Update on ovarian hyperstimulation syndrome: Part 1--Incidence and pathogenesis.

Ovarian hyperstimulation syndrome (OHSS) is a rare and potentially life-threatening complication during controlled ovarian stimulation. It can be associated with severe morbidity and may even be fatal. The etiology of the condition and predisposing factors are still not fully understood. Data concerning pathophysiology in patients with OHSS were searched using PubMed and other medical data bases. The incidence of severe OHSS, as calculated by World Health Organization (WHO), is 0.2-1% of all stimulation cycles in assisted reproduction. Considerations on OHSS classifications and forms of manife

Novel therapies blocking the renin-angiotensin-aldosterone system in the management of hypertension and related disorders.

Although significant advances have been made in the therapeutic blockade of the renin-angiotensin-aldosterone system (RAAS) using angiotensin-converting enzyme (ACE) inhibitors, angiotensin receptor blockers and non-selective aldosterone receptor antagonists, there is a clear need for both additional blocking strategies and enhancements of current therapeutic approaches. Vasopeptidase inhibition may still find a role despite the small incremental value of this approach and the obvious issue of kinin-mediated adverse effects still to be fully addressed. Blockade of the RAAS upstream using renin

[Genes in rennin-angiotensin system].

A large number of association studies revealed the importance of gene polymorphisms in renin-angiotensin system(RAS) in the pathogenesis of hypertension. Among these many polymorphisms of RAS genes, M235T or G-6A polymorphism of angiotensinogen gene increases the risk for salt sensitivity, lacunar infarction, non-dipper type of blood pressure variation, etc. and plasma angiotensinogen level. On the other hand, homozygous deletion polymorphism of angiotensin converting enzyme gene(ACE DD) increases the risk for male hypertension, ischemic heart disease and renal disease via increase of plasma a

Ovarian hyperstimulation syndrome-model rats; the manifestation and clinical implication.

Ovarian hyperstimulation syndrome (OHSS) is the most serious complication of ovulation induction with the gonadotropins. Though vascular endothelial growth factor (VEGF) has been implicated as a prime causative factor of OHSS progression, other factors must also be involved in the pathogenesis of OHSS. We have established an experimental model of OHSS in immature female rats. The ovarian weights and vascular permeability of the OHSS model rats are significantly increased by the ovarian stimulation with human chorionic gonadotropin (hCG). hCG elicits VEGF production in OHSS model rat ovaries. A potential mechanism of propofol-induced pain on injection based on studies using nafamostat mesilate.

To elucidate the mechanism of propofol-induced pain on injection, we performed several studies using nafamostat mesilate, a kallikrein inhibitor, or lidocaine. As both pretreatment and low-dose mixing with nafamostat produced the same effects on pain reduction, we used the latter method in the following experiments. Low-dose mixing had the same effect on injection pain as mixing with lidocaine. The extent of pain was assessed by measuring bradykinin concentrations by mixing with blood. Propofol and its lipid solvent mixed with blood produced approximately two-fold generation of bradykinin comp

[Kidney involvement in liver diseases. Pathophysiology and clinical course].

Renal disorders complicating liver disease are a frequent finding. Extrahepatic causes like intoxications and circulatory dysfunction or diseases that simultaneously affect both the liver and the kidney, like multisystem or viral diseases (hepatitis B) have to be differentiated from clinical entities in which, like in liver cirrhosis or in fulminant hepatitis, the manifestation of renal disease has to be understood as a consequence of the hepatic disorders. Functional disturbances like the increases in tubular sodium reabsorption or the hepatorenal syndrome have been thoroughly investigate bec

Humoral factors in the etiopathogenesis of arterial hypertension.

On the basis of a critical analysis of the literature and of his own results, the author submits a survey of the role of humoral factors in the etiopathogenesis of the various types of arterial hypertension. The role of pressor and depressor factors in the pathogenesis of hypertension must be comprehended in close association with the other, non-humoral mechanism of blood pressure regulation. Hypertension can develop as a result of hyperactivity of one or more factors of pressor systems, including the sympathoadrenal system, the renin-angiotensin system and mineralocorticoids, or of reduced ac

Effects of variations in sodium intake on the acute vasodepressor response to kininase II inhibition in rats with mild two-kidney, one-clip hypertension.

1. Unilateral renal artery constriction in rats maintained on a sodium-deplete, but not sodium-replete, diet induced an augmented acute vasodepressor response to kininase II inhibition produced by an intravenous injection of the dipeptidyl carboxypeptidase inhibitor captopril (250 microgram) during continuous saralasin-induced angiotensin II blockade (10 microgram/min). Dietary sodium restriction alone in sham-operated rats had no effect. 2. Acute bilateral adrenalectomy (18-24 h) did not preclude the demonstration of an augmented response to kininase II inhibition in sodium-depleted rats with

The response of the newborn rat to injury. I. Vascular and humoral aspects.

The development of oedema in response to irritants injected into the skin, or pleural cavity of rats, has been shown to differ according to the age of rat studied. It was found that newborn rats were less responsive, with respect to oedema formation, at certain times after the injection of a particular irritant. Attempts to suppress the reactions of these animals using anti-histamines, anti-serotonin and kinin-depleting agents were less successful than in adults. Analysis of serum, plasma and exudate proteins revealed quantitative and qualitative differences between newborn and adult rats. The

The comparison of efficacy of different imaging techniques (conventional radiography, ultrasonography, magnetic resonance) in assessment of wrist joints and metacarpophalangeal joints in patients with psoriatic arthritis.

BACKGROUND: Psoriatic arthritis (PsA) is a chronic inflammatory joint disease which develops in patients with psoriasis. The rheumatoid factor is characteristically absent in the serum of PsA patients. Etiology of the disease is still unclear but a number of genetic associations have been identified. Inheritance of the disease is multilevel and the role of environmental factors is emphasized. Immunology of PsA is also quite complex. Inflammation is caused by immunological reactions leading to a release of kinins. Destructive changes in bones usually appear after a few months from the onset of …

Pharmacology of the neurogenic oedema response to electrical stimulation of the saphenous nerve in the rat.

The pharmacology of the early and delayed phases of the neurogenic oedema responses to electrical stimulation of the saphenous nerve was studied in anesthetized rats using a quantitative Evans blue dye leakage technique. The immediate response to 5 min nerve stimulation was not reduced by aprotinin or mepyramine in combination with methysergide. However the response measured 10 min later and also that to 15 min nerve stimulation were reduced by these agents indicating that kinins and mast cell amines might be released after some delay, but they did not contribute significantly to the early pha…

Hypotensive peptides from milk proteins.

Hypertension is the major controllable risk factor associated with cardiovascular disease (CVD) events such as myocardial infarction, stroke … of ACE inhibitory peptides. Several studies in spontaneously hypertensive rats show that these casokinins and lactokinins can significantly reduce blood pressure. Furthermore, a limited number of human studies have associated milk protein-derived peptides with statistically significant hypotensive effects (i.e., lower systolic and diastolic pressures). …

Some characterization of the responses to substance P and other tachykinins in rabbit iris sphincter muscle.

Contractile responses to substance P, physalaemin and eledoisin, three members of the tachykinin family, were compared and characterized in rabbit iris sphincter smooth muscle. Eledoisin and physalaemin were approximately 5 times more potent than substance P, and the maximum responses to substance P and physalaemin were about 85 percent of those to eledoisin and carbachol. The contractile responses to the three tachykinins were not affected by tetrodotoxin (3 X 10(-6) M) and atropine (10(-6) M). The contractions induced by substance P and physalaemin were well sustained even after they were th…

The action of physalaemin on electrolyte excretion by the mandibular and sublingual salivary glands of the rat.

The effect of physalaemin, an undecapeptide belonging to a family known collectively as the tachykinins, on water and electrolyte excretion of the mandibular and sublingual salivary glands of the rat has been investigated and compared to that of acetylcholine. Drugs were administered intravenously or by close-arterial infusion. Physalaemin is a powerful stimulant of fluid secretion by both glands although less potent than acetylcholine. The Na and K excretion patterns in physalaemin-evoked saliva resembled, but were by no means identical to those evoked by acetylcholine and other parasympathom…

The necessity of enzymatically hydrolyzing walnut protein to exert antihypertensive activity based on in vitro simulated digestion and in vivo verification.

Since not all proteins are suitable for preparing bioactive peptides by enzymatic degradation, the purpose of this study is to evaluate the necessity of walnut protein (WP) enzymolysis to exert its potential antihypertensive activity. Five proteases were used to hydrolyze WP to produce WP hydrolysate (WPH) enzymatically. The angiotensin-I-converting enzyme (ACE) inhibitory activity of WP and WPH before and after simulated digestion in vitro was measured, and the antihypertensive effect was evaluated in vivo. The results showed that after simulated digestion in vitro, the ACE inhibitory activit

SARS-CoV-2 and ACE2: The biology and clinical data settling the ARB and ACEI controversy.

BACKGROUND: SARS-CoV-2 enters cells by binding of its spike protein to angiotensin-converting enzyme 2 (ACE2). Angiotensin-converting enzyme inhibitors (ACEIs) or angiotensin II receptor blockers (ARBs) have been reported to increase ACE2 expression in animal models, and worse outcomes are reported in patients with co-morbidities commonly treated with these agents, leading to controversy during the COVID-19 pandemic over whether these drugs might be helpful or harmful. METHODS: Animal, in vitro and clinical data relevant to the biology of the renin-angiotensin system (RAS), its interaction wi
