Idiopathic normal pressure hydrocephalus: a systematic review of diagnosis and outcome.

OBJECTIVE: Patient selection for cerebrospinal fluid diversion is difficult, because idiopathic normal pressure hydrocephalus (INPH) mimics other neurodegenerative disorders and no findings reliably predict outcome. The literature was reviewed to identify diagnostic criteria that predict shunt response and to formulate prognostic expectations. METHODS: MEDLINE was searched, and 44 articles meeting predetermined criteria were included. RESULTS: Clinical series were frequently retrospective with small patient numbers and unstandardized outcome evaluation. Clinical findings suggestive of shunt

[Postural and locomotor evaluation of normal pressure hydrocephalus: a case report].

Generally, the diagnosis of idiopathic normal pressure hydrocephalus (NPH) is raised in elderly patients with an association of gait disturbances, cognitive impairment, urinary incontinence and widening of the ventricles. Gait disturbances appear as the predominant clinical symptom of NPH, and may precede other symptoms. The main objectives of this study were to determine the effect of ventriculoperitoneal shunt (VPS) on gait and to compare the results obtained in HPN with those obtained in Parkinson's disease (PD). The performance of a patient with NPH on specific postural and locomotion task

Tau protein is a potential biological marker for normal pressure hydrocephalus.

A biological marker for normal pressure hydrocephalus (NPH) is beneficial for evaluation of its severity and of indications for shunt operation. Tau protein was initially considered as a biological marker in cerebrospinal fluid (CSF) from Alzheimer's patients. Recently, it has been demonstrated that degeneration in the brain causes elevation of tau in CSF. Therefore, the tau level in CSF from NPH patients was evaluated. Tau levels in CSF from NPH patients were significantly higher than that in controls. The tau levels were correlated with the severity of dementia, urinary incontinence, and gai

[Psychophysiology, signs and symptoms of disease the course of normal pressure hydrocephalus].

Between May 1982 until January 1997 we investigated 200 patients suspected for normal pressure hydrocephalus (NPH) by performing an intrathecal infusion test. According to our experiences gait ataxia is the leading symptom of NPH. Regarding dementia we could not find a significant difference compared to cerebral atrophy. Bladder incontinence can be characterised as a symptom of late-stage NPH. The complete Hakim trias should not be overestimated in differential diagnostic considerations. Graduation of NPH and cerebral atrophy after the results of the infusion test in an early and late stage en

Gait abnormality is not the only motor disturbance in normal pressure hydrocephalus.

Thirteen different motor and balance functions were examined in 76 patients age 65 +/- 13 (mean +/- SD) years with normal pressure hydrocephalus (NPH) before and 3 months after a ventriculoperitoneal shunt operation. Preoperatively, the tests were performed before and after lumbar tap of 50 cc cerebrospinal fluid. The patients showed significant improvement in all thirteen functions at the 3-month examination. The improvement was approximately 25% in all the motor functions, except for balance, in which the improvement was even more striking. Different etiological subgroups of NPH patients, in

[Acetazolamide: an alternative to shunting in normal pressure hydrocephalus? Preliminary results].

From a series of 15 consecutive patients with a normal-pressure hydrocephalus examined over 2 years. 10 showed frank improvement with oral acetazolamide. The drug was a first-choice treatment in 5 cases and was given up after a spinal tap in 5 cases. Clinical response occurred even in the most severe cases, although it was slightly less impressive for intellectual impairment than for gait or bladder disturbances. Tolerance was excellent with a daily dose of 250 to 500 mg. The benefit remained stable on a more than 1 year follow-up in 8 cases. We suggest that acetazolamide should be tried in pa

Computed tomography of the brain in the diagnosis of and prognosis in normal pressure hydrocephalus.

Thirty-eight patients with normal pressure hydrocephalus were examined by CT before and after a ventriculo-peritoneal shunt operation. Evans ratio, periventricular hypodensity and width of hemispheric sulci, sylvian fissures, cella media, temporal horns and third and fourth ventricle were examined. Twenty-eight patients improved after the operation while 10 were unchanged (non responders). Those patients who improved had more often enlarged third ventricle, enlarged temporal horns and normal sylvian fissures than those who did not improve. No single CT parameter or combination of CT parameters

[Normal pressure hydrocephalus in geriatric patients--a treatable form of dementia].

Six geriatric patients are presented who were treated because of normal pressure hydrocephalus. A tentative diagnosis was made if dementia, gait disturbance, and incontinence together with a typical CAT-scan were present. In these cases an operation was carried out. After ventriculo-peritoneal shunting clinical improvements were seen in four patients. Postoperative complications were hygromas in three patients and a shunt-dislocation in another patient. A chronic subdural bleeding was recorded also in one of these patients with only a temporary success in this case. Two patients showed no clin

[Biochemical investigation of normal pressure hydrocephalus in the assessment of shunt effectiveness].

It is well known that, among patients of normal pressure hydrocephalus (NPH), there is a group in which a shunting procedure had no effect on improving clinical signs (shunt-ineffective group). To differentiate the shunt-effective group from the shunt-ineffective group, we performed measurements of cerebrospinal fluid (CSF) levels of uric acid (UA) and gamma-aminobutyric acid (GABA) in patients of NPH. We investigated the age-related CSF levels of UA and GABA in the normal subjects and compared them with those in the patients of NPH including the shunt-ineffective group and other related state

[A case of pineocytoma presenting with symptoms like normal pressure hydrocephalus].

A pineocytoma in an old man, whose initial symptoms resembled a normal pressure hydrocephalus, is reported. This 67-year-old man gradually became uncommunicative and difficult to walk alone in three months. Just before visiting our clinic, his family also noticed his nocturnal urinary incontinence. CT scan on admission disclosed tumor in the posterior wall of the third ventricle, and subsequent hydrocephalus. This oval, isodense tumor was homogeneously enhanced after the injection of contrast medium on CT scan. The vertebral angiography showed a mass in the pineal region with no vascular stain

Prognosis of dementia in normal-pressure hydrocephalus after a shunt operation.

Forty patients who had normal-pressure hydrocephalus were examined neuropsychologically before and 12 months after a ventriculoatrial shunt operation. Comparison of the preoperative and postoperative test results showed that cognitive functions improved in 16 patients, were unchanged in 19, and deteriorated in 5. Moreover, we found that the outcome of the operation depended on patient selection criteria. When three or more of the following preoperative signs were present (known cause, short history, low cerebrospinal fluid outflow, small sulci and/or periventricular hypodensity indicated by co

The predictive value of conductance to outflow of CSF in normal pressure hydrocephalus.

Conductance to outflow of CSF was measured in a prospective study of 80 patients with normal pressure hydrocephalus. Measured outflow was compared with clinical findings, computed tomography, intracranial pressure measurements and with the results of shunting. Follow-up time was one year. Outflow measurements predicted the outcome of shunting in 96 per cent of the patients. Patients with the combination of known aetiology, short history, gait disturbances, severe dementia, urinary incontinence and hydrocephalus on CT can be shunted without further investigations. Periventricular hypodensity on

Results and complications in 55 shunted patients with normal pressure hydrocephalus.

This retrospective study of 55 shunted patients with "normal pressure" hydrocephalus evaluates the surgical results. Sixty percent of these patients had objective improvement following cerebrospinal fluid diversion but 44% had shunt-related complications of which 9% were fatal. Those patients who received a single theco-peritoneal shunt had a higher improvement rate and a lower incidence of postoperative complications. Successfully treated complications did not preclude postoperative improvement.

Relationship between disproportionately enlarged subarachnoid-space hydrocephalus and white matter tract integrity in normal pressure hydrocephalus.

Normal pressure hydrocephalus (NPH) patients had altered white matter tract integrities on diffusion tensor imaging (DTI). Previous studies suggested disproportionately enlarged subarachnoid space hydrocephalus (DESH) as a prognostic sign of NPH. We examined DTI indices in NPH subgroups by DESH severity and clinical symptoms. This retrospective case-control study included 33 NPH patients and 33 age-, sex-, and education-matched controls. The NPH grading scales (0-12) were used to rate neurological symptoms. Patients with NPH were categorized into two subgroups, high-DESH and low-DESH groups, b
