Dendritic cells regulate natural killer cell activation and epithelial injury in experimental biliary atresia.

Biliary atresia is the most common cholangiopathy of childhood. During infancy, an idiopathic activation of the neonatal immune system targets the biliary epithelium, obstructs bile ducts, and disrupts the anatomic continuity between the liver and the intestine. Here, we use a model of virus-induced biliary atresia in newborn mice to trace the initiating pathogenic disease mechanisms to resident plasmacytoid (pDCs) and conventional (cDCs) dendritic cells. We found pDCs to be the most abundant DC population in the livers of newborn mice, and we observed pDCs in the livers of infants at the time

Congenital cardiac malformations in neonates with apparently isolated gastrointestinal malformations.

BACKGROUND: The association of congenital cardiac malformations (CCM) with malformations of the gastrointestinal tract/abdominal wall is kno … phageal fistula/esophageal atresia, in 2/25 (8%) with diaphragmatic hernia and in 5/17 (29.4%) with intestinal atresia. In 11 of 36 patients (30.6%) with CCM, the cardiac problems were hemodynamically significant, requiring anti-congestive and/or surgical treatment. CONCLUSION: A significant number of neonates with apparently isolated gastrointestinal malformations …

The objective of the present work is a review of contemporary therapeutic possibilities of neonates with serious developmental defects. Neon … ele, 93% with gastroschisis, 90% with oesophageal atresia, 97% with anorectal atresia and 100% with intestinal atresia. With declining mortality, in the foreground of interest are problems of early definitive correction of long-term results and quality of life. With regard to the low incidence of different congenital anomalies and the contemporary birth rate in the …

Prenatal diagnosis of persistent cloaca associated with VATER (vertebral defects, anal atresia, tracheo-esophageal fistula, and renal dysplasia).

The cloaca is a single canal from which the urinary, genital, and intestinal tracts arise around gestational weeks 5-6. Persistent cloaca can result from cystic mass formation within the pelvis, which is commonly association with multiple developmental defects. VATER association, which is a spectrum of anomalies, manifested by vertebral defects, anal atresia, tracheo-esophageal fistula with esophageal atresia, and renal dysplasia, arises from abnormalities in mesodermal differentiation. Recently, both conditions have been proposed to represent a continuous spectrum of anomalies, but the pathop

Large congenital transmesenteric hernia: a missed small-bowel atresia?

Internal hernias are uncommon diagnoses and represent rare causes of intestinal obstruction. Diagnoses are frequently made perioperatively. We present herein an illustrated case of transmesenteric hernia diagnosed in a pregnant woman who consulted for severe abdominal and dorsal pains. As CT scan was not possible because of the obstetric history, a decision to perform surgery was made because of acute pain, no history of previous surgery, and a plain erect X-ray disclosing early signs of intestinal obstruction. Perioperative findings were an ileal volvulus through a transmesenteric fossa circl

A further family of Stromme syndrome carrying CENPF mutation.

Stromme syndrome is a rare genetic disorder characterized by microcephaly, anterior ocular chamber anomalies, and "apple peel" type jejunal atresia. Here, we report a Stromme syndrome family with two affected siblings with a homozygous truncating frameshift mutation in CENPF. A 3-month-old girl was hospitalized due to prenatally diagnosed microcephaly, microphthalmia, and dysmorphological features. The history of a previous child with the same findings in addition to "apple peel" intestinal atresia had been noted. Regarding the clinical features of both affected siblings, a diagnosis of Stromm…

Biliary atresia splenic malformation syndrome--is it a result of embryonically midline rotational defects? A case report.

The authors present a case of biliary atresia splenic malformation (BASM) syndrome in a 34-week gestational age boy who died shortly after birth. Autopsy results showed polysplenia, short pancreas, extrahepatic biliary atresia, heterotaxy of liver, tracheoesophageal and duodenal atresia, right lung hypoplasia, cardiac defects with tetralogy of Fallot, axial skeletal defects, and minor genitourinary disorders. Additional histopathologic findings were congenital hepatic fibrosis and bronchopulmonary dysplasia. The authors consider that the abnormalities of this case are possibly a unique combina

[Intestinal malrotation in children].

Intestinal malrotation has received, so far, little attention in the Spanish medical literature. This is probably due to the fact that is variable forms of clinical presentation may be overlooked during infancy and childhood and be present or not later in adult life. This is a report on 33 patients between 1 week and 8 years old operated upon, from 1965 to 1985, at the Clinica Infantil "La Paz", Madrid, with pre or postoperative diagnoses of intestinal malrotation (patients with Bochdalek's hernia, omphalocele and gastrosschisis are excluded). Midgut volvulus has been the most frequent preoper

Use of trans-biliary rendezvous technique for stenting of an impassable duodenal stricture.

Stenting of the gastric outlet is an accepted method for palliation of symptoms secondary to inoperable malignancy and is successful in most cases. Failure of stenting is most commonly due to inability to cross the stricture. We describe a rendezvous technique of duodenal stenting via transhepatic biliary access when conventional endoscopic or fluoroscopic methods fail.

Neonatal abdominal X-rays: indications, procedure and interpretation.

Abdominal X-rays (AXRs) are one of the most common imaging modalities used to investigate suspected abdominal conditions in neonates. They are frequently requested for a variety of suspected conditions including necrotising enterocolitis, congenital diaphragmatic hernia, duodenal atresia and Hirschsprung's disease. Nurses play an important role in preparing and supporting neonates who are undergoing an AXR, and in some centres advanced neonatal nurse practitioners may be involved in requesting and interpreting AXRs as part of their enhanced role in patient management. It is therefore important

[Antenatal diagnosis and management of ileal atresia].

Ileal atresia is a rare congenital malformation which affects a small percentage of the population, with an incidence of 1 in 5000 cases. It may be suspected and diagnosed by ultrasound at the end of the second and third trimester. Obstetrical and surgical consultation is the key to success here. Eliminating a systemic disease with poor prognosis, fighting to reduce premature births and immediately entrusting the child to the surgeon are the main objectives to achieve. During surgery, the surgeon will determine the type of atresia, its location, single or multiple areas of occlusion and its le

[Oral cholangiography and duodenal atresia].

A newborn male patient with trisomy-21 presented with bilious hemesis. The patient was icteric with slight hepatomegaly. Simple abdominal X-ray and upper gastrointestinal series with barium showed a dilated duodenal loop and inflammatory changes involving the duodenal mucosa. This image known as "double bubble" is characteristic of congenital duodenal obstruction. Simultaneously the gallbladder and choledochus were visualized. The former X-ray finding is very unusual. An uneventful Kimura procedure was performed.

Sutured Versus Sutureless Enterostomies: An Alternative Technique in Critically Sick Neonates.

Background and objective Intestinal perforations and necrotizing enterocolitis (NEC) requiring the formation of temporary intestinal stoma are prevalent conditions worldwide. This prospective study aimed to address the following research question: does sutureless enterostomy lead to fewer complications compared to conventional enterostomy in critically sick surgical neonates who need a stoma? Methods We conducted a randomized control trial (TCTR20211011004) from October 2020 to October 2021. A total of 120 patients were randomized to the following two study groups: Group A: conventional suture

Preduodenal portal vein and anomalous continuation of inferior vena cava: CT findings.

Four cases of a rare congenital anomaly, preduodenal portal vein (PPV), are presented. Preduodenal portal vein is known to be frequently associated with other anomalies including intestinal malrotation, situs inversus, biliary atresia, and pancreatic, splenic, and cardiac anomalies. Of our four cases, three had azygos or hemiazygos continuation of the inferior vena cava and other anomalies. We want to call attention to association of azygos or hemiazygos continuation of inferior vena cava with PPV.
