Organisational culture and trust as influences over the implementation of equity-oriented policy in two South African case study hospitals.

BACKGROUND: This paper uses the concepts of organisational culture and organisational trust to explore the implementation of equity-oriented policies - the Uniform Patient Fee Schedule (UPFS) and Patients' Rights Charter (PRC) - in two South African district hospitals. It contributes to the small literatures on organisational culture and trust in low- and middle-income country health systems, and broader work on health systems' people-centeredness and "software". METHODS: The research entailed semi-structured interviews (Hospital A n = 115, Hospital B n = 80) with provincial, regional, distri

Content-Area Framework for Conducting Family Meetings for Acutely Ill Psychiatric Patients.

Family meetings are a critically important component of managing acutely psychiatrically ill patients, and learning how to conduct such a meeting is critically important in the training of psychiatrists. Because we found no published comprehensive tools that dealt with the biopsychosocial content areas to be covered in family meetings in acute psychiatric settings, we developed and present such a comprehensive tool that is based in part on a review of existing tools utilized by other disciplines. This article describes the specific steps involved in premeeting planning, the formal topic areas

[Emergency situations concerning eating disorders].

Patients with eating disorders often require long and multidisciplinary therapeutic care of various types. The cornerstone of this care is still the care contract. Due to its preparation and development, it allows to offset the two extreme situations, which are the denial of massive problems and the need for immediate care, and to fully invest in the therapeutic project. Emergency care is, nevertheless, sometimes necessary in response to a severe physical or psychological situation, or in case of a family crisis. When a life is at risk, emergency hospitalization is a must. In this way, hospita

Assessment of adherence problems in patients with serious and persistent mental illness: recommendations from the Expert Consensus Guidelines.

Poor adherence to medication treatment can have devastating consequences for patients with serious mental illness. The literature review and recommendations in this article concerning assessment of adherence are reprinted from The Expert Consensus Guideline Series: Adherence Problems in Patients with Serious and Persistent Mental Illness, published in 2009. [Occupational therapy and possibilities for interpersonal contact in a psychiatric department of a social psychiatry center].

Patients of the Department of Psychiatry of the Municipal Hospital Centre in Duisburg-Rheinhausen, Germany, are offered work therapy mainly by the nearby Sociopsychiatric Centre "Cuckoo's Nest". The possibilities of working and establishing contacts are described in detail after a brief review of the development and the advantage of shifting work therapy to an external, non-clinical site. Finally, the question is discussed whether the areas "work" and "leisure time" complement or substitute each other.

[Subjective construction of psychiatric hierarchy].

A survey was undertaken amongst student nurses at a psychological clinic with the aim of reconstructing the way in which the participants project values and expectations onto the established hierarchy of the institution. The method used for the survey was the Repertory-Grid-Technique; the grids were evaluated in a formal logic way. It can be seen a) how students especially favour the team as a niche at the workplace and how they seek to dissociate it from other role-bearers at the clinic, b) how patients are categorized at the very bottom of the hierarchy of values, c) how in some grids an int

Using a hospitalwide performance improvement process for patient education documentation.

BACKGROUND: Decreases in length of hospital stay have increased pressure on staff to provide appropriate patient education. A hospitalwide continuous quality improvement (CQI) program was implemented to improve patient education documentation at Raritan Bay Medical Center (RBMC; Perth Amboy, NJ). IMPROVING THE PATIENT EDUCATION DOCUMENTATION PROCESS: In March 1995 the patient education committee was given the responsibility to develop a CQI program to improve documentation of patient education. Patient interviews revealed that assessment of patients' initial knowledge was inadequate and goal s

Improving care for patients who experience miscarriage in emergency departments: a practice innovation.

Vaginal bleeding during pregnancy is a common patient presentation in emergency departments (EDs), and in some cases this will occur due to miscarriage. However, there are several barriers to effective and sensitive communication with patients experiencing a miscarriage. Women presenting to EDs who are experiencing a miscarriage are more likely to be psychosocially vulnerable and less satisfied with their care compared with those seeking care in the outpatient setting. There is a gap in nursing and advanced practice provider preparation regarding techniques for breaking bad news to patients in

Examining Geographic Variation of Opioid Use Disorder Encounters in the USA.

OBJECTIVES: The objectives were (1) to characterize patient encounters of opioid use disorder (OUD) using Health Facts® database; and (2) to identify geographic variation, patient characteristics, and facility characteristics impacting patients' reduced OUD encounters over time. METHODS: Patient encounters were included if the patient (1) was 18 years old or greater; (2) had an index encounter; (3) survived at least 30 days after the discharge. The OUD encounter was based on ICD-10 codes. The date at which a patient first had an OUD encounter was the index date. For the first objective, OUD e

Communicating psychosocial well-being in motor neurone disease to staff: results from a World Café approach.

Prior to the workshop and 2 weeks post-workshop, staff completed a questionnaire on their knowledge of this topic. A selection of

[Psychiatric emergencies in drug addiction].

The practitioner is very frequently confronted by emergencies in drug-addicted patients also having psychiatric symptomatology. In this article the authors will address emergencies related to alcohol (notably intoxication, pre-DTs and the encephalopathies); emergencies related to cannabis (notably intoxication, psychotic states and panic attacks); and emergencies related to other psycho-active substances (overdoses, drug-withdrawal, psychiatric complications related to cocaine or amphetamines). In the domain of drug addiction, as in psychiatry, the practitioner must give as much importance to
