Awareness of Care Managers Concerning Grief Care for Older Bereaved Individuals Living Alone Following the Loss of Their Spouse: A Qualitative Research.

Background/Objectives: Japan has the most rapidly aging population worldwide, with older adults expected to reach 38.7% by 2070. Furthermore, individuals ≥ 60 years old desire to spend their final days at home. Following the establishment of the Long-Term Care Insurance System in 2000, care managers at home care support facilities continually assist most older adults receiving home care. Moreover, home-based end-of-life care is expected to increase. Therefore, the care manager's role will be crucial in providing end-of-life care support and offering grief care to bereaved families following a

Experiences of mothers of long-term surviving patients with cerebral adrenoleukodystrophy: a qualitative study.

BACKGROUND: Adrenoleukodystrophy (ALD) is an X-linked peroxisomal disorder. Its cerebral form presents as a learning and behavioral disorder that, if untreated, leads to rapid neurological regression, disability, and death within 10 years of diagnosis. Therefore, the disease significantly impacts patients' quality of life, making quality of life assessment crucial for effective medical treatment and care. However, no disease-specific quality of life scale exists for ALD. Therefore, we conducted qualitative research to determine the experiences of patients and their families as a preliminary st

Future care planning.

Future care planning (FCP) is increasingly discussed within supportive and palliative care policy contexts in the UK and internationally. This article incorporates a scoping review of the literature to provide a summary overview. FCP is discussed as a policy element improving end-of-life care by encompassing advance care planning and furthermore including individuals with diminished decision-making capacity through a best-interests approach. The evidence base further indicates that FCP is being increasingly applied in palliative care settings worldwide and has been adopted as part of several n

"People don't understand what goes on in here": A consensual qualitative research analysis of inmate-caregiver perspectives on prison-based end-of-life care.

BACKGROUND: The age demographic of the incarcerated is quickly shifting from young to old. Correctional facilities are responsible for navigating inmate access to healthcare; currently, there is no standardization for access to end-of-life care. There is growing research support for prison-based end-of-life care programs that incorporate inmate peer caregivers as a way to meet the needs of the elderly and dying who are incarcerated. AIM: This project aims to (a) describe a prison-based end-of-life program utilizing inmate peer caregivers, (b) identify inmate-caregiver motivations for particip

ICU nurses' perceptions of potential constraints and anticipated support to practice defibrillation: a qualitative study.

AIM: The study examines the experience of intensive care nurses in caring for patients in cardiac arrest, and their perceptions of introducing nurse-led defibrillation. METHOD: This was a descriptive, exploratory and qualitative study at an intensive care unit (ICU) of an acute regional hospital in Hong Kong. Twelve registered nurses were purposefully selected for interview. RESULTS: Although all the participants were trained in basic life support, only 50% were trained in advanced cardiac life support (ACLS), and those trained in ACLS described having limited opportunities to apply their de

Introduction of novice oncology nurses to end-of-life care.

Dying patients and their families often have unique physical, psychosocial, social, and spiritual needs that require special ized end-of-life (EOL) skills. EOL preparation of nurses has been inconsistent. Novice nurses need guidance to develop the knowledge, clinical skills, and coping strategies to provide high-quality and compassionate EOL care. Inadequate preparation for providing EOL care can lead to anxiety, stress, and burnout. Barriers that prevent adequate preparation and support of novice nurses in EOL care include lack of education, financial constraints, poor staffing, and major lif

Should noninvasive ventilation be used with the do-not-intubate patient?

Most of the large quantity of data on noninvasive ventilation (NIV) in acute respiratory failure is from patients who want all possible treatments and life-support. Few data are available on NIV in patients who have elected specific limits on life support and treatments (eg, patients with do-not-intubate [DNI] orders) and patients who are near the end of life and will receive comfort measures only (CMO). The most critical issue regarding NIV in DNI and CMO patients is informed consent. The patient must be informed of the risks and potential benefits of NIV, and must consent to NIV. We have few

Advance Care Planning as Key to Person-Centered Care: A Co-Design Study Involving Adults with Chronic Kidney Disease, Family Members, and Healthcare Professionals.

INTRODUCTION: Advance care planning is a process that supports adults at any age or stage of health in understanding and sharing their personal values, life goals, and preferences regarding future medical care. Advance care planning is recommended for adults with chronic kidney disease and is an approach to facilitating person-centered care. The approach is useful at all stages of the disease and not solely related to end of life or serious disease events but also concerns management of physical, emotional, and psychological challenges throughout the illness trajectory. Living with chronic kid

Describing the elements of an effective dementia palliative care service.

BACKGROUND: As dementia is a life-limiting illness, it is now widely accepted that people with dementia benefit from palliative care. The core components of palliative care for people with dementia have been suggested, however little is known about what an effective dementia palliative care service looks like in practice. While some services exist, a lack of description and scant detail on how and why they work makes it difficult for others to learn from existing successful models and impedes replication. Accordingly, we set out to describe an effective dementia palliative care service using p

Self-Care Processes as Predictors of Quality of Life in Adults With Moyamoya Disease: A Structural Equation Modeling Study.

Background: Moyamoya disease is a chronic cerebrovascular condition requiring ongoing self-management to prevent complications. Riegel's theory of self-care in chronic illness posits that factors such as disease knowledge, self-efficacy, social support, and access to care influence self-care maintenance, monitoring, and management, which in turn affect health outcomes and quality of life (QoL). Aim: To test a structural equation model of factors influencing self-care and outcomes in adults with moyamoya disease based on Riegel's self-care theory. Methods: A cross-sectional survey of 211 adults

Ensuring Optimal Outcomes for Preterm Infants after NICU Discharge: A Life Course Health Development Approach to High-Risk Infant Follow-Up.

Children born prematurely (<37 weeks' gestation) have an increased risk for chronic health problems and developmental challenges compared to their term-born peers. The threats to health and development posed by prematurity, the unintended effects of life-sustaining neonatal intensive care, the associated neonatal morbidities, and the profound stressors to families affect well-being during infancy, childhood, adolescence, and beyond. Specialized clinical programs provide medical and developmental follow-up care for preterm infants after hospital discharge. High-risk infant follow-up, like most

From hope and expectation to unexpected death after cardiac surgery.

OBJECTIVE: Relatives of patients undergoing cardiac surgery expect successful outcome but sometimes complications cause death. The aim was to interview relatives of patients who have died in connection with cardiac surgery and describe their experiences of information, reception and care. METHODS: Data were obtained from semi-structured interviews with 18 relatives of deceased patients and then analysed using qualitative content analysis. RESULTS: Two main groups emerged: "Analysing the situation" with the sub-groups: knowledge of cardiac disease, the road to operation, hope and despair, inf

Beyond the Bedside: Addressing Structural Gaps in the Care of Undocumented, Seriously Ill Patients.

Undocumented patients face structural constraints to accessing life-sustaining treatments in the subacute setting due to gaps in health care coverage. We present the case of a 38-year-old Spanish-speaking woman with short bowel syndrome, dependent on prolonged parenteral nutrition after critical illness, who faced additional challenges as an undocumented and uninsured patient. Despite medical stabilization and a consistent desire to pursue life-prolonging care, discharge planning was complicated by a health care system not designed to support individuals without legal status or insurance.
