A meme's eye view of speech-language pathology.

In this article, the reason why certain terms, labels, and ideas prevail, whereas others fail to gain acceptance, will be considered. Borrowing the concept of "meme" from the study of evolution of ideas, it will be clear why language-based and phonological disorders have less widespread appeal than, for example, auditory processing and sensory integration disorders. Discussion will also center on why most speech-language pathologists refer to themselves as speech therapists or speech pathologists, and why it is more desirable to have dyslexia than to have a reading disability. In a meme's eye …

Language fMRI abnormalities associated with FOXP2 gene mutation.

Half the members of the KE family suffer from a speech and language disorder caused by a mutation in the FOXP2 gene. We examined functional brain abnormalities associated with this mutation using two fMRI language experiments, one involving covert (silent) verb generation and the other overt (spoken) verb generation and word repetition. The unaffected family members showed a typical left-dominant distribution of activation involving Broca's area in the generation tasks and a more bilateral distribution in the repetition task, whereas the affected members showed a more posterior and more extens…

Sign language tics in a prelingually deaf man.

Gilles de la Tourette syndrome is characterized by vocal and motor tics starting in childhood. Vocal tics may be either noises or words, and the vocal language tics may consist of obscenities (coprolalia) and repetitions of speech that has been heard (echolalia). We describe a prelingually deaf man who has the full array of tics seen in Gilles de la Tourette syndrome, but in whom vocal language tics are replaced by equivalent sign language tics. This is, to our knowledge, the first report of sign language tics in a person with prelingual deafness. The implications of this phenomenon for the se…

Speech and language therapy for aphasia following stroke.

BACKGROUND: Aphasia describes language impairment associated with a brain lesion. OBJECTIVES: The objective of this review was to assess the effects of formal speech and language therapy and non-professional types of support from untrained providers for people with aphasia after stroke. SEARCH STRATEGY: We searched the Cochrane Stroke Group Trials Register (last searched: March 1999), and reference lists of relevant articles to December 1998. We also contacted academic institutions and other researchers to identify further published and unpublished trials. We searched The International Journ…

Cerebral lateralization: relationship of language and ideomotor praxis.

OBJECTIVE: To determine the relationship of language lateralization and hand preference to praxis performance following left and right hemispheric amobarbital-induced inactivations. BACKGROUND: Patients who are aphasic from left cerebral dysfunction also frequently exhibit ideomotor apraxia in which they make temporal, spatial, and postural errors of learned skilled movements. However, hemispheric lateralization of the systems mediating ideomotor praxis in patients with atypical cerebral language dominance (i.e., bilateral or right hemispheric language function) remains uncertain. METHODS: S…

Trying to Make Sense of Developmental Language Disorders.

In this article, I share my thoughts concerning what children with developmental language disorders should be called, how they should be defined, and how we might differentiate children with specific language impairment (SLI) from other children with developmental language disorders. Among other things, I attempt to show why a lack of congruence between clinical and research constructs should be expected. Researchers and clinicians use different identification criterion and procedures because clinical and educational objectives are different from research objectives. While recognizing these di…

Language, speech, and educational outcomes of otitis media.

OBJECTIVE: This article, through a summary of the current literature available, studies the relationship between recurrent otitis media (OM) and communication (speech and language) and educational development. Areas discussed include historical and current views of their relationship, hearing loss and auditory abilities, language and speech development, and educational (cognitive and academic) performance of children with histories of OM. As well, the authors present a model for the use of physicians, communications specialists, and educators when dealing with such children. CONCLUSION: Despi…

Language and Williams syndrome: how intact is "intact"?

It has been claimed that Williams syndrome (WS), a rare neurodevelopmental disorder, is characterized by serious cognitive deficits alongside intact language. The syndrome is often used as a prime example of the modularity of an innate faculty for morphosyntactic rules. We challenge this claim and hypothesize that morphosyntax, although surprisingly good given WS level of mental retardation, is by no means intact. We make an initial test of this hypothesis through an analysis of the receptive language of a group of English-speaking WS individuals on a standardized morphosyntactic test. We then…

[Language rehabilitative training for postoperative aphasia in neurosurgery].

Rehabilitation nursing focus on assisting patients to reconstruct their health. We trained 21 cases of aphasia after operation with different types of early comprehensive speech reconstruction exercises, including hearing, speaking, reading and writing. We achieved expected results. Anomia aphasia has the best outcome, next is motor and sensory aphasia, and combined aphasia is less effective. In addition, patients with aphasia before surgery got slightly better rehabilitation than those whose aphasia was resulted from surgery.

Brain morphology in children with specific language impairment.

The planum temporale and pars triangularis have been found to be larger in the left hemisphere than the right in individuals with normal language skills. Brain morphology studies of individuals with developmental language disorders report reversed asymmetry or symmetry of the planum, although the bulk of this research has been completed on adults with dyslexia. Pars triangularis has not been studied in the developmental language impaired population. In this study, magnetic resonance imaging (MRI) was used for quantitative comparisons of the planum temporale (Wernicke's area) and pars triangula…

Role of intelligence tests in speech/language referrals.

This study examined the relation of the WISC-R Verbal IQ with measures of oral and written language among 190 students referred to a private educational clinic over a 5-yr. period. Correlations of Verbal IQ with scores on measures of oral language, written language, receptive language, reading comprehension, and basic reading skills were calculated for the total sample and by Grades 1-3, 4-7, and 8-11. Standard regression coefficients were used to estimate the proportion of variance explained by these five measures. Significant correlations were found for Verbal IQ with the measures, ranging f…

A new classification of developmental language disorders (DLD).

Eighty children with DLD were examined with 18 language tests, mainly derived from a neuropsychological investigation called NEPSY (NEuroPSYchological Investigation for Children). The children were 6-0 to 7-9 years old and attended kindergarten. The test profiles of the first 40 children, Group 1, were utilized for the elaboration of a classification of DLD. The test profiles were grouped into five subgroups with the aid of a Q-type factor analysis. Then the classification was modified to suit clinical application by collapsing two pairs of subgroups. The resulting categories were called: the …

Profiles of language impairment in primary progressive aphasia.

OBJECTIVE: The profile of language impairment in patients with primary progressive aphasia in comparison with the language impairment in patients with Alzheimer's disease and after stroke. DESIGN: The Western Aphasia Battery and the Mattis Dementia Rating Scale evaluated the language and cognitive impairment. Follow-up studies were done 1 to 5 years after the initial testing in seven of 10 patients with primary progressive aphasia, and a postmortem examination of the brain was done in two patients. SETTING: All 10 patients with primary progressive aphasia were previously healthy, community-d…

Language analysis skills of children with mental retardation.

The ability of children with moderate mental retardation to analyze orally presented sentences into words and words into syllables and phonemes was studied. The subjects, aged 10 to 15, were grouped by method of reading instruction. All of them could analyze spoken sentences into words and words into syllables equally well. However, subjects receiving code-emphasis reading instruction performed significantly better on the more difficult phoneme manipulation tasks than did subjects receiving whole-word instruction. Results suggest that code-emphasis reading instruction for some subjects with me

Gestalt imagery: A critical factor in language comprehension.

Gestalt imagery-the ability to create imaged wholes-is a critical factor in oral and written language comprehension. Despite good decoding, good vocabulary, and adequate background experiences, many individuals experience weak gestalt imagery, thus processing "parts" rather than "wholes," from verbal stimuli, spoken or written. This contributes to a Language Comprehension Disorder that may be accompanied by a commonality of symptoms: weak reading comprehension, weak oral language comprehension, weak oral language expression, weak written language expression, difficulty following directions, an…

Using sign language with tracheotomized infants and children.

Children's ability to communicate with others is important for cognitive development. However, the number of infants and young children deprived of vocalization (aphonia) is increasing in the pediatric patient population due to tracheotomy. The Communication Program for Infants and Parents (CPIP) uses sign language as an alternative system to verbal communication.
