[Late reoperation in patients with carcinoma of the esophagus and cardia].

The authors give an account of 17 late re-operations performed after 14 resections and three palliative operations of carcinoma of the oesop … in two patients, seven times an endoprosthesis was introduced, three times a bypass was made, once jejunostomy and once the operation ended by probatory laparotomy. The mortality of all re-operations was 35.7%, the survival was 6-11 months.

Delayed primary anastomosis in poor-risk patients with esophageal atresia associated with tracheoesophageal fistula.

Twelve patients with esophageal atresia with tracheoesophageal fistula (TEF) who were treated with delayed primary anastomosis were reviewed. All patients except two were classified as Waterston's group C, for whom multiple staged procedures usually can be considered. Gastrostomy for decompression was performed as the initial operation shortly after admission and adequate nutritional support was provided by a transpyloric or jejunostomy tube. Extrapleural division of the TEF and esophagoesophagostomy were delayed until these operations were considered safe to perform. Delayed primary anastomos…

[Current resources of microsurgery for the re placement of the cervical oesophagus and the hypopharynx (author's transl)].

A technique of oesophago-jejunostomy in the neck is described using an isoperistaltic jejunal Y loop with added vascularisation from the internal mammary vessels via microsurgical anastomoses. This technique is used either for solely palliative purposes when the tumour is inoperable, or after oesophagectomy as a method of reconstruction of the digestive tract. An experimental study in the dog of replacement of the cervical oesophagus using a free gastric, jejunal or colonic transplant indicates the possibility of envisaging these techniques in the near future in man after circular total pharyn…

Case Report: a Single-Patient Experience with Trans-Sphincteric Esophagojejunostomy for Concurrent Achalasia and Morbid Obesity.

DISCLAIMER: This case report describes an investigational, non-standard technique performed in a high-expertise center. It should not be con … echnique that addresses both morbid obesity and achalasia using a stapled trans-sphincteric esophagojejunostomy, achieving full-thickness myotomy while preserving the stomach and avoiding the technical limitations of standard dual approaches. SETTING: Academic tertiary care center, United States. METHODS: We present a case of a 54-year-old woman with a BMI …

Robotic-assisted cystojejunostomy and cystocystostomy: a video case report and literature review.

A pancreatic pseudocyst (PC) is a capsulated collection of homogeneous fluid that is a common complication of pancreatitis, resulting from damage to the pancreatic ducts caused by gallstones or excessive alcohol consumption. Symptoms are often non-specific, but symptomatic cysts that do not resolve spontaneously require operative treatment. It consists of percutaneous drainage (PD), ultrasound endoscopy (EUS) drainage and surgical drainage. When surgery is the only choice possible, the most common surgical procedures are cystogastrostomy (CG) and the Roux-en-Y-type cystojejunostomy (RCGJ). Thi…

[Unformed conglomerate debilitating jejunal fistulas and their two-stage surgical treatment via posterolateral laparotomy].

OBJECTIVE: To create a method of two-stage repair of high unformed conglomerate delimited debilitating jejunal fistulas via posterolateral l … ection of jejunal fistula. This exclusion turns the fistula into analogue of the definitive Meidl's jejunostomy, unloads the intestinal anastomosis and increases the quality of suture. New strategy reduced the risk of complications and mortality.

Robotic surgery for congenital biliary dilatation using the scope switch technique (with video).

TECHNIQUE: Minimally invasive congenital biliary dilatation (CBD) surgery is technically demanding. However, few studies have reported surgical approaches of robotic surgery for CBD. This report presents robotic CBD surgery using a scope-switch technique. Our robotic surgery technique for CBD consisted of four steps: step 1, Kocher's maneuver; step 2, dissection of the hepatoduodenal ligament using the scope switch technique; step 3, preparation for the Roux-en-Y loop; and step 4, hepaticojejunostomy. RESULTS: The scope switch technique can provide different surgical approaches for dissecting…

A New Approach to the Complicated Liver Hydatid Cyst--Laparoscopic Roux-en-Y Cystojejunostomy.

Surgery is still the gold standard in the treatment of hepatic hydatid disease. One of the major problems associated with hydatid cyst surge … e liver. Laparoscopic exploration demonstrated a cysto-biliary communication, and laparoscopic cystojejunostomy was performed to prevent long-term bile leakage. The patient was discharged on postoperative day 5 without any problems. In 45th days, nearly complete obliteration of the cavity was observed. Laparoscopic cystojejunostomy can be an effective and saf …

Surgical approaches to treatment of gastroparesis: gastric electrical stimulation, pyloroplasty, total gastrectomy and enteral feeding tubes.

Gastric electrical stimulation (GES) is neurostimulation; its mechanism of action is affecting central control of nausea and vomiting and enhancing vagal function. GES is a powerful antiemetic available for patients with refractory symptoms of nausea and vomiting from gastroparesis of idiopathic and diabetic causes. GES is not indicated as a way of reducing abdominal pain in gastroparetic patients. The need for introducing a jejunal feeding tube means intensive medical therapies are failing, and is an indication for the implantation of the GES system, which should always be accompanied by a py

Post-pyloric feeding.

Postpyloric feeding is an important and promising alternative to parenteral nutrition. The indications for this kind of feeding are increasing and include a variety of clinical conditions, such as gastroparesis, acute pancreatitis, gastric outlet stenosis, hyperemesis (including gravida), recurrent aspiration, tracheoesophageal fistula and stenosis in gastroenterostomy. This review discusses the differences between pre- and postpyloric feeding, indications and contraindications, advantages and disadvantages, and provides an overview of the techniques of placement of various postpyloric devices

[Isolated complete duodenal avulsion at the pylorus after blunt abdominal trauma].

We would like to place on record a case of an isolated complete duodenal avulsion distal to the pylorus after blunt abdominal trauma, in a 78-year-old patient. From all relevant literature, this is the first case known to us of duodenal rupture, around its complete circumference, in close proximity to the pylorus, which did not result in the injury of any further organs, in the absence of chronically infectious alterations. A duodenal injury in the case of blunt abdominal trauma is rare and amongst injuries to hollow organs is of no great importance. In isolated cases of duodenal injuries, it

A technique of laparoscopic gastrectomy and Billroth II gastrojejunostomy.

This report describes a technique of total intraabdominal laparoscopic gastrectomy using laparoscopic stapling and suturing. The reestablishment of continuity of the upper gastrointestinal tract was performed by gastrojejunostomy (Billroth II) using the Endo GIA stapler. This procedure merits inclusion in the armamentarium of approaches toward surgical treatment of recurrent gastric ulcer.

Is intestinal lengthening effective in treating extreme short bowel syndrome?

Our report concerns a child with short bowel syndrome who had 20 cm of small intestine distal to the pylorus (duodenum and jejunum) which remained after subtotal intestinal resection; the resection was necessitated by intrauterine volvulus with intestinal necrosis as a result of gastroschisis. In addition, only 25 cm of the colon remained. Despite continuous enteral nutrition with a semi-elementary diet and conservative therapy, it was not possible to provide within six months at least occasionally half of the caloric intake required on an enteral basis. In such cases treatment is usually disc

Effect of intestinal resection on two juvenile horses with granulomatous enteritis.

Two horses were presented with lethargy, weight loss, anorexia, and swelling of the limbs and ventral body wall. One horse, a 12-month-old American Paso Fino colt, also had acute abdominal pain. The other horse, a seven-month-old Tennessee Walking Horse (TWH) filly passed diarrheic stools during the initial examination. Each horse had low serum protein, neutropenia, and a normal packed cell volume (3.2 g/dl, 1300 cells/ul, and 38%, respectively, for the colt, and 2.4 g/dl, 696 cells/ul, and 44%, respectively for the filly). After intravenously administering plasma, the colt's PCV dropped to 23

Management of gastric emptying disorders following the Roux-en-Y procedure.

In 46 patients with gastric resection and Roux-en-Y gastrojejunostomy, gastric emptying was studied with the gamma camera. Seventeen patients were free of symptoms, 11 vomited occasionally (less than 5 times weekly), and 18 were severely incapacitated with daily vomiting, weight loss, and bezoar formation. Patients with occasional vomiting had early rapid emptying similar to that seen in the patients who were without symptoms and responded satisfactorily to nonsurgical therapy. The 18 patients with severe vomiting showed a marked delay in the emptying of the solid meal (p less than 0.01) but n…

A Rare Finding at the Hepaticojejunostomy in a Postliver Transplant Patient.

A 30-year-old man with a history of liver transplantation (LT) for primary sclerosing cholangitis presented with a clinical picture compatible with ascending cholangitis. The patient was started on antibiotic therapy, and a magnetic resonance imaging with cholangiopancreatography (MRCP) revealed biliary dilation. The patient then underwent double-balloon endoscopic retrograde cholangiopancreatography (DBE-ERCP). Endoscopic findings revealed an ulcerated mass obstructing the hepatico-jejunostomy (HJ), and biopsies were taken. Pathology was compatible with nondestructive post-transplant lymphopr…
