Impact of orthotic therapy for improving activities of daily living in individuals with spinal cord injury: a retrospective cohort study.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To determine the impact of leg orthotic therapy for improving activities of daily living after spinal cord injury. SETTING: Participating acute care and rehabilitation hospitals across Japan. METHODS: We retrospectively identified individuals with spinal cord injury admitted to eight participating hospitals in 2015-2016 from the Japan Rehabilitation Database. Data for 293 individuals were analyzed. Propensity score analysis by inverse probability weighting (IPW) was applied to adjust for potential bias and create two comparable groups. Ou

Minimally invasive fasciotomy in chronic exertional compartment syndrome and fascial hernias of the anterior lower leg: short- and long-term results.

This study evaluated the efficacy of a minimally invasive percutaneous fasciotomy in chronic exertional compartment syndrome and fascial hernias of the anterior lower leg (N = 118). Approximately one-third of symptomatic legs (n = 41) demonstrated fascial herniation and underwent fasciotomy without compartment pressure measurements via a small percutaneous incision using a fasciotome. Symptomatic legs with an intact fascia (n = 77) received similar operative treatment based on pressure measurements. Relief of symptoms was obtained in all but two patients. Postoperative complications included h

Circumferential suicidal cutting of the lower legs.

A 69-year-old woman committed suicide by cutting her lower legs with a safety razor blade. Autopsy revealed horizontal incised wounds around the circumference of the lower legs, 24 cm above the heels, cutting superficial veins on both sides, the underlying lower legs muscles, and extending into both lower leg bones with associated hematoma formation. Also, there were a large number of superficial tentative or "hesitation" cuts ranging from 3 to 6 cm over the lower legs. Death was attributed to hemorrhagic shock following injuries to blood vessels of the lower legs.

The effect of automatic brake system on the response of the lower limbs based on active muscle force.

This article employs a finite element model integrated with the Hybrid III dummy to investigate how automatic braking and active muscle forces influence lower-limb injuries in frontal collisions. Prolonged braking can increase the tibial index, indicating more severe injury to the lower leg. Braking mitigated thigh injury at 50 km/h but exacerbated it at 40 km/h. Active muscle activation increased the femoral axial force and tibial index but decreased tibial and fibular peak stresses by approximately 0.006-0.009 GPa. These findings highlight the complex role of braking and muscle activation in

Simultaneous Bilateral Knee Dislocation During Weight Training: A Case Report and Review of the Literature.

CASE: We describe the clinical course and treatment of a patient who sustained simultaneous bilateral knee dislocation under low-velocity atraumatic conditions, and provide a review of the literature. Dislocations of the native knee joint are uncommon orthopaedic injuries but they are true emergencies because of the concern for concomitant neurovascular injury; therefore, they may be limb-threatening injuries. CONCLUSION: To our knowledge, there are few reports of simultaneous bilateral knee dislocation and no reports of this occurring during weight training. The risk of knee dislocation can

Soccer injuries and their mechanisms: a prospective study.

In order to study the incidence and mechanisms of injury in soccer and to recommend prophylactic measures, 180 players in a senior male socc … ost frequent (32%). Overuse injuries were most frequent in the preseason training period. Traumatic leg injuries involved players with inadequate or no shin guards. Of the traumatic knee injuries, 11 of 18 (61%) occurred during a collision; non-contact knee injuries were frequently seen in those players with a history of knee injury and existing instability. S …

Injury risk of playing football in Futsal World Cups.

BACKGROUND: While injuries in football (soccer) have been the subject of numerous publications, little information is available about the injury risk in futsal. OBJECTIVE: Analysis of the incidence and characteristics of injury in futsal players during top-level international tournaments. DESIGN: Prospective survey. METHODS: Player injuries during three consecutive Futsal World Cups were analysed using an established injury report system. The physicians of all participating teams reported all injuries after each match on a standardised injury report form. The average response rate was 93%.

Angle-specific torque profiles of concentric and eccentric thigh muscle strength 20 years after anterior cruciate ligament injury.

Thigh muscle weakness prevails following anterior cruciate ligament (ACL) injury, as usually evaluated by peak concentric quadriceps strength. Assessment throughout the range of motion (ROM), and for antagonists may provide more comprehensive information. We evaluated angle-specific torque profiles and ratios of isokinetic thigh muscle strength in 70 individuals 23 ± 2 years post-ACL injury (44males, 46.9 ± 5.4 years); 33 treated with ACL-reconstruction (ACL-R), and 37 treated only with physiotherapy (ACL-PT), and 33 controls. Quadriceps and hamstrings torques for concentric/eccentric contract

"Run-and-jump failure": new injury patterns in indoor bouldering.

In recent years, professional and non-professional bouldering have seen a fundamental change in movement patterns towards complex movement sequences. This is associated with increased demands on the musculoskeletal system, especially the lower extremities, which can lead to new injury patterns. In the course of our clinical work, we have already seen an increasing number of lower extremity injuries after run-and-jump sequences, with a severe one being highlighted in this case report.

Penetrating nontorso trauma: the extremities.

Similar to penetrating torso trauma, nontorso injuries have undergone a fascinating oscillation between invasive and noninvasive approaches. This article discusses an organized approach to the evaluation and initial treatment of penetrating extremity injuries based on regional anatomy and clinical examination. The approach is reliable, efficient and minimizes both delays in diagnosis and missed injuries. Outpatient follow- up is particularly important for patients with extremity injuries who are discharged home from the emergency department.

Medical interventions in the management of hamstring muscle injury.

Acute muscle belly injuries to the semitendinosus, semimembranosus and biceps femoris (the 'hamstring' muscles) remain a common problem in the sporting population. Physiotherapy-led rehabilitation remains the mainstay of treatment, and the physician's input is often minimal. Anecdotally, many different topical, oral and injectable therapies are used around the world in an effort to accelerate the healing of these injuries and to prevent their recurrence. This article reviews the evidence available to support some of the most commonly used medical therapies and the pathophysiological basis for
