[Treatment of postoperative pain in elderly oncology patients with intravenous administration of a 50% glucose solution].

Postoperative pain is the most important factor od so called "tumor-promotive effect of surgery" i.e. of endocrine-metabolic changes having the consequence drop in immune, antiinfective and antitumor defense. Due to presence of organic involutive changes, old people (= 65 years), often have serious side effects during application of usual analgetics. Since hypertonic glucose (33%) given i.v. or per os, works analgesically in small children there is assumption that it can be used in treatment of postoperative pain in old oncology patients. We tested the hypothesis that postoperative pain in old

Benefit of slow infusion of hypertonic saline/dextran in swine with uncontrolled aortotomy hemorrhage.

In laboratory models of uncontrolled hemorrhage, immediate resuscitation from hemorrhage is associated with high mortality. However, in clinical practice, resuscitation is often delayed and the rate of fluid administration is limited. We hypothesized that a slow rate of infusion after delayed resuscitation, reflecting the clinical environment, might improve survival in the presence of uncontrolled hemorrhage. To investigate the rate of administration in the presence of delayed resuscitation, we subjected anesthetized swine weighing 35 to 45 kg to wire suture abdominal aortotomy that resulted i

Induction of renal tumor necrosis factor-α and other autacoids and the beneficial effects of hypertonic saline in acute decompensated heart failure.

Although administration of hypertonic saline (HSS) in combination with diuretics has yielded improved weight loss, preservation of renal function, and reduction in hospitalization time in the clinical setting of patients with acute decompensated heart failure (ADHF), the mechanisms that underlie these beneficial effects remain unclear and additional studies are needed before this approach can be adopted on a more consistent basis. As high salt conditions stimulate the production of several renal autacoids that exhibit natriuretic effects, renal physiologists can contribute to the understanding

Regulatory volume increase and regulatory volume decrease responses in HL-1 atrial myocytes.

BACKGROUND/AIMS: we have investigated whether cultured cardiomyocytes of the cell line HL-1 have the ability to perform regulatory volume responses both in hypotonic and hypertonic conditions. Furthermore, we characterized those regulatory responses and studied the effects of bumetanide and DIDS in volume regulation of HL-1 cells. METHODS: we used a light scattering system to measure the transient volume changes of HL-1 cells when subjected to osmotic challenge. RESULTS: We found that HL-1 cells correct for their volume excess by undergoing regulatory volume decrease (RVD), and also respond

Modified perfusion fluid for renal transplantation in developing countries: our initial experience.

INTRODUCTION: Organ preservation is an important step in transplantation. Perfusion fluid plays a vital role in this matter. As in many other developing countries, perfusion fluid is not readily available and is costly. Alternatively, the commonly used Hartmann's solution does not have preservation quality. Hence, to meet the demand, a modified perfusion fluid was prepared following standard preservative flush principles. MATERIALS AND METHODS: Hartmann's solution was modified by adding potassium chloride, magnesium sulfate, mannitol, dexamethasone, and insulin. The whole process was performe

Renal responses to plasma volume expansion and hyperosmolality in fasting seal pups.

Renal responses were quantified in northern elephant seal (Mirounga angustirostris) pups during their postweaning fast to examine their excretory capabilities. Pups were infused with either isotonic (0.9%; n = 8; Iso) or hypertonic (16.7%; n = 7; Hyper) saline via an indwelling catheter such that each pup received 3 mmol NaCl/kg. Diuresis after the infusions was similar in magnitude between the two treatments. Osmotic clearance increased by 37% in Iso and 252% in Hyper. Free water clearance was reduced 3.4-fold in Hyper but was not significantly altered in Iso. Glomerular filtration rate incre

A comparison of hypertonic to isotonic fluid in the resuscitation of brain injury and hemorrhagic shock.

We studied the early and late effects of hypertonic resuscitation (HR) on the injured brain using a porcine model of hemorrhagic shock and focal cryogenic brain injury. After shock, swine were randomly assigned to receive a bolus (4 cc/kg) of either Ringers lactate (RL) or 7.5% hypertonic saline in 6% Dextran 70, followed by either RL or hypertonic sodium lactate to restore mean arterial pressure to baseline. All animals were studied for 24 hr after the start of resuscitation. Bolus HR improved cerebral blood flow (CBF) with a lower intracranial pressure (ICP) than RL. Continued hypertonic res

The efficacy of iso- and hyperosmotic fluids as volume expanders in fixed-volume and uncontrolled hemorrhage.

Rapid blood volume expansion is the goal in prehospital hemorrhage resuscitation. A comparison was made for fixed volume and uncontrolled hemorrhage between three fluid regimes: Ringer's lactate (RL), Macrodex (MD), or a small volume of a hyperosmotic solution (HSD, 7.5% NaCl/6% dextran 70) followed by RL. A mathematical model was developed to simulate blood volume restoration for these given hemorrhage and resuscitation situations. Model predictions for a fixed hemorrhage (35%) and reinfusion of each fluid regime (1 mL.min-1.kg-1) were found to agree with hematocrit and osmolality changes fro

Involvement of adenosine triphosphate-sensitive potassium channels in the response of membrane potential to hyperosmolality in cultured human aorta endothelial cells.

The membrane potential of endothelial cells is an important determinant of endothelial functions, including regulation of vascular tone. We investigated whether adenosine triphosphate-sensitive potassium (K(ATP)) channels were involved in the response of membrane potential to hyperosmolality in cultured human aorta endothelial cells. The voltage-sensitive fluorescent dye, bis-(1,3-diethylthiobarbiturate)trimethine oxonol, was used to assess relative changes in membrane potential semiquantitatively. To investigate the effect of mannitol-, sucrose-, and NaCl-induced hyperosmolality on membrane p

The effects of electrolyte, lactate, high-concentrated glucose, and dialysate on peritoneal mesothelial cells.

In order to investigate which component of dialysate is responsible for mesothelial cell damage, we studied the chronic cytotoxicity to mesothelial cells by using four types of solution. Four different types of solution (Ringer's solution, lactate, high-concentrate glucose, and 4.25% Dianeal) were prepared and injected into the abdominal cavity of five ddY-mice, 5 mL every day for one month. The monolayer of the mesothelial cells was carefully removed from the liver surface by the imprint technique. Then, the mesothelial cell damage was evaluated by using trypan blue staining and comparing the

Light scattering and cell volumes in osmotically stressed and frozen-thawed cells.

Recent reports, indicating that under some conditions the intensity of light scattering from cells is a nonlinear function of cell volume, have led to the widespread generalization that intensity of low-angle light scattering indicates cell size. This study was performed to measure the relationships between light scattering and cell volumes in an-isotonic solutions and after a freeze-thaw stress. Cell volumes in isolated human lymphocytes, human granulocytes, and hamster fibroblasts were deliberately altered by exposure to anisotonic solutions. Boyle-vant Hoff plots of cell volume as a functio

Successful extended lung preservation with UW solution.

Maximum preservation times of 4-6 hr continue to plague lung transplantation. The high-potassium colloid University of Wisconsin solution (UWS) has proved superior to the crystalloid modified Eurocollins' solution (ECS) for preservation of the liver, kidney, and pancreas. The purpose of this study was to compare UWS and ECS for extended lung preservation using a technique of combined pulmonary and bronchial artery perfusion. Simultaneous pulmonary artery and bronchial artery (via a closed aortic segment) perfusion was employed to harvest the lungs of ten mongrel dogs (wt 25-35 kg) using either

Factors increasing severity of peritonitis in long-term peritoneal dialysis patients.

Peritonitis is the most frequent complication and a leading cause of discontinuation of peritoneal dialysis (PD). Intact epithelial lining, sufficient blood flow, and adequate immunologic responses are vital to eradicate infection. In long-term PD, various pathological changes such as denudation of peritoneal mesothelial cells, duplication of submesothelial and/or capillary basement membranes, submesothelial fibrin deposit, and peritoneal fibrosis have been reported. Causes of these changes of the peritoneum are multifactorial. Commonly used dialysis solutions that are acidic, hypertonic, cont

Platysmectomy: an effective intervention for facial synkinesis and hypertonicity.

OBJECTIVES: To describe a procedure to permanently address platysmal synkinesis and hypertonicity and to report changes in quality of life associated with platysmectomy using the Facial Clinimetric Evaluation instrument. METHODS: Chemodenervation significantly relieves platysmal synkinesis in almost all patients with significant face and neck synkinesis associated with dynamic facial expressions. We recently began to offer platysmectomy as part of a permanent solution to chronic superficial torticollis-like neck symptoms. For a 10-month period, 24 patients underwent the procedure, and preoper

Stimulation of erythrocyte phosphatidylserine exposure by chlorpromazine.

Side effects of treatment with chlorpromazine include anaemia which could result from decreased formation or accelerated clearance of circulating erythrocytes. Recently, a novel mechanism leading to erythrocyte clearance has been disclosed. Osmotic shock, oxidative stress and glucose deprivation lead to activation of cation channels, Ca(2+) entry, activation of a Ca(2+)-sensitive erythrocyte scramblase and subsequent exposure of phosphatidylserine at the erythrocyte surface. As macrophages are equipped with phosphatidylserine receptors, they bind, engulf and degrade phosphatidylserine exposing

Time-dependent efficacy and safety of hypertonic saline versus mannitol in pediatric traumatic brain injury: a systematic review and meta-analysis.

BACKGROUND: Traumatic brain injury (TBI) represents a significant cause of morbidity and mortality in children, with secondary injuries such as elevated intracranial pressure (ICP) significantly impacting outcomes. Hyperosmolar therapy is a cornerstone of acute management, with hypertonic saline (HTS) and mannitol as principal agents; however, comparative evidence regarding their efficacy and safety in pediatric populations remains inconclusive. METHODS: Comprehensive searches of PubMed, Embase, and CENTRAL were performed up to June 2025. Primary outcomes were ICP reduction and all-cause mort

Cough and Airway Responses to Direct and Indirect Challenges: An Integrative Review.

Individuals with classic asthma can present with wheezing, dyspnea, chest tightness, and/or cough. Notably, cough can be the main or only symptom of asthma, termed cough variant asthma. Although there are numerous guidelines for asthma, the diagnosis and management of cough in asthma remain challenging because of its complex physiology. By definition, cough variant asthma excludes individuals with normal airway sensitivity to methacholine (MCh). However, many individuals with chronic cough who are being evaluated for possible cough variant asthma have normal airway sensitivity, yet cough durin

Modified enhanced pleural fixation: combined intra- and postoperative hypertonic glucose in elderly and high-risk pulmonary bullae patients - a retrospective cohort study.

BACKGROUND: Although mechanical pleurectomy effectively prevents recurrent pneumothorax in elderly patients with diffuse emphysematous bullae, it entails substantial surgical trauma and postoperative pain. This study evaluates an enhanced pleurodesis technique-combining intraoperative hypertonic glucose spraying with three-day postoperative intrapleural perfusion-as a minimally invasive alternative. METHODS: A retrospective analysis included 155 patients who underwent thoracoscopic bullectomy: 72 in the glucose protocol group and 83 in the parietal pleurectomy control group. Following 1:1 pro
