Advances in the treatment of hepatitis C virus infection.

Therapy for chronic hepatitis C virus (HCV) infection with pegylated interferon α and ribavirin leads to suboptimal rates of viral eradication in patients with genotype 1 HCV, the most common viral strain in the United States and many other countries. Recent advances in the study of viral kinetics, host factors that predict response to antiviral therapy, and viral protein structure have established the foundation of a new era in the treatment of HCV infection. The HCV NS3/4A protease inhibitors boceprevir and telaprevir, the first 2 agents in a new and promising generation of direct-acting ant

Human guanylate-binding protein (GBP) 1 inhibits replication of severe acute respiratory syndrome coronavirus 2.

Severe respiratory syndrome coronavirus 2 (SARS-CoV-2) infections are associated with significant morbidity and mortality worldwide. Identification and characterization of intracellular proteins with antiviral activity (known as restriction factors) is a key first step towards the future development of novel host-directed antiviral therapies. In this study, we investigated the antiviral activity of 14 different interferon-stimulated gene (ISG) proteins against SARS-CoV-2. Overexpression of human guanylate binding protein (GBP) 1 resulted in potent inhibition of the ancestral SARS-CoV-2 strain,

Emerging Strategies for Antitumor Immunotherapy and Antiviral Defense Through the cGAS-STING Pathway.

The cyclic GMP-AMP synthase-stimulator of interferon genes (cGAS-STING) pathway is a central regulator of innate immunity and plays a critical role in inducing pro-inflammatory cytokines and type I interferons (IFN-I). This pathway has emerged as a promising target for cancer immunotherapy and antiviral treatments. Despite its promise, the clinical translation of STING agonists is hindered by several challenges, including structural instability, high production costs, and inefficient delivery systems. These barriers underscore the urgent need for further research and innovation to optimize STI

p53 serves as a host antiviral factor that enhances innate and adaptive immune responses to influenza A virus.

Several direct target genes of the p53 tumor suppressor have been identified within pathways involved in viral sensing, cytokine production, and inflammation, suggesting a potential role of p53 in antiviral immunity. The increasing need to identify immune factors to devise host-targeted therapies against pandemic influenza A virus (IAV) led us to investigate the role of endogenous wild-type p53 on the immune response to IAV. We observed that the absence of p53 resulted in delayed cytokine and antiviral gene responses in lung and bone marrow, decreased dendritic cell activation, and reduced IAV

The functions and inhibitors of protein tyrosine phosphatase B from Mycobacterium tuberculosis.

Mycobacterium tuberculosis (Mtb) secretes the low molecular weight protein tyrosine phosphatase B (PtpB), which disrupts key host innate immune defenses. This review synthesizes current knowledge regarding PtpB, highlighting its unique structural features, including a ubiquitin-activated dynamic lid that regulates its catalytic activity toward diverse substrates. PtpB is known to suppress pro-inflammatory cytokine production by dephosphorylating essential components of the MAPK and JAK-STAT signaling pathways. Additionally, upon activation by host ubiquitin, PtpB directly dephosphorylates plas

NSP7 Molecular Degrader Attenuates Coronaviral Infection Through the β-TrCP1/FBXO5 Axis.

NSP7 (Non-Structural Protein 7) of SARS-CoV-2 is a crucial component for viral replication and transcription. In this study, it is revealed that the host E3 ubiquitin ligase FBXO5 suppresses viral replication by mediating NSP7 lysine-48-linked ubiquitination and subsequent proteasomal degradation. Interestingly, it is also determined that NSP7 expression impairs the host antiviral response by inhibiting the ISGylation of melanoma differentiation-associated protein 5 (MDA5), a key sensor for viral RNA. Through an unbiased esiRNA screen, it is identified that NSP7 ubiquitination is coregulated b

Multiomic profiling of cutaneous leishmaniasis infections reveals microbiota-driven mechanisms underlying disease severity.

Leishmania braziliensis is a parasitic infection that can result in inflammation and skin injury with highly variable and unpredictable clinical outcomes. Here, we investigated the potential impact of microbiota on infection-induced inflammatory responses and disease resolution by conducting an integrated analysis of the skin microbiome and host transcriptome on a cohort of 62 patients infected with L. braziliensis. We found that overall bacterial burden and microbiome configurations dominated with Staphylococcus spp. were associated with delayed healing and enhanced inflammatory responses, es

Multi-cohort analysis of host immune response identifies conserved protective and detrimental modules associated with severity across viruses.

Viral infections induce a conserved host response distinct from bacterial infections. We hypothesized that the conserved response is associated with disease severity and is distinct between patients with different outcomes. To test this, we integrated 4,780 blood transcriptome profiles from patients aged 0 to 90 years infected with one of 16 viruses, including SARS-CoV-2, Ebola, chikungunya, and influenza, across 34 cohorts from 18 countries, and single-cell RNA sequencing profiles of 702,970 immune cells from 289 samples across three cohorts. Severe viral infection was associated with increas

Evolution of efficacious pangenotypic hepatitis C virus therapies.

Hepatitis C compromises the quality of life of more than 350 million individuals worldwide. Over the last decade, therapeutic regimens for treating hepatitis C virus (HCV) infections have undergone rapid advancements. Initially, structure-based drug design was used to develop molecules that inhibit viral enzymes. Subsequently, establishment of cell-based replicon systems enabled investigations into various stages of HCV life cycle including its entry, replication, translation, and assembly, as well as role of host proteins. Collectively, these approaches have facilitated identification of impo

Photodynamic therapy induces an immune response against a bacterial pathogen.

Photodynamic therapy (PDT) employs the triple combination of photosensitizers, visible light and ambient oxygen. When PDT is used for cancer, it has been observed that both arms of the host immune system (innate and adaptive) are activated. When PDT is used for infectious disease, however, it has been assumed that the direct antimicrobial PDT effect dominates. Murine arthritis caused by methicillin-resistant Staphylococcus aureus in the knee failed to respond to PDT with intravenously injected Photofrin(®). PDT with intra-articular Photofrin produced a biphasic dose response that killed bacter

Candida albicans interface infection after deep anterior lamellar keratoplasty.

PURPOSE: To report a case of interface infection by Candida albicans after deep anterior lamellar keratoplasty (DALK). METHODS: A 30-year-old man with keratoconus underwent DALK. Four weeks after surgery, the patient developed multiple infiltrates in the graft-host interface with absence of intraocular infection. Donor rim cultures grew C. albicans. Penetrating keratoplasty (PK) was performed because of worsening of the infection despite topical, local, and systemic antifungal therapy. RESULTS: Cultures carried out on the excised donor cornea confirmed donor-to-host transmission of C. albica

An Engineered Triple-Functional Nanoplatform for Effective Sepsis Therapy via Macrophage-Targeted Polo-like Kinase 1 Inhibition.

Sepsis accounts for 20% of global mortality, largely driven by dysregulated hyperactivation of macrophages that disrupts immune homeostasis. Current anti-inflammatory strategies often compromise pathogen clearance and exacerbate immunosuppression. Precisely targeting hyperactivated macrophages while preserving antimicrobial function represents a promising therapeutic approach. Through high-throughput screening of 390 kinase inhibitors in hyperactivated macrophages, we identified the polo-like kinase 1 (PLK1) inhibitor GSK461364 as a potent modulator of hyperactivation. However, its clinical tr

Integrated approaches to target nuclear receptors for managing the co-morbidity of tuberculosis and diabetes.

INTRODUCTION: The dual epidemic of tuberculosis (TB) and type 2 diabetes mellitus (T2DM) presents a critical global health challenge, as diabetic immunosuppression increases TB susceptibility while TB infection exacerbates glucose intolerance. Because nuclear receptors (NRs) regulate both metabolic pathways and infectious disease responses, they represent promising therapeutic targets. This study aimed to evaluate the therapeutic potential of targeting overlapping NRs to manage TB-T2DM comorbidity. METHODS: A comorbid mouse model was established by inducing T2DM via a high-fat diet and strept

Diagnosis and management of immune effector cell-associated enterocolitis after ciltacabtagene autoleucel.

AbstractImmune effector cell-associated enterocolitis (IEC-EC) has been observed after ciltacabtagene autoleucel (cilta-cel [Carvykti]) chimeric antigen receptor T-cell (CAR-T) therapy. It is associated with dismal outcomes and is poorly characterized. Here, we examined its clinical features, risk factors, and proposed management strategies. Among the 229 consecutive patients who received cilta-cel at Mayo Clinic, 9 (3.9%) presented with a nonresolving, nonbloody, grade 3 diarrhea, often requiring prolonged total parenteral nutrition. Gastrointestinal coinfections were seen in most patients. M

[Personalized treatment of viral hepatitis of the present and the future : Hepatitis B, C, delta, and E].

Precision medicine is also possible for infectious diseases as shown for the treatment of chronic viral hepatitis, especially if different options are available. In hepatitis B virus (HBV) infection, treatment indication as well as the choice of treatment and the decisions to stop treatment are based on viral markers and alanine aminotransferase (ALT) level. Future therapies for HBV infection aiming for functional cure or even virus elimination may be even more personalized and have to take into account the immune status of a given patient. Such treatment modalities might also increase the cha

Gut microbiota, microbial metabolites, and microbiome-directed nutritional interventions in hyperuricemia and gout: mechanisms and translational opportunities.

Hyperuricemia (HUA) and gout are increasingly prevalent metabolic disorders, imposing a substantial global health burden. Although conventional urate-lowering therapies remain the clinical cornerstone, their long-term effectiveness is frequently constrained by adverse effects, variable patient responses, and limited capacity to address underlying metabolic and inflammatory disturbances. Emerging evidence highlights the gut microbiota and its interactions with host metabolic pathways as important modulators of systemic urate homeostasis, positioning microbiome-directed nutritional strategies as

Berbamine suppresses intestinal SARS-CoV-2 infection via a BNIP3-dependent autophagy blockade.

SARS-CoV-2, the causative virus of COVID-19, continues to threaten global public health. COVID-19 is a multi-organ disease, causing not only respiratory distress, but also extrapulmonary manifestations, including gastrointestinal symptoms with SARS-CoV-2 RNA shedding in stool long after respiratory clearance. Despite global vaccination and existing antiviral treatments, variants of concern are still emerging and circulating. Of note, new Omicron BA.5 sublineages both increasingly evade neutralizing antibodies and demonstrate an increased preference for entry via the endocytic entry route. Alte

Navigating the Complex Landscape of Ebola Infection Treatment: A Review of Emerging Pharmacological Approaches.

In 1976 Ebola revealed itself to the world, marking the beginning of a series of localized outbreaks. However, it was the Ebola outbreak that began in 2013 that incited fear and anxiety around the globe. Since then, our comprehension of the virus has been steadily expanding. Ebola virus (EBOV), belonging to the Orthoebolavirus genus of the Filoviridae family, possesses a non-segmented, negative single-stranded RNA genome comprising seven genes that encode multiple proteins. These proteins collectively orchestrate the intricate process of infecting host cells. It is not possible to view each pr

Effects of Pseudevernia furfuracea (L.) Zopf on Helicobacter pylori induced gastritis in rats: anti- Helicobacter pylori and anti gastritis properties.

We evaluated the antimicrobial and host-directed effects of Pseudevernia furfuracea (L.) Zopf (P. furfuracea) against Helicobacter pylori (H. pylori) associated gastritis. The acetone extract inhibited H. pylori growth in vitro, with a minimum inhibitory concentration (MIC) of 15.625 µg/mL, and modestly reduced pre-formed biofilms. In a rat model of indomethacin- and H. pylori induced gastritis, oral P. furfuracea treatment lowered gastric bacterial load and urease activity, shifted the cytokine profile toward an anti-inflammatory pattern (reduced interleukin-1β [IL-1β] and restored interleuki

MASLD beyond the liver: decoding the gut-genetic-metabolic nexus.

Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly non-alcoholic fatty liver disease, is the most prevalent chronic liver disorder globally and is tightly linked to obesity, diabetes, dyslipidemia, and metabolic syndrome. MASLD spans a continuum from simple steatosis to metabolic dysfunction-associated steatohepatitis, fibrosis, cirrhosis, and hepatocellular carcinoma. Its pathogenesis is driven by complex interactions among host genetics, metabolic stress, dietary factors, gut microbiota dysbiosis, immune dysregulation, and oxidative injury. The gut-liver axis plays a
