He also developed hypernatremia (serum sodium 155 mEq/L) on day 3 of the desmopressin and had an inappropriately increased urine output consistent with NDI. The most likely etiology for the NDI was liposomal amphotericin B and its associated hypokalemia. DISCUSSION: The observati …

Panhypopituitarism from neurosarcoidosis in an adolescent: case report and literature review.

Background Neurosarcoidosis is not a common entity from an endocrinology standpoint. Prompt diagnosis is essential in order to determine appropriate treatment and to establish prognosis. Case presentation Our case illustrates the difficulties encountered in making a diagnosis of sarcoidosis in an adolescent who presented with polyuria, polydipsia and hypernatremia. She developed panhypopituitarism and her deficits did not improve after treatment. Conclusion Pediatric endocrinologists diagnose and treat panhypopituitarism associated with many conditions. Neurosarcoidosis should be considered as…

Hypernatremic myopathy caused by a hypothalamic mixed germ cell tumor mimicking polymyositis.

Hypernatremic myopathy was rarely reported in the literature and its clinical features have never been well-described. We present a 22-year-old man who had adipsic hypernatremia manifested with progressive proximal muscle weakness and remarkably high creatine kinase level that has never been reported in the cases of hypernatremic myopathy. His initial presentations were similar to that of polymyositis without the evidence of central nervous system dysfunction and hypopituitarism. The serum level of sodium at the beginning of myopathy is the lowest known in the literature. All the clinical pres…

Transient, traumatically induced, central diabetes insipidus in a dog.

A 9-year-old mixed-breed dog was evaluated for dental malocclusion secondary to mandibular fractures that had been repaired after the dog had been hit by a car. The dog had hypernatremia, high plasma osmolality, low urine osmolality, and hyposthenuria with adequate fluid administration. Skull radiography revealed a fracture line at approximately the level of the pituitary fossa. Administration of exogenous vasopressin resulted in an increase in urine specific gravity and urine osmolality, a decrease in serum osmolality, and a normalization of serum sodium concentrations. Follow-up evaluation r…

Central Diabetes Insipidus Following Steroid Replacement for Hypoadrenalism in Pembrolizumab-Induced Hypophysitis.

Immune checkpoint inhibitors are increasingly associated with complex immune-related endocrine adverse events that may evolve dynamically over time. We report a rare case of pembrolizumab-induced hypophysitis complicated by central diabetes insipidus, which was unmasked following glucocorticoid replacement. A 68-year-old female patient receiving immunotherapy for lung adenocarcinoma presented with hyponatremia and secondary adrenal insufficiency, followed by abrupt severe hypernatremia after steroid initiation. Subsequent evaluation confirmed central diabetes insipidus, and treatment with desm…

Osmotic demyelination syndrome diagnosed by Magnetic Resonance Imaging in a child with tetraplegia: Case report.

Osmotic demyelination syndrome (ODS) is a rare acute demyelinating process that typically develops within days following osmotic stress. Although most commonly associated with the rapid correction of hyponatremia, chronic hypernatremia has also been reported as an etiological factor. Herein, we present a 16-year-old female patient who developed ODS secondary to electrolyte imbalance following treatment for acute myeloid leukemia (AML). Brain Magnetic Resonance Imaging (MRI) plays a pivotal role in enabling accurate diagnosis. Due to the involvement of both upper motor neurons (UMN) and lower m…

Primary antiphospholipid syndrome and panhypopituitarism: a unique presentation.

Lymphocytic hypophysitis (LH) has been described previously in systemic lupus erythematosus (1.3%), Sjögren's syndrome (0.8%). Lymphocytic hypophysitis (LH) is rarely associated with rheumatic diseases, although three cases of pituitary disease associated with antiphospholipid syndrome (APS) have been described. Here, we report a possible association between APS and LH for the first time. A 34-yr-old woman with primary APS presented with polyuria, polydipsia, hypernatremia and impaired vision. Her hormone profile was compatible with panhypopituitarism, and sellar magnetic resonance imaging (MR…

Autosomal dominant nephrogenic diabetes insipidus in one family caused by a novel AQP2 mutation.

A 9-month-old male presented with vomiting and dehydration with mild hypernatremia in the context of failure to thrive. He was later diagnosed with nephrogenic diabetes insipidus (NDI) during this hospitalisation and was also found to have eosinophilic esophagitis (EoE). He has since been growing well after EoE and NDI were properly managed. Molecular genetic testing revealed an unreported deletion in AQP2 which was deemed pathogenic and of autosomal dominant inheritance when correlated with his clinical findings and family history. This case report describes the clinical course of this patien…

Hypernatremic dehydration due to lactation failure in an exclusively breastfed neonate.

We report a 13-day-old exclusively breastfed neonate, admitted with a history of fever, poor feeding, lethargy and decreased urine output. The mother had history of lactation failure. The neonate had severe hypernatremia, acute renal failure and metabolic acidosis. Renal ultrasound was normal, but the sodium level in mother's milk was 96 mEq/L (normal 7 ± 2 mEq/L). The neonate required peritoneal dialysis on the second day of admission. The biochemical investigations gradually returned to normal and the neonate was discharged on day 12 of admission. We highlight the importance of lactation man…

[Diabetes insipidus in a pacient with multiple sclerosis].

Multiple Sclerosis (ME) is a chronic progressive disease characterized by relapses of demyelination that can occur anywhere in the brain stem, spinal cord and optic nerve. Since central diabetes insipidus (DI) is mainly caused by central nervous system damage (such as trauma, surgery, tumor, infection, sarcoidosis), ME is included among its possible etiologies. However, this association is not commonly described. The clinical suspicion must be made in the presence of polyuria and polydipsia or refractory hypernatremia (in patients without free access to water) during the evolution of ME. We wi…
