We studied the value of ultrasound examination combined with conventional clinical evaluation in screening for congenital defects of the hip in a study including more than 3,000 neonates. We used two kinds of ultrasound methods, i.e. the morphological method, that is highly standardized and seeks to identify several well-defined anatomic landmarks (Graf's technique), and the dynamic method, that uses transverse flexion, frontal flexion or medial sections to determine the degree of stability of the hip. Early diagnosis and treatment are the keys to a successful result in infants with congenital dislocation of the hip. In the neonatal period, a majority of infants with hips that would later be found to be dislocated can be detected and effectively treated. In a retrospective review of 70 hips in 35 patients with arthrogryposis 56 of all hips studied were affected by the disorder. Clinically 4 types of arthrogrypotic hip deformity could be distinguished. In 19 hip joints not only a deformity and a restriction of movement but also a dislocation was found. The hip joint is unique anatomically, physiologically, and developmentally. Because of these features, diagnosis of pathologic conditions is more difficult than for most joints. Because delay in diagnosis and treatment may result in crippling osteoarthritis, it is imperative that diagnostic pitfalls be avoided.

We reviewed 27 teratologic hip dislocations in 17 patients. Four hips underwent closed reduction, 10 hips had medial adductor open reduction, 9 hips had iliofemoral open reduction, and 4 hips had iliofemoral open reduction and femoral shortening. Avascular necrosis occurred in 48%, redislocation in 19%, and subluxation in 22% of the hips. Results were best in those hips treated by anterior open reduction and femoral shortening and worst in those having closed reduction. Poor results correlated with multiple other involved joints, hypotonia, developmental delay, and poor head control.

Morphologically, enthesopathy appears as dystrophic calcification or ossification of tendon insertion. In the study of the initial stages of arthrosis on necroptic material roentgenologically 158 and of these histologically 62 hip joints were investigated. Traumatic dislocation of the hip (TDH) is an absolute orthopedic emergency that is increasing steadily in incidence. Sixty-two to ninety-three percent of reported cases were the result of high-speed motor vehicle accidents in which seat belts were not used. Analysis of 57 traumatic dislocations of the hip, excluding central dislocations, is reported. No anterior dislocations occurred. Road accidents caused most of the dislocations and several patients, most of them young men, had additional serious injuries. Grading the dislocations according to Stewart and Milford (15) is useful for the prognosis, and should to a greater extent be used as a therapeutic guide.
