Bellmunt risk score enables survival prediction in men with metastatic castration resistant prostate cancer (mCRPC) undergoing PSMA-targeted radioligand therapy (LUMEN).

PURPOSE: [177Lu]Lu-PSMA-617 therapy has emerged as a promising radioligand treatment for metastatic castration-resistant prostate cancer (mCRPC). Given the overall poor prognosis of mCRPC, accurately predicting the key endpoint overall survival (OS) remains an ongoing challenge, crucial for optimizing risk/benefit assessment prior to treatment. This study investigates the prognostic utility of the simple designed Bellmunt Risk Score (BRS) in mCRPC patients treated with [177Lu]Lu-PSMA-617. METHODS: We retrospectively evaluated data from 386 mCRPC patients who had received [177Lu]Lu-PSMA-617 th

Outcomes for [177Lu]Lu-PSMA-617 with and Without Concurrent Use of Androgen Receptor Pathway Inhibitors in Patients with Metastatic Castration-resistant Prostate Cancer.

BACKGROUND AND OBJECTIVE: Large clinical trials, including VISION, have established the safety of combining [177Lu]Lu-PSMA-617 with an androgen receptor pathway inhibitor (ARPI). Most patients receiving [177Lu]Lu-PSMA-617 in the real-world setting have already progressed on an ARPI. This study aimed to assess the efficacy of [177Lu]Lu-PSMA-617 in patients with and without the concurrent use of an ARPI. METHODS: We analyzed data from the Mayo Clinic Rochester radiopharmaceutical database, focusing on patients with metastatic castration-resistant prostate cancer (mCRPC) who had progressed after

PSMA Expression Heterogeneity in Metastatic Prostate Carcinoma: A Case of Discordant Liver Metastases on 68 Ga PSMA PET/CT Showing Uptake on Post-Lu-177 PSMA Therapy Scan.

We report a case of metastatic castration-resistant prostate carcinoma with discordant PSMA (prostate specific membrane antigen) expression- 68 Ga-PSMA PET/CT (positron emission tomography/computed tomography) showed PSMA-avid skeletal metastases but nonavid liver lesions. The patient received 177 Lu-PSMA-617 on compassionate grounds. Post-therapy imaging revealed radiotracer uptake in both skeletal and previously nonavid hepatic metastases, suggesting possible increased lesion amenability. This case highlights the dynamic nature of PSMA expression and the potential role of post-therapy scans

Pretherapeutic estimated glomerular filtration rate predicts development of chronic kidney disease in patients receiving PSMA-targeted radioligand therapy.

BACKGROUND: Prostate-specific membrane antigen (PSMA)-targeted radioligand therapy (RLT) may be associated with renal toxicity. We aimed to identify predictive parameters for the development of chronic kidney disease (CKD) in patients with metastatic castration resistant prostate cancer (mCRPC) undergoing RLT. METHODS: In 46 mCRPC patients scheduled for Lu-177-PSMA-RLT, pretherapeutic estimated glomerular filtration rate (eGFR [ml/min/1.73 m2 ]), Tc-99m-mercaptoacetyltriglycine (Tc-99m-MAG3) clearance and baseline Ga-68-PSMA-ligand positron emission tomography (PET)-derived renal cortical upt

Evaluation of [131I]I- and [177Lu]Lu-DTPA-A11 Minibody for Radioimmunotherapy in a Preclinical Model of PSCA-Expressing Prostate Cancer.

PURPOSE: Radioimmunotherapy uses tumor-specific antibodies to deliver therapeutic radionuclides, but hematological toxicity due to the long serum half-life of intact antibodies remains a challenge. We evaluated a smaller antibody fragment, the minibody, with faster kinetics and a potentially improved therapeutic index. PROCEDURES: The anti-prostate stem cell antigen (PSCA) minibody (A11 Mb) was radiolabeled with iodine-124 ([124I]I-A11 Mb) or conjugated with deferoxamine (DFO) and labeled with zirconium-89 ([89Zr]Zr-DFO-A11 Mb) for surrogate immunoPET to profile pharmacokinetics in a human pr

S-values for bone marrow dosimetry in preclinical radiopharmaceutical studies with rodents.

BACKGROUND: Development of novel radiopharmaceuticals involves dosimetry calculations to validate safety and aid with selection of those that should be translated into the clinical environment. Dosimetry is critical for limiting radiation damage to organs at risk. The bone marrow is a limiting organ in radiopharmaceutical therapies (RPTs) for metastatic prostate cancer, for example, but there is room for improvement of bone marrow dosimetry in preclinical studies. Bone marrow S-values for Lutetium-177 (177Lu) in rodents have been published but they have not included tumor xenografts inoculated

[177Lu]Lu-DOTATATE Molecular Radiotherapy During Induction Chemotherapy for First-Line Stage 4 High-Risk Neuroblastoma in South African Children: Protocol for a Phase 1 Randomized Controlled Trial (LuDO-SA Trial).

BACKGROUND: In South Africa, the metastatic response rate for stage 4 high-risk neuroblastoma (HR-NB) to induction chemotherapy is lower than that of similar regimens in well-resourced settings. Augmenting current induction chemotherapy regimens with molecular radiotherapy, without increasing toxicity, is an attractive systemic treatment in radioisotope-avid tumors, especially in a resource-limited setting where stem cell transplantation and immunotherapy are unavailable and costly. Clinical trials of [177Lu]Lutetium DOTATATE ([177Lu]Lu-DOTATATE) have primarily focused on refractory or relapse

Clinical Impact of Changes in Tumor Uptake and Volume on PSMA PET/CT During [177Lu]Lu-PSMA Therapy in Metastatic Castration-Resistant Prostate Cancer.

Although tumor volume and new lesions (NLs) have been investigated previously as measures of response, the clinical impact of changes in tumor uptake on prostate-specific membrane antigen (PSMA) PET remains largely unknown. Methods: This multicenter retrospective study investigated the clinical impact of changes in tumor uptake and volume on PSMA PET during [177Lu]Lu-PSMA in metastatic castration-resistant prostate cancer (mCRPC). The primary outcomes were the associations of changes in SUVmax (ΔSUVmax) and SUVmean (ΔSUVmean), changes in total tumor volume (ΔTTV), and occurrence of NLs with pr

Leveraging [18F]F-Piflufolastat PET for PBPK Prediction of Tissue Distribution and Therapy Optimization in Prostate Cancer.

Prostate-specific membrane antigen (PSMA) is overexpressed in over 90% of prostate cancer tumors, making it a strong target for imaging and therapy. The diagnostic tracer [18F]F-Piflufolastat provides high affinity and image quality, while the β-emitter [177Lu]Lu-PSMA-617 enables targeted treatment. Despite differences in radioactive half-life, their shared PSMA-binding and pharmacokinetic profiles suggest potential for translational pharmacokinetic modeling. We hypothesized that the published full-body physiologically based pharmacokinetic (PBPK) model could be directly translated across the

Assessing PSMA Receptor Availability Using 68Ga-PSMA-11 PET/CT Following 177Lu-PSMA-617 Therapy Administration.

PURPOSE: Prostate cancer treatment with 177Lu-PSMA-617 depends on specific binding to the PSMA protein. If receptors become saturated by the radioligand, the uptake of the drug may be affected. Our goal was to measure the pharmacological effect of standard 7.4 GBq (200 mCi) administration of 177Lu-PSMA-617 based on the uptake of 68Ga-PSMA-11 on subsequent PET. PATIENTS AND METHODS: We performed 68Ga-PSMA-11 PET/CT within 0.5-75 hours after cycle 1 of therapy administration in 6 patients with metastatic castrate-resistant prostate cancer who volunteered for imaging. SUVmean of tumors (N=27) an

Cutaneous, Subcutaneous, and Bilateral Adrenal Gland Metastases in Progressive Prostate Carcinoma: Theranostic Potential of 68 Ga-PSMA-11 PET/CT Imaging and 177 Lu-PSMA-617 Therapy.

Prostate carcinoma (PC) is the second most common malignant tumor in males globally. The metastatic spread of PC usually involves the pelvic and abdominal lymph nodes and the skeletal system. Cutaneous metastases are exceedingly uncommon and typically manifest themselves late in the disease course, considered as ominous sign with limited treatment options and a poor prognosis. We describe a patient wherein 68 Ga-PSMA-11 PET/CT detected multiple uncommon metastatic sites in the cutaneous region of the scrotum, penis, and thigh, as well as in the subcutaneous region of anterior abdominal wall, a

Predictive factors of tumor sink effect: Insights from 177Lu-Prostate-specific membrane antigen therapy.

OBJECTIVE: Tumor sink effect (TSE) has been defined as; decreased uptake in healthy tissue with increased tumor sequestration of the radiopharmaceuticals. It enables us to give high tumoral radiation doses while resulting in lower absorbed radiation to critical organs. However, the factors which influence this effect are yet to be defined. In this study, we have investigated the predictive factors of the tumor sink effect in a group of patients who received 177Lu-Prostate-specific membrane antigen (PSMA) therapy due to progressive metastatic castration-resistant prostate cancer (mCRPC).
