Factors Influencing Recovery From Pediatric Stroke Based on Discussions From a UK-Based Online Stroke Community: Qualitative Thematic Study.

BACKGROUND: The incidence of stroke in children is low, and pediatric stroke rehabilitation services are less developed than adult ones. Survivors of pediatric stroke have a long poststroke life expectancy and therefore have the potential to experience impairments from their stroke for many years. However, there are relatively few studies characterizing these impairments and what factors facilitate or counteract recovery. OBJECTIVE: This study aims to characterize the main barriers to and facilitators of recovery from pediatric stroke. A secondary aim was to explore whether these factors last

Emergency medical services and sudden cardiac arrest: the "chain of survival" concept.

Early access to the EMS system helps insure early CPR, defibrillation, and advanced care. Early access is easiest to achieve with 911 systems and widespread community education and publicity. It may also be taught during citizen-CPR classes. Early CPR helps patients by slowing the process of dying, but its effectiveness disappears within minutes, and defibrillation must soon follow. A citizenry well-informed about cardiac emergencies and well-trained in CPR appears to be the best method to achieve early recognition and early CPR. The earliest possible delivery of defibrillation is critical and

Gaussian process emulation to improve efficiency of computationally intensive multidisease models: a practical tutorial with adaptable R code.

BACKGROUND: The rapidly growing burden of non-communicable diseases (NCDs) among people living with HIV in sub-Saharan Africa (SSA) has expanded the number of multidisease models predicting future care needs and health system priorities. Usefulness of these models depends on their ability to replicate real-life data and be readily understood and applied by public health decision-makers; yet existing simulation models of HIV comorbidities are computationally expensive and require large numbers of parameters and long run times, which hinders their utility in resource-constrained settings. The effectiveness of COVID-19 vaccines to prevent long COVID symptoms: staggered cohort study of data from the UK, Spain, and Estonia.

BACKGROUND: Although vaccines have proved effective to prevent severe COVID-19, their effect on preventing long-term symptoms is not yet fully understood. We aimed to evaluate the overall effect of vaccination to prevent long COVID symptoms and assess comparative effectiveness of the most used vaccines (ChAdOx1 and BNT162b2). METHODS: We conducted a staggered cohort study using primary care records from the UK (Clinical Practice Research Datalink [CPRD] GOLD and AURUM), Catalonia, Spain (Information System for Research in Primary Care [SIDIAP]), and national health insurance claims from Eston

Is It Fair That Patient-Reported Outcome Measures Completion Is Tied to Reimbursement? Patient Demographics Are Associated With Rates of PROM Completion and Potential Health Disparities.

BACKGROUND: Patient-reported outcome measures (PROM) allow clinicians to measure functional outcomes following surgery. The Centers for Medicare & Medicaid Services (CMS) has made reporting of PROMs mandatory for inpatient total joint arthroplasty (TJA) reimbursement. Despite these mandates, there is lack of research identifying patient factors associated with PROM noncompliance. The goal of this study was to identify significant predictors of which patients are less likely to complete PROMs. METHODS: This retrospective study included patients who underwent primary TJA between January 1, 2022

Use of antidepressants before, during, and after pregnancy between 2015 and 2021 in Switzerland: a retrospective analysis of Swiss health care claims data.

OBJECTIVE: Depression and anxiety are common during pregnancy, but the prevalence of antidepressant use during pregnancy in Switzerland is not well documented. We explored antidepressant use before, during, and after pregnancy in Switzerland. METHODS: We conducted a descriptive study using a Swiss health care insurance claims database (Helsana) between 2015 and 2021. We included pregnancies resulting in birth of women who were continuously insured during three 270-day periods: preconceptional, pregnancy, and postpartum. In these periods, we quantified the exposure prevalence to antidepressant

Out-of-pocket healthcare payments on chronic conditions impoverish urban poor in Bangalore, India.

BACKGROUND: The burden of chronic conditions is on the rise in India, necessitating long-term support from healthcare services. Healthcare, in India, is primarily financed through out-of-pocket payments by households. Considering scarce evidence available from India, our study investigates whether and how out-of-pocket payments for outpatient care affect individuals with chronic conditions. METHODS: A large census covering 9299 households was conducted in Bangalore, India. Of these, 3202 households that reported presence of chronic condition were further analysed. Data was collected using a s

The "Iron Triangle" of Healthcare Services: Evaluating an Integrated Healthcare Alliance's Impact on Primary Care in Shenzhen, China.

INTRODUCTION: In response to the challenges of inequitable healthcare access and underdeveloped primary care systems, integrated care reforms have emerged as a promising solution. This study examines the effectiveness of one such reform, i.e. the establishment of a cross-regional, tightly integrated healthcare alliance (specifically an urban healthcare group) in the resource-constrained Dapeng District of Shenzhen, China. We assess its impact on service delivery performance through the lens of three core health system objectives: accessibility, quality, and affordability. METHODS: We utilised

Supplementary orthopaedic screening for children and adolescents to prevent permanent skeletal deformities - protocol for the "OrthoKids" study.

BACKGROUND: Skeletal deformities (SD) in children and adolescents can lead to arthritic conditions, impairment of quality of life, and high treatment costs in the long term. However, comprehensive data on the prevalence of SDs in children and adolescents are limited and it remains therefore unclear whether there is a healthcare gap. "OrthoKids" is a project that addresses this evidence gap by implementing an orthopaedic screening for children and adolescents that supplements existing detection examinations within statutory standard care in Germany. OBJECTIVE: To detect SDs so that they can be

[Common lumbago and returning to work: various thoughts about a complex problem].

Non specific low back pain (NSLBP) is commonly a persistent or recurrent problem. In general the longer a worker is off work with NSLBP, the more disabling the condition becomes, the less successful any form of treatment, and the greater the probability of long term sickness absence. The obstacles to return to work are diverse. Scientific evidence shows that the development of chronic low back pain and disability depends more on individual and work-related psychosocial issues than on physical or clinical features. There is strong empirical evidence that treatment at the subacute stage (NSLBP l

Effect of seated aerobic exercise on physical endurance in long-term care recipients: secondary analysis of a cluster non-randomized controlled trial.

BACKGROUND: Japan's long-term care insurance (LTCI) system provides rehabilitation services. This study examined whether adding seated aerobic exercises to daycare rehabilitation improves endurance and frailty. METHODS: We performed a secondary analysis of a cluster nonrandomized trial conducted at 10 Japanese daycare facilities. Certified older adults requiring care were enrolled in September 2023. Facilities were allocated to the aerobic program or the usual rehabilitation. The aerobic program included seated stepping and arm movements at 120 bpm for two 2-minute sets during rehabilitation

Decreasing barriers to the utilization of cryopreserved sperm in male cancer survivors: an expert review and guide.

BACKGROUND: Sperm cryopreservation offers male cancer patients a critical opportunity to preserve fertility prior to gonadotoxic therapy, yet utilization of banked sperm remains modest, typically under 10%. A structured understanding of barriers across the cancer survivorship continuum is needed to support patients in utilization of cryopreserved sperm in alignment with their reproductive goals. MATERIALS AND METHODS: This expert review synthesizes current evidence and clinical experience to explore the multifaceted barriers to cryopreserved sperm use and propose specific solutions. Association between community-level health literacy and frailty in community-dwelling older adults.

AIMS: We aimed to investigate whether high community-level health literacy, beyond individual-level health literacy, is associated with a low prevalence of frailty among community-dwelling older adults. METHODS: A large cross-sectional questionnaire survey was conducted among citizens in Maizuru City, Kyoto, Japan, aged 65 years or older who were not certified as "support" or "care" level according to Japan's public long-term care insurance system, who could perform basic activities of daily living, and who did not have dementia or Parkinson's disease. Frailty status was assessed using the Ki

Delay in Cleft Lip and Palate Surgical Repair: An Institutional Review on Cleft Health Disparities in an Urban Population.

Access to specialized medical care is critical to decrease complications and minimize long-term morbidity, yet racial disparities in cleft surgery persist as time to initial reconstruction remains delayed among minority patients. Research has demonstrated an average 3-week delay in surgery for minority patients nationally. A retrospective chart review of patient demographics, visit timing, and surgical history was performed for patients who underwent primary cleft lip with or without palate (CL + P) reconstruction between 2002 and 2016 at an urban craniofacial center. Of the 89 children who un

The association between participation in a pay-for-performance program and macrovascular complications in patients with type 2 diabetes in Taiwan: A nationwide population-based cohort study.

OBJECTIVE: Diabetes and diabetes-related complications are major causes of morbidity and mortality worldwide and contribute substantially to health care costs. Proper care can prevent or delay vascular complications in people with type 2 diabetes. We sought to examine whether a diabetes pay-for-performance (P4P) program under Taiwan's National Health Insurance program decreased risk of macrovascular complications in type 2 diabetes patients, and associated risk factors. RESEARCH DESIGN AND METHOD: We conducted a longitudinal observational case and control cohort study using two nationwide pop
