Magnesium metabolism in health and disease.

Magnesium is an important element for health and disease. Magnesium, the second most abundant intracellular cation, has been identified as a cofactor in over 300 enzymatic reactions involving energy metabolism and protein and nucleic acid synthesis. Approximately half of the total magnesium in the body is present in soft tissue, and the other half in bone. Less than 1% of the total body magnesium is present in blood. Nonetheless, the majority of our experimental information comes from determination of magnesium in serum and red blood cells. At present, we have little information about equilibr…

Recent data on magnesium and osteoporosis.

Larger and more perfect bone mineral crystals and decreased bone magnesium concentration were found in postmenopausal osteoporosis, senile osteoporosis, alcoholic osteoporosis and osteoporosis associated with thalassaemia. The decreased bone magnesium concentration and the increased retention of magnesium in the magnesium load test suggest magnesium deficiency in post-menoposal osteoporosis, probably caused by magnesium malabsorption.

Effects of drugs on magnesium requirements.

A number of drugs can influence Mg metabolism and produce changes in serum Mg concentrations. These alterations must always be considered in the differential diagnosis of cases with Mg disturbances. Hypermagnesemia is seen, particularly after administration of Mg-containing drugs in renal insufficiency. Marked hypomagnesemia can be observed in a great number of patients, particularly in those treated with loop diuretics, osmotic substances and with antineoplastics, such as cisplatin.

Type A behavior and magnesium metabolism.

An analysis of the literature suggests the possible role of Mg deficiency in the susceptibility to cardiovascular diseases, observed among subjects displaying a type A behavior pattern. Experimental data which support this hypothesis are reviewed. Type A subjects are more sensitive to stress and produce more catecholamines than type B subjects. This, in turn, seems to induce an intracellular Mg loss. In the long run, type A individuals would develop a state of Mg deficiency, which may promote a greater sensitivity to stress and, ultimately, lead to the development of cardiovascular problems. T

Magnesium deficiency in the Japanese quail.

Morphological effects of magnesium deficiency on liver cells and general aspects of its influence on the metabolism were investigated in young quails. Magnesium deficiency was characterized by a depressed growth, a high mortality rate, a decrease in hematocrit and magnesium and calcium plasma concentrations. Magnesium deficiency reduced the magnesium concentration in heart by 44%, but did not affect the concentration in liver. Ultrastructural aspect of liver parenchymal cells revealed that the number of mitochondria per cell section was decreased and the average area of a mitochondrion was gre…

Prophylaxis and treatment of magnesium depletion.

Magnesium deficiency may develop during diuretic treatment, causing serious cardiac arrhythmias hitherto ascribed to potassium depletion. Intravenous magnesium replacement therapy replenishes intracellular magnesium and potassium and inhibits cardiac arrhythmias which may occur during diuretic treatment; oral magnesium supplementation is also effective. The co-administration of amiloride with a distal-tubule diuretic prevents increased renal excretion of magnesium. Doses of diuretics should be restricted to the minimum compatible with the achievement of clinical objectives.

Magnesium deficiency in children with urolithiasis.

In a group of 57 children with urolithiasis hypomagnesaemia was found in 15 cases (26.3%). All children but one with abnormally low serum magnesium levels had recurrent or bilateral nephrolithiasis or nephrocalcinosis. Prevalence of hyperoxaluria and hypercalciuria, marked severity of the clinical features, abnormality of Ca metabolism and its responsiveness to MgO treatment were demonstrable in Mg deficiency.

The skin in magnesium-deficient rats.

The cutaneous vasodilatation occuring in the early stages of dietary deficiency of magnesium has been investigated in rats. While the time of onset of erythema varies in proportion to the weight of the animal, the duration is not related to weight. In severe states of vasodilatation, the skin is thickened and infiltrated with mononuclear cells, apparently derived from the blood. Intact sympathetic and sensory innervation are not necessary for the development of vasodilatation in the skin. Neither can the genesis of the erythema be attributed to degranulation of mast cells. From consideration o…

Effect of ethanol on magnesium excretion.

The effect of ethanol on magnesium excretion was studied in three normal subjects. It was found that the ingestion of 2 ml ethanol/kg body weight produced a marked immediate increase in urinary magnesium excretion, but there was no significant effect on overall magnesium balance when this amount was taken daily for eight days.
