A 59-year-old woman with a painful right knee that became locked in extension after a trivial trauma was seen at the emergency room. This was caused by unloaded hyperextension in bed. She was diagnosed with a superior dislocation of the patella. A closed reduction was performed, but a recurrent episode was seen within a week. An arthroscopy was performed, in which the causative osteophytes were removed. In the 12-month follow-up after treatment, no recurrence was seen. A superior dislocation of the patella is caused by patellofemoral osteophytes that interlock. This can cause a degenerative kn

PURPOSE: The purpose of this study was to evaluate the effect of surgical reinsertion of the medial patellofemoral ligament (MPFL) to the adductor tubercle compared with conservative treatment in patients with primary dislocation of the patella.
METHODS: Eighty patients with primary patella dislocation were included in the study. Delayed arthroscopy (mean, 50 days after injury) was performed to assess cartilage injury and status. During arthroscopy, patients were randomized to surgical reinsertion of the MPFL or to conservative treatment. Conservative treatment was use of a brace with 0 degre

BACKGROUND: Recurrent dislocation of the patella in patients with below knee amputation is a known entity. Abnormally high-riding patella (patella alta) and medial patellofemoral ligament insufficiency in these patients predisposes them to patellar instability. The established treatment of this problem is surgical realignment.
CASE PRESENTATION: A 25 year old male patient with bilateral below knee amputation presented with bilateral recurrent dislocation of the patella while walking on knees on uneven ground. Clinical and radiographic studies showed patella alta. A simple shoe modification wa

STUDY OBJECTIVE: We present our experience with a novel technique for the reduction of acute hip dislocation in the emergency department (ED).
METHODS: We searched the medical records of all patients with a hip dislocation treated in our ED during a 4-year period. We recorded patient demographics, reduction technique, outcome and disposition, and whether the patient had a prosthetic hip. We reported characteristics of the entire study group and of the subset of patients for whom the Captain Morgan technique was used. Briefly, the technique involves placing the physician's knee behind the supi

A knee dislocation combined with a patellofemoral dislocation is an extremely rare and complex injury that reflects severe multidirectional instability and extensive soft-tissue disruption. We show an open single-stage anatomic reconstruction method for a patient with type IV tibiofemoral dislocation, patellofemoral dislocation, and bucket-handle medial meniscus tear. A midline parapatellar approach provided direct access to both the medial and lateral compartments, allowing for the identification of all ligament attachments and the repair of the meniscal tear. Cruciate, collateral, posterolat…

AIM: To study clinical outcomes of the "4-in-1" surgical technique in pediatric cases of atraumatic habitual patellar dislocation.
MATERIAL AND METHODS: A prospective case series including seven patients with atraumatic habitual patellar dislocation were studied at a tertiary care center. After formal Orthopaedic examination and imaging, all patients underwent a "4-in-1" surgical procedure. Postoperatively, an AK POP slab with the knee in 30° of flexion was applied until suture removal at two weeks. Rehabilitation was started after the slab removal, and swimming was encouraged after eight wee

BACKGROUND: The aim of this study was to analyse the biomechanical role of medial retinaculum, as a stabilising factor against lateral patellar dislocation.
MATERIALS AND METHODS: This cadaveric-biomechanical study included the patellae of 10 cadaveric knees, which were surgically exposed and the medial retinaculum of each one was located. A stable 24.51 N force was applied to the four parts of the quadriceps, and an increasing lateral displacing force was applied to the patella, up to 5 mm dislocation. The study was repeated for 0o, 45o, and 90o of knee flexion, with the medial retinaculum i

Chondral fractures of the patella are associated with acute dislocation of the patella. Osteochondral fracture in patellar dislocation is located in the medial facet of the patella. This article presents a case of a 15-year-old female ballerina with isolated displaced osteochondral fracture of the patella without patellar dislocation. She had no history of trauma. A Merchant's view of both knees showed mild subluxation of the patella, a small fragment on the lateral aspect of the knee, and a small defect of the centromedial patella. Axial magnetic resonance imaging (MRI) revealed an osteochond

This study evaluated a technique using the anteroposterior axis of the distal femur, rather than the transepicondylar or posterior femoral condylar axis, to establish rotational alignment of the femoral component in valgus knees. The anteroposterior axis of the distal femur was defined by a line through the deepest part of the patellar groove anteriorly and the center of the intercondylar notch posteriorly. Total knee arthroplasty was done in 46 valgus knees between 1980 and 1986 using the posterior femoral condyles as landmarks for rotational alignment. From January 1986 through January 1992

The original Journey BCS total knee arthroplasty (Smith and Nephew, Watford, United Kingdom) is a bicruciate-stabilized (BCS) total knee arthroplasty. Dislocations and anterior knee pain led to the introduction of the Journey II system in 2013. We reviewed the 10-year survival in our institution. All Journey BCS total knee replacements were included. The study period was from November 2010 to July 2013. Microsoft Excel was used. The primary outcome was 10-year survival using life tables; end points were re-operation for any reason; revision excluding dislocation-related issues; revision exclud

Injury to the proximal tibiofibular joint is typically seen in athletes whose sports require violent twisting motions of the flexed knee. Instability of this joint may be in the anterolateral, posteromedial, or superior directions. With acute injury, patients usually complain of pain and a prominence in the lateral aspect of the knee. A closed reduction should be attempted in patients with acute dislocation. If this is unsuccessful, open reduction and stabilization of the joint with repair of the injured capsule and ligaments can be done. Patients with chronic dislocation or subluxation report

Chronic patellar dislocation is a rare condition where the patella remains dislocated throughout knee range of motion during flexion and extension. In adults, the delayed presentation of this condition is often due to symptoms caused by the onset of severe secondary osteoarthritis. To the authors' knowledge, all of the cases reported in the literature have been treated by patellofemoral or total knee replacements depending on patient age and the extent of the arthritis. This article describes a rare case of a 22-year-old woman who sustained a traumatic chronic patellar dislocation for 5 months

BACKGROUND: Nonweightbearing preoperative assessments avoid quadriceps contraction that tends to affect patellar motion and appear to be inaccurate in quantifying anatomic factors, which can lead to incorrect corrections and postoperative complications.
QUESTIONS/PURPOSES: (1) Does the relationship of patellar axial malalignment and other anatomic factors change during weightbearing? (2) What anatomic factor was most strongly correlated with recurrent patellar dislocation during weightbearing?
METHODS: This prospective, comparative, observational study recruited participants at our instituti

Posterior fracture dislocation of the femoral head is a rare entity usually requiring open reduction and internal fixation. Results of different fixation methods have been reported, including countersinking screws, headless screws, and bioabsorbable implants. Osteochondral autologous transfer (mosaicplasty) is an established method of treatment of full thickness cartilage defects of the knee, ankle, and elbow. At our institution, posterior fracture-dislocations of the femoral head were treated with femoral head fragment fixation using osteochondral autografts through surgical hip dislocation.

Introduction: This case report presents the multifactorial surgical management and long-term outcome of a permanent patellar dislocation in a 16-year-old female patient. Case presentation: A 16-year-old female with permanent patellar dislocation of the left knee complained of progressive pain and functional limitations. Physical examination revealed a 20-degree passive flexion contracture, significant muscle weakness in knee extension, and a permanently laterally dislocated patella that could not be manually repositioned. Imaging studies revealed a complex knee deformity characterised by femor

Seventeen patients (18 knees) with recurrent patellar dislocation were identified with increased quadriceps angles secondary to excessive isolated external tibial torsion. Traditional realignment procedures attempted in these knees were unsuccessful because of failure to align the biomechanical axis of the extensor mechanism. Derotational osteotomies of the tibia just proximal to the patella tendon insertion were used to reduce the quadriceps angle to within normal limits to improve the biomechanics of the extensor mechanism. Seventeen (94%) knees were available for clinical and subjective fol

Medial dislocation of patella has been commonly reported as a complication of the surgical release of the lateral patella-femoral ligament. A traumatic medial eversion-dislocation of the patella has not been earlier reported. We report an unusual case of traumatic medial patellar eversion-dislocation in a young adult patient whose magnetic resonance imaging (MRI) showed a discontinuity in medial and lateral patellar retinacula. However, intraoperatively, the retinacula were found intact, and relocation of the patella was achieved by flipping the patella through a small hole in the patellar ten

Experience with thirty-eight Asian children and adolescents who presented with either stiffness of the knee, genu recurvatum, habitual dislocation of the patella or congenital lateral dislocation of the patella showed that all those disorders were manifestations of contracture of the extensor mechanism, which fell into two groups according to the components involved. In Group I the main components affected were in the midline of the limb, namely rectus femoris and vastus intermedius; these patients presented with varying degrees of stiffness of the knee, or worse, with genu recurvatum. In Grou

BACKGROUND: Traumatic proximal tibiofibular fracture and dislocation (PTFD) have been rarely studied and are easily missed in clinical practice. PTFD is considered a marker of severely traumatized knees. The purpose of this study was to retrospectively analyze the incidence and impact of PTFD in traumatized knees with vascular injury.
METHODS: Patients with knee trauma and vascular injury were included from January 2022 to October 2023. X-rays and CT scans of included patients were retrospectively analyzed to determine the presence of PTFD. Patients were further divided into PTFD group and no
