Editorial Commentary: External Fixation and/or Vascular Injury Is Associated With Postoperative Arthrofibrosis After Multiple-Ligament Knee Injury Reconstruction.

Patients with a body mass index over 30 do not have a significant increase in postoperative arthrofibrosis after multiple-ligament knee injury (MLKI) reconstruction compared with patients with a body mass index under 30. However, although this may be associated with the severity of injury, recent research has shown that patients who undergo external fixation at index surgery and/or who have vascular injury are at increased risk of requiring manipulation under anesthesia. This finding is clinically significant in that it is reassuring that stiffness requiring manipulation is no more likely to d

Combined common peroneal and tibial nerve injury after knee dislocation: one injury or two? An MRI-clinical correlation.

OBJECT Knee dislocations are often accompanied by stretch injuries to the common peroneal nerve (CPN). A small subset of these injuries also affect the tibial nerve. The mechanism of this combined pattern could be a single longitudinal stretch injury of the CPN extending to the sciatic bifurcation (and tibial division) or separate injuries of both the CPN and tibial nerve, either at the level of the tibiofemoral joint or distally at the soleal sling and fibular neck. The authors reviewed cases involving patients with knee dislocations with CPN and tibial nerve injuries to determine the localiz

Effectiveness of low-level laser therapy in the rehabilitation of knee meniscus injuries: a randomized, double-blind, placebo-controlled trial.

To evaluate the effectiveness of low-level laser therapy (LLLT) as an adjunct to rehabilitation in reducing pain and improving knee function in adults with MRI-confirmed meniscus injuries. In this randomized, double-blind, placebo-controlled trial, 40 adults with unilateral meniscus injuries were assigned to either an LLLT group or a placebo group. All participants received a standardized rehabilitation program over six weeks. The primary outcome was pain intensity measured using a visual analogue scale (VAS). Secondary outcomes included knee function (Lysholm score), range of motion (ROM), an

Three-dimensional-printed patient-specific instrumentation is an accurate tool to reproduce femoral bone tunnels in multiple-ligament knee injuries.

PURPOSE: Multiple-ligament knee reconstruction techniques often involve the creation of several bone tunnels for various reconstruction grafts. A critical step in this procedure is to avoid short tunnels or convergences among them. Currently, no specific template guide to reproduce these angulations has been reported in the literature, and the success of the technique still depends on the experience of the surgeon. The aim of this study is to analyze the accuracy and reliability of 3D-printed patient-specific instrumentation (PSI) for lateral and medial anatomical knee reconstructions. METHOD

The relationship between the height of the medial longitudinal arch (MLA) and the ankle and knee injuries in professional runners.

BACKGROUND: Usually the rate of lower extremity's injuries in sports such as running is known to be correlated with the height of foot arches. Foot pressure measurement studies have shown controversial results in this issue, mainly due to the complexity of the foot structure. This study aimed to investigate if any relationship exists between the MLA height and the ankle/knee injuries in professional runners. It was also aimed to find out any association between the foot pressure patterns and the clinical navicular drop test in the subjects. MATERIALS AND METHODS: Forty-seven professional runn…

Knee injuries in motor vehicle collisions: a study of the National Accident Sampling System database for the years 1979-1995.

A detailed study of knee injuries recorded in the 1979-1995 National Accident Sampling System database maintained by the National Highway Traffic Safety Administration was conducted. Injuries to other body regions were also considered in order to illustrate the relative frequency of knee injuries. This study demonstrated that knee injuries constitute approximately 10% of all injuries recorded every year. However, the majority of these injuries were of low severity (i.e. contusions, abrasions, lacerations) with an abbreviated injury score (AIS) of 1. Most knee injuries occurred following a fron…

How iliotibial tract injuries of the knee combine with acute anterior cruciate ligament tears to influence abnormal anterior tibial displacement.

A knowledge of the patterns of injury to the components of the iliotibial tract allows a clearer interpretation of motion limits testing in patients with abnormal anterior tibial translation of the knee (anterior cruciate ligament-deficient knees). Eighty-two consecutive patients with acute knee injuries were classified as anteromedial-anterolateral rotatory instability (anterior cruciate ligament-deficient) based on the abnormal motion demonstrated by clinical examination tests for instability. At surgery, injuries to the intraarticular and extraarticular anatomic structures were identified a…

[Osteosynthesis with a rod in crushing injuries of the knee joint and periarthric fractures of the femur and crural bones].

The authors describe the treatment of 14 patients having crushing injuries of the knee joint of periarthric fragmentation fractures of femoral and crural bones with a wide zone of soft tissues damage. Economic resection of the joint with subsequent formation of a juncture between the femoral and tibial bones with a long rod (65--75 cm) seems to be mostly rational.

Current concepts in isolated and combined injuries of the medial collateral ligament complex, part II: The anteromedial knee instability classification.

The medial collateral ligament (MCL) has a strong self-healing capacity; nevertheless, not every MCL injury heals adequately. Persisting medial and anteromedial knee laxity should be avoided as it may lead to increased strains and wear of the surrounding tissue, including the anterior cruciate ligament (ACL), meniscus and cartilage. Better understanding of MCL anatomy and biomechanics, especially in combined ACL-MCL injuries, has necessitated the need for updated clinical examination, classification and treatment approaches. The newly introduced anteromedial knee instability (AMKI) classificat

A dynamic knee function scoring system for anterior cruciate ligament injuries based on normative six-degree-of-freedom gait pattern.

BACKGROUND: Traditional examinations of anterior cruciate ligament (ACL) injuries focus primarily on static assessments and lack the ability to evaluate dynamic knee stability. Hence, a dynamic scoring system for knee function is needed in clinical settings. This study aimed to propose a dynamic scoring system based on a large sample of normative six-degree-of-freedom (6-DOF) knee kinematics during gait, and validate its correlation with conventional outcome measurements in assessing ACL-injured knees. METHODS: A total of 500 healthy Chinese participants were enrolled to establish a large dat
