BACKGROUND: Testicular vein syndrome is a rare and under-recognized cause of ureteral obstruction due to extrinsic compression by a dilated or tortuous testicular vein. With fewer than 20 cases reported in the literature, it often presents with nonspecific symptoms, and no standardized diagnostic criteria currently exist.
CASE PRESENTATION: We report the case of a 35-year-old Arab male with a 2-year history of intermittent left flank pain without urinary symptoms or nephrolithiasis. Imaging studies revealed left-sided hydronephrosis with no calculi. Contrast-enhanced computed tomography in th…

INTRODUCTION: Prenatal urologic consultation (PUC) is now offered to many prospective parents for fetal prenatal hydronephrosis (PH). This study aims to describe patient- and community-level differences in prenatal and postnatal urologic care patterns for patients with PH and to identify potentially modifiable factors that would improve timely access to care.
METHODS: We identified all patients with PH aged <6 mos seen at a single freestanding children's hospital from January 2021 to December 2022. Data were collected on patient gender, race/ethnicity, characteristics of upper tract dilatatio…

Neonatal hydronephrosis is a common urinary tract anomaly often detected antenatally through routine fetal ultrasonography, requiring postnatal evaluation to determine its cause, severity, and need for intervention. We report the case of an infant diagnosed with isolated unilateral hydronephrosis shortly after birth, with serial imaging confirming persistent severe pelvicalyceal dilatation without contralateral involvement. Due to progressive obstruction and impaired drainage, the patient underwent unilateral pyeloplasty at six months of age. The postoperative course was uneventful, and follow…

OBJECTIVE: To evaluate whether grade 4-5 vesicoureteral reflux (VUR) can be predicted from renal ultrasound (RUS) findings and perform voiding cystourethrograms (VCUGs) only on high-risk patients.
METHODS: The RUS and VCUG images of infants with prenatally detected hydronephrosis admitted to our institution between 2003 and 2013 were re-evaluated. The UTI episodes were collected retrospectively from patient journals. Patients with complex urinary tract anomalies were excluded.
RESULTS: One hundred eighty, 44 female and 136 male, patients (352 renal units (RU)), 23 (30 RU) of them having grad…

OBJECTIVE: Renal disorders can cause hypertension, but less is known about the influence of hydronephrosis on blood pressure. Hydronephrosis due to pelvo-ureteric junction obstruction (PUJO) is a fairly common condition (incidence in newborns of 0.5-1%). Although hypertensive effects of hydronephrosis have been suggested, this has not been substantiated by prospective studies in humans [1-3]. Experimental studies with PUJO have shown that animals with induced hydronephrosis develop salt-sensitive hypertension, which strongly correlate to the degree of obstruction [4-7]. Moreover, relief of the…

INTRODUCTION: The objective of this study is to clarify whether symphysiotomy is an essential procedure combined with the laparoscopic pyeloplasty for the surgical treatment of ureteropelvic junction obstruction related to horseshoe kidney.
METHODS: We retrospectively reviewed five horseshoe kidney patients with symptomatic hydronephrosis who underwent laparoscopic transperitoneal Anderson-Hynes pyeloplasty without symphysiotomy between July 2002 and October 2011.
RESULTS: All procedures were completed successfully without open conversion. Mean operative time and estimated blood loss were 20…
