Infections that cause vasculitis.

An infectious cause has long been suspected for most forms of vasculitis. In most cases vascular damage has been ascribed to immune-mediated mechanisms rather than direct microbial toxicity. Evidence cited in this review provides further support for a causal role of infection in the vasculitides by strengthening the link between group A streptococci and hepatitis B virus with polyarteritis nodosa and Kawasaki syndrome. It is anticipated that the application of new molecular technology will provide definitive evidence for the role of infection in the etiopathogenesis of the vasculitides.

[Bacterial infections in pregnancy].

The spectrum of bacterial infections during pregnancy is broad. Among blood-transmitted infections especially listeriosis endangers the child. More frequent are ascending and peripartal infections, which imperil both mother and child. The disturbed vaginal flora, which occurs in 10-30% of all pregnant women, has to be regarded as its reservoir. Cervicitis caused by gonococci or chlamydiae, but also high concentrations of facultative pathogenic bacteria are regarded as the main cause of the preterm rupture of membranes leading to preterm delivery. Especially feared is the group-A-streptococci i…

Superficial oral fungal infections.

Fungal infections in humans are provoked and exacerbated by defects in the cellular immune system. Hence, the emergence of novel clinical variants of oral candidoses and rare mycoses with the pandemic spread of human immunodeficiency virus infection is not surprising. The new clinical entities of oral candidoses that have been described in the past few years have had a significant impact on the classification of these diseases. Classification of oral candidoses is an issue addressed in some detail here. Angular cheilitis is a disease commonly associated with Candida infection. In the West, it …

Prophylaxis of postoperative infections.

The antibiotic most appropriate for prophylaxis of postoperative infections depends on the nature of the operation. In aseptic (clean) operations, gram-positive postoperative infections are the primary concern, and cefazolin is recommended because of its excellent pharmacokinetics and good activity against gram-positive pathogens, including staphylococci. In those operations where violation of the digestive tract creates a contaminated field, a cefotaxime-generation cephalosporin is the agent of choice because of the excellent safety profiles and the capability of agents of this class to kill …

Fluoroquinolones in respiratory infections.

The fluoroquinolones have excellent activity against a number of respiratory pathogens, especially gram-negative bacteria, including beta-lactamase-producing Hemophilus influenzae and Moraxella catarrhalis. Several studies have shown ciprofloxacin to be effective in the treatment of acute exacerbations of chronic bronchitis, some community-acquired and nosocomial pneumonia, and acute exacerbations of bronchopulmonary infections in cystic fibrosis. The fluoroquinolones have less activity against Streptococcus pneumoniae and limited anaerobic activity, which should limit the use of these drugs i…

[Infections in organ transplantations].

Since cyclosporin A was introduced clinically, transplantation of solid organ grafts, has become a routine therapy for untreatable endstage- … s reduced and the risk of infection is diminished. --During the first postoperative month bacterial infections commonly appear. Thereafter viral infections can be observed more frequently. Cytomegalovirus infections are very dangerous in CMV-seronegative recipients with a lethality up to 90%. So a CMV-cross-match between the donor and recipient has to be per …

Missing infections in AIDS.

In north America and Europe, the opportunistic infections from which patients with acquired immune deficiency syndrome (AIDS) frequently suffer are Pneumocystis carinii pneumonia and Mycobacterium avium-intracellulare: in central Africa these infections are uncommon or non-existent. Serious infections with Entamoeba histolytica and Strongyloides stercoralis would be expected to occur in AIDS patients: they do not. Falciparum malaria might be expected to interact with HIV infection: it does not. The epidemiology and pathophysiology of these infections are discussed with respect to HIV co-infect…

Upper respiratory tract infections.

Upper respiratory tract infections are among the most common acute infections in humans. This review discusses the clinically important aspects of the epidemiology, etiology, clinical presentation, diagnosis, management, complications, and prevention of the common cold, pharyngitis, otitis media, and sinusitis. Most episodes of the common cold and pharyngitis are of viral origin, and curative therapy is not available. Streptococcal pharyngitis, acute otitis media, and sinusitis are secondary to bacterial infections, and antibiotic therapy is important.

Extrapulmonary Pneumocystis carinii infections.

A case of disseminated infection with Pneumocystis carinii is presented, and the English-language literature is reviewed for cases of documented extrapulmonary infection with this organism. In this case--with P. carinii diffusely replacing the bone marrow and causing hepatic, adrenal, and glomerular tuft necrosis--the clinical illness and multiple-organ dysfunction attributed to disseminated P. carinii were more severe than had previously been described. Because the rate of extrapulmonary P. carinii infection found at autopsy in patients with AIDS is at least 2.5% at our institution, we cautio

Infections associated with pets.

Dogs, cats, rodents, reptiles and birds can transmit numerous diseases. Some are localized infections resulting from bites, scratches or other skin contact. Occasionally, a minor scratch or bite can lead to more serious infection, such as cat-scratch disease. Under certain circumstances, serious gastrointestinal, respiratory or multisystem illness can develop. Accurate diagnosis and identification of the pet source are necessary to prevent further transmission.
