food intake, leading to emaciation and psychological maladjustment. Treatment outcomes are often discouraging, with most interventions displaying a recovery rate below 50%, a dropout rate from 20% to 50%, and a high risk of relapse. Patients with anorexia nervosa often display anxiety and aversive behaviors toward food. Virtual reality has been successful in treating vertigo, anxiety disorder, and posttraumatic stress syndrome, and could potentially be used as an aid in treating eating di

PheValuator: Development and evaluation of a phenotype algorithm evaluator.

BACKGROUND: The primary approach for defining disease in observational healthcare databases is to construct phenotype algorithms (PAs), rule-based heuristics predicated on the presence, absence, and temporal logic of clinical observations. However, a complete evaluation of PAs, i.e., determining sensitivity, specificity, and positive predictive value (PPV), is rarely performed. In this study, we propose a tool (PheValuator) to efficiently estimate a complete PA evaluation. METHODS: We used 4 administrative claims datasets: OptumInsight's de-identified Clinformatics™ Datamart (Eden Prairie,MN)…

Comparing de novo genome assembly: the long and short of it.

Recent advances in DNA sequencing technology and their focal role in Genome Wide Association Studies (GWAS) have rekindled a growing interest in the whole-genome sequence assembly (WGSA) problem, thereby, inundating the field with a plethora of new formalizations, algorithms, heuristics and implementations. And yet, scant attention has been paid to comparative assessments of these assemblers' quality and accuracy. No commonly accepted and standardized method for comparison exists yet. Even worse, widely used metrics to compare the assembled sequences emphasize only size, poorly capturing the c…

From multisensory integration to synaesthesia: a boundary-preserving continuum and research framework.

Synaesthesia is commonly defined as an involuntary and consistent phenomenon in which stimulation in one sensory or cognitive domain elicits an additional experience in another domain. Yet definitions derived from perceptual psychology, neuroscience, aesthetics, cultural anthropology, and artistic practice often speak past one another. This article proposes a boundary-preserving conceptual review that uses the evolutionary history of sensing as a meta-theoretical platform for dialogue among these definitions. It makes four contributions. First, it clarifies the boundaries among congenital or d

Classifying strategy use in multiattribute subjective choice: Application to conjoint experiments in political science.

Conjoint experiments are extensively used across multiple disciplines to make inferences about preferences over multiattribute alternatives. Much previous research has analyzed the importance of attributes at the aggregate level. Here, we focus on understanding individual-level behavior and, specifically, on strategy use. We present a Bayesian cognitive model of strategy use that performs attribute weight inference (how important an attribute is), attribute level inference (which value of the attribute is favored), and latent-mixture inference to classify strategy use among well-established st

A European Blueprint for Community Nursing: Building Care Protocols on International Organization for Standardization (ISO) 7101:2023.

Care across Europe is moving out of the hospital and into homes and communities. The family and community nurse now stands at the center of this shift, holding continuous responsibility for people where they live. Yet the procedures that guide this work are often written locally and vary widely, with no shared quality architecture behind them. This editorial proposes International Organization for Standardization (ISO) 7101:2023 as a possible common European reference model for designing community nursing care protocols, anchoring each procedure to quality principles such as people-centered ca

A two-stage fuzzy flexible flow shop scheduling problem with no-wait and reentrant constraints using adaptive particle swarm optimization.

To address increasing production and logistical challenges in the fresh livestock processing industry, this study investigates a two-stage multiprocessor flow shop scheduling problem with fuzzy processing times and time window constraints. Ensuring product freshness under urban congestion and limited storage capacity requires precise and flexible scheduling to minimize early and late completion penalties. This study proposes an adaptive particle swarm optimization algorithm that integrates fuzzy processing environments with time-constrained scheduling. The main contributions are threefold: (1)

A Quadripartite Compass for Holistic Nursing: Philosophy of Science, Islamic Teleology, and Culturally Responsive Care.

A Monte Carlo-Based 3D Whole Lung Model for Aerosol Deposition Studies: Implementation and Validation.

A detailed picture of how an aerosol is transported and deposited in the self-affine bronchial tree structure of patients is fundamental to design and optimize orally inhaled drug products. This work describes a Monte Carlo-based statistical deposition model able to simulate aerosol transport and deposition in a 3D human bronchial tree. The model enables working with complex and realistic inhalation maneuvers including breath-holding and exhalation. It can run on fully stochastically generated bronchial trees as well as on those whose proximal airways are extracted from patient chest scans. Ho

On Modeling Ovarian Aging and Menopause Timing.

Mathematical modeling of ovarian aging and menopause timing has a long history, dating back a half-century to the models of Nobel Prize winner Robert G. Edwards. More recently, such models have been used to investigate clinical interventions for women, which underscores the importance of scientific rigor in model development and analysis. In this paper, we analyze a recent model published in the biophysics literature. We first correct an error which invalidates claims about menopause age in different populations. We then use stochastic analysis to show how this model is a reparameterization of
