Tenofovir disoproxil fumarate is a nucleotide reverse transcriptase inhibitor with activity against both HIV and the hepatitis B virus. It has had minimal nephrotoxic effects in early clinical trials, but as clinical use has widened, case reports describing tenofovir-induced renal tubular damage, Fanconi's syndrome and diabetes insipidus have been described. The authors review the pharmacokinetics, mechanism of action and clinical uses of tenofovir disoproxil fumarate. The large clinical trials, as well as the case reports of tenofovir-induced kidney injury, are also reviewed. The potential me…

The amyloidoses are a group of disorders in which soluble proteins aggregate and deposit extracellularly in tissues as insoluble fibrils, causing progressive organ dysfunction. The kidney is one of the most frequent sites of amyloid deposition in AL, AA, and several of the hereditary amyloidoses. Amyloid fibril formation begins with the misfolding of an amyloidogenic precursor protein. The misfolded variants self-aggregate in a highly ordered manner, generating protofilaments that interact to form fibrils. The fibrils have a characteristic appearance by electron microscopy and generate birefri…

The most effective treatment of end-stage renal disease is renal transplantation; its superiority to prolong the longevity of patients is well established. Patient and graft survivals have improved with more potent immunosuppression but this advance has been associated with an increased incidence of cancer. The aim of this study was to assess the prevalence of cancer among 265 kidney transplant recipients engrafted between 1968 and October 2004. The overall prevalence of de novo malignancies was 3%. The mean age at diagnosis was 53.3 years (range, 28-63 years) and the duration of the transplan…

Cardiovascular risk is dramatically increased in patients with end-stage renal disease (ESRD). However, even minor dys-functions such as microalbuminuria or a mild increase in serum creatinine (Cr) have a major impact on cardiovascular risk. Increased cardiovascular risk is present in multiple populations, including general populations, patients with moderate risk such as hypertensives, and high-risk patients including patients with heart failure and myocardial necrosis. There are many mechanisms underpinning the increased cardiovascular risk. Regarding atherosclerosis, the kidney can be victi…

Indications, procedures, complications, pharmacokinetics and outcomes of renal transplantation are different in children and in adults. Subjects <18 yrs old, are often included in a unique list as in Italy, benefiting from donors <15 yrs old, and the waiting time is reduced to <12 months in 71% of cases. The risk of thrombosis limits the use of donors <2 yrs and trans-plantation in infants <1 yr. The age at kidney transplantation is <5 yrs in 20-30% of children. In Italy living-related trans-plantation (LRT) is performed in 7% of cases, while in the USA it is more common (57%) and is often pre…

Osteoporosis, renal osteodystrophy and osteoarthropathy derived from hemodialysis associated amyloidosis are three major bone diseases after the kidney transplantation. Osteoporosis that is induced by steroid administration is an inevitable side effect and develops more frequently in the elderly recipients. After the recovery of renal function due to grafted kidney, secondary hyperparathyroidism usually improves and the serum level of PTH-intact downs to the almost normal range. However, the patients who have tertiary hyperparathyroidism are unable to show the enough lowering of PTH-intact. In…

Renal cancer is rare in pregnant women. Through a study of two cases, the authors underline diagnostic, therapeutic and pathogenic aspects of this association. Two female patients, 26 and 34 years old, were admitted for renal tumor, discovered respectively at the first and the second trimester of pregnancy. Investigation's exams for tumor extension were negative. They were based on ultrasound exam (2 cases), CT scan (1 case) and chest X ray (2 cases). The histological examination concluded to a bright cell carcinoma. In the former case, evolution was marked by spontaneous abortion at the fifth

DE NOVO DIABETES AND CARDIOVASCULAR RISK: Certain kidney transplant recipients who develop de novo diabetes have an unfavorable cardiovascular risk profile, comparable to patients with type 2 diabetes mellitus, with advanced age, dyslipidemia, obesity and high blood pressure. MYOCARDIAL INFARCTION IN THE PERIOPERATIVE PERIOD: Among kidney transplant recipients, those whose risk factors include male gender diabetes, age over 50 years and prior revascularization procedure for coronary artery disease have a higher risk for myocardial infarction in the perioperative period. The usefulness of antic…

We report a case of posttransplant malaria in which the patient developed progressive thrombocytopenia after receiving living related donor kidney transplantation. The donor, who flew in from Sri Lanka for the procedure, had suffered from malaria 18 months earlier. Malaria should be suspected in transplant patients receiving organs from donors originating from countries with a high prevalence of malaria.

OBJECT: Evaluate the incidence of ureterovesical stenosis in the renal transplant and its outcomes in the evolution of the allograft.
MATERIAL AND METHOD: Seventy three renal transplants were made between August 1988 and December 1995 in Italian Hospital in Buenos Aires. The mean follow up period was 35 months. The incidence of ureterovesical stenosis and its outcomes in the renal allograft were evaluated. Seven cases of ureterovesical stenosis were found. Clinic diagnosis was made in all cases (decreased filtration, diuresis rythm diminished, pain over the implant) confirmed with ecography t

In recent years major research findings have revealed a strong correlation between the genes involved in the pathogenesis of renal tumours and the histopathological and clinical behavioural features. This new genetic information has provided the basis for the recent Heidelberg and Mayo Clinical Classifications for renal tumours. WilmsO tumour has been shown to arise from abnormalities in one of at least three genes. The first WilmsO tumour gene identified WT1, located on chromosome 11p13, encodes a zinc finger binding protein which is important in regulating the formation of the early nephron.

OBJECTIVE: Multilocular cyst of kidney is a rare, unique and characteristic renal lesion with benign biologic behavior. We report a case of the disease.
METHODS: It was confirmed by operation and pathology.
RESULTS: It is difficult to distinguish radiologically multilocular cyst of kidney from cystic renal carcinoma and wilms tumor.
CONCLUSION: Surgical intervention is the only effective treatment and method to differentiate multilocular cyst of kidney from a malignant lesion of the kidney.

OBJECTIVES: The importance of establishing a protocol for the urological evaluation of patients awaiting renal transplantation is due to the fact that 30% of these adult patients and up to 70% of these children develop renal failure due to a urological condition. The present study analyzes the importance of urodynamic evaluation in these patients.
METHODS: 650 patients awaiting renal transplantation were evaluated by patient history, physical examination, US, Cystouretyhrography, and flowmetry. Thereafter 201 patients with the following conditions were selected for urodynamic assessment: vesi

BACKGROUND: A very important issue in living kidney donor transplantation is whether the donation is safe for the donor. The aim of this study was to examine survival and causes of death in kidney donors and to assess the renal function in those who had donated a kidney more than 20 years ago.
METHODS: A total of 459 living donor nephrectomies were performed in Stockholm from 1964 until the end of 1994. By using national registers, all 430 donors living in Sweden were traced. Donor survival was analysed using the Kaplan-Meier method. Expected survival was computed using the Hakulinens method …
