[Osteosynthesis in dislocated fractures of the humerus head].

The operative treatment of displaced fractures of the proximal humerus has changed during the last few years. Minimal osteosynthesis takes into consideration to a great extent the biological aspects of these fractures and provides sufficient stability for early functional therapy. Neer's classification has proved very useful for both the indication for and the choice of an operative procedure. The high necrosis rate of the operatively stabilized humeral head has considerably influenced the operative strategy. This is shown in two separately controlled patient groups. From 1970 to 1980, 65 pati…

Shoulder Osteonecrosis: Pathogenesis, Causes, Clinical Evaluation, Imaging, and Classification.

The humeral head is the second most common site for nontraumatic osteonecrosis after the femoral head, yet it has attracted relatively little attention. Osteonecrosis is associated with many conditions, such as traumatism, corticosteroid use, sickle cell disease, alcoholism, dysbarism (or caisson disease), and Gaucher's disease. The diagnosis is clinical and radiographic with MRI, with radiographs being the basis for staging. Many theories have been proposed to decipher the mechanism behind the development of osteonecrosis, but none have been proven. Because osteonecrosis may affect patients w…

[Fresh and habitual dislocation of the shoulder joint (author's transl)].

In case of the most common anterior-inferior dislocation of the shoulder joint the humeral head is forced out of the glenoidal cavity and tears the capsule. In about 20% of cases the dislocation is complicated by tears of the rotatory cuff, avulsion fractures and lesions of vessels and nerves. Reduction as soon as possible according to the method of Arlt or Hippokrates is recommended to avoid further damage. X-ray examination in 2 directions is necessary to confirm the position of the head. Clinical examination must be done to detect lesions of nerves or vessels caused by the procedure. In cas…

Anatomic Reconstruction of Acute Posterior Shoulder Instability With Reverse Hill-Sachs Lesions: A Report of Two Cases.

Posterior shoulder dislocation associated with a reverse Hill-Sachs lesion is an uncommon injury that may result in persistent instability when the humeral head defect is engaged. Management depends on lesion size, engagement, chronicity, and reconstructibility; both non-anatomic procedures and anatomic reconstruction strategies have been described for acute lesions within the joint-preserving range. We present two cases of acute posterior shoulder instability with engaging reverse Hill-Sachs lesions treated with a combined arthroscopic-open approach. In both patients, arthroscopic posterior l…

Bursal Acromial Resurfacing Improves Shoulder Biomechanics in a Cadaveric Model of Massive Rotator Cuff Tear.

PURPOSE: To investigate the effectiveness of bursal acromial resurfacing (acromiograft) on acromiohumeral distance, subacromial contact area, and pressure in a cadaveric model of massive rotator cuff tear. METHODS: Eight fresh-frozen cadaveric shoulders were tested using a customized shoulder testing system. Humeral head translation, subacromial contact pressure, and the subacromial contact area were evaluated across 4 conditions: (1) intact shoulder; (2) simulated massive rotator cuff tear, (3) 3-mm acromiograft condition, and (4) 6-mm acromiograft condition. The acromiografts were simulated…

Effectiveness of replacement arthroplasty with calcar grafting and avoidance of greater tuberosity osteotomy for the treatment of humeral surgical neck nonunions.

A retrospective review was conducted on 12 patients (mean age, 63 years) undergoing replacement arthroplasty with calcar grafting for humeral surgical neck nonunions. Patients were evaluated preoperatively for pain and range of motion and postoperatively by use of the American Shoulder and Elbow Surgeons and Constant scores. Indications included severe pain, disability, humeral head destruction, and inadequate bone stock. Nine patients were available for a mean of 69 months' follow-up. Eight underwent humeral head replacement, and one underwent total shoulder replacement. None required greater…

Secondary radial head dislocation and dysplasia of the lateral condyle after elbow trauma in children.

We report five cases of a rare complication of childhood fractures of the elbow region. The complication consists of posttraumatic dissolution of the lateral humeral condyle followed by secondary radial head overgrowth and dislocation. The initial injuries ranged from displaced lateral condyle fractures (three patients) to a supracondylar fracture and an open elbow dislocation. Dysplasia of the lateral humeral condyle was first noted 1 to 4 years after the trauma (mean, 2.5 years) and seemed to be caused by removal of the displaced fracture fragment in one patient, and possibly by malfixation

Technical tips for fixation of proximal humeral fractures in elderly patients.

Despite the application of modern locking plate technology, complications remain common after fixation of proximal humeral fractures in elderly patients. Varus deformity and intra-articular hardware are most often responsible; fortunately, both of these complications can be avoided. Recent advances in imaging, reduction techniques, fixation methods, and postoperative care have made surgical outcomes more reliable. Particular attention should be directed to obtaining high-quality fluoroscopic images, avoiding varus reductions, supporting the osteoporotic humeral head, using appropriate screw le…

Reverse shoulder arthroplasty following a diagnosis of bilateral posterior shoulder dislocation secondary to electric shock: a case report and literature review.

A 62-year-old man presented with bilateral posterior shoulder dislocations 2 months after sustaining an accidental electric shock while fishing. Imaging confirmed chronic locked posterior dislocations with reverse Hill-Sachs lesions and large humeral head articular defects (approximately 60% on the left and 55% on the right), together with partial-thickness supraspinatus and subscapularis tendon tears. The patient underwent staged reverse total shoulder arthroplasty. At the 12-month follow-up, he was free from pain with a markedly improved active range of motion (forward flexion, 130°; abducti…

Use of the subscapularis preserving technique in anatomic total shoulder arthroplasty.

Subscapularis tenotomy for total shoulder arthroplasty has been the standard approach for shoulder surgeons that utilize the deltopectoral approach. The risk of subscapularis insufficiency after this approach has been well documented. In order to avoid subscapularis complications, Lafosse reported a technique for total shoulder arthroplasty that utilizes a trans-deltoid approach through the rotator interval that yielded satisfactory clinical outcomes. However, he also reported concerns about humeral head undersizing and inadequate osteophyte resection. We present an alternative subscapularis p…

Biomechanical investigation of arm position on deforming muscular forces in proximal humerus fractures.

BACKGROUND: Muscular forces drive proximal humeral fracture deformity, yet it is unknown if arm position can help mitigate such forces. Our hypothesis was that glenohumeral abduction and humeral internal rotation decrease the pull of the supraspinatus and subscapularis muscles, minimizing varus fracture deformity. METHODS: A medial wedge osteotomy was performed in eight cadaveric shoulders to simulate a two-part fracture. The specimens were tested on a custom shoulder testing system. Humeral head varus was measured following physiologic muscle loading at neutral and 20° humeral internal rotat…

Long-term outcome after total scapulectomy for the treatment of osteosarcoma in a cat.

CASE SUMMARY: A 10-year-old neutered female domestic shorthair cat was presented for acute lameness of the right forelimb after a pathological fracture caused by scapular osteosarcoma. Total scapulectomy was performed, including the glenoid, and the humerus was sutured to the second rib via a bone tunnel in the humeral head. Two months postoperatively, the cat showed mild lameness without any other impairment or signs of pain. The owner reported that the cat was back to normal activity, including climbing and running. Follow-up checks 6 and 18 months after surgery were unremarkable. At the fin…

Open anterior glenohumeral dislocation with associated supraspinatus avulsion: A case report.

BACKGROUND: Open dislocation of the glenohumeral joint is a very rare complication with only six cases described in the literature. Most cases resulted in severe complications like avascular necrosis of the humeral head or septic arthritis with poor functional results. The "mandatory" treatment of choice is surgery, with accurate debridement and reconstruction of the damaged soft tissues. However, the results in these cases do not approach those seen in classical dislocations. CASE SUMMARY: This case report is the first description of an anterior open glenohumeral dislocation without associat…
